Dan Cohen founded Music &Memory in 2008 to bring personalized music to residents of long-term care communities by utilizing iPods. His initial idea was simple- if he ended up in a nursing home, he wanted to be able to listen to his favorite ‘60s music. His goal was to assist residents in accessing their favorite music in order to improve their quality of life. Quickly the benefits were realized and since that time, the Music & Memorysm program has proven to be an exemplary way for those suffering from dementia, mental illness, or simply boredom to access the transformative, therapeutic benefits of personalized music. The program has spread throughout the country and is now utilized internationally. The Illinois Pioneer Coalition (IPC) is the leading organization seeking to improve the quality of life for residents of long-term care facilities by promoting culture change and person-centered care. In step with this mission, the IPC has obtained funding to award grants to 25 organizations to become Music and Memorysm Certified.

Who Can Apply? Applications for the Music & Memorysm Certification Grant are available to groups serving the senior population and/or those with mental illnesses or disabilities in a group setting, such as those who reside or participate in:  Skilled Nursing Homes

Requirements. To participate in the IPC Music & Memorysm Certification program you must… . be a member, or become a member, of the Illinois Pioneer Coalition and regularly attend regional meetings to share your personalized music program stories with other participants. (Rates: $50 Individual/ $100 Community. Members receive a reduced rate for annual Summit) . provide the needed equipment for facilitation of the project. The grant assists with the cost of training (webinars) and support from Music & Memorysm project. . Provide appropriate staff to complete training and administer and evaluate the program consistently . Nursing homes will complete a participation agreement to join Telligen’s Nursing Home Collaborative . implement and actively administer the program within your community for a minimum of 1 year. . Include program within your written QAPI Plan
