All individuals wanting to attend the Gaithersburg Homecoming Dance who are not Gaithersburg students must see that this form is completed and returned to the main office at Gaithersburg High School before you can purchase a ticket.

Expectations: 1. Guest must conform to all expectations in the Gaithersburg High School Code of Conduct. 2. Guest may NOT be middle school students. 3. Guest must present a photo ID to enter the dance. 4. This application must be completed and approved before tickets will be sold. 5. Guest must conform to the dress and decorum standards. 6. Bring the completed form to the main office at Gaithersburg High School before you purchase your ticket. No forms will be accepted later than Thursday, October 25, 2012.

I am asking that you complete this form affirming that this individual is/was a student in good standing and who exhibited good citizenship at your school. Your signature below will serve as verification. Please include your contact phone number as well as I plan to confirm each form which we receive. Please DO NOT sign if you cannot attest to this person’s credibility.

Make a copy of the guest’s identification (driver’s license, passport, etc.) and attach to this sheet.
