I give my student permission to travel (to and from), and participate in all activities conducted at the North Caldwell Town Pool on Thursday, June 14, 2012.

I ______(Student Name), hereby agrees to the following list of conditions while I am participating in the West Essex Senior Luau/Pool Party. CONDITIONS

 I will engage only in behavior that is appropriate and acceptable as established, outlined, and reviewed by school chaperones. I understand the use, possession, and sale of alcohol and/or illegal substances is prohibited.

 I will conduct myself as a cooperative and mature senior, demonstrate self-respect, respect for my fellow seniors, respect for the chaperones, respect for the other North Caldwell Pool attendees, and otherwise abide by the rules and regulations established by the school.

 I will arrange transportation and arrive safely and promptly 20 MINUTES AFTER THE COMPLETION OF GRADUATION PRACTICE on Thursday morning. I understand that entrance into the Pool will be denied if I do not arrive in due time.

 All parties must sign and return this form to the Senior Class Advisor, or he/she will not be permitted to go to the Senior Luau.

SHOULD ANY OF THESE CONDITIONS BE VIOLATED, I UNDERSTAND THAT MY PARENT/GUARDIAN WILL BE NOTIFIED IMMEDIATELY BY TELEPHONE. IF HE/SHE CANNOT BE REACHED, THE LOCAL POLICE WILL BE NOTIFIED. I WILL BE SUBJECT TO ANY AND ALL DISCIPLINARY ACTIONS TAKEN BY SCHOOL AUTHORITIES, WHICH MAY INCLUDE A DISCONTINUANCE OF THE TRIP’S ACTIVITIES, AND THAT I WILL FORFEIT THE PRIVILEGES OF CONTINUED PARTICIPATION IN GRADUATION.
