You must have the following on file in the Activities 1. Eligibility requirements Office to receive your eligibility card. You must have the card BEFORE beginning practice.

Online registration must be completed and the activity fee paid or a paperwork packet with the following requirements must be completed and the activity fee paid. 1. A signed 2014-2015 Dassel-Cokato Schools Participation Policy #564 (7-12) (Required for all activities) 2. 2014-2015 MSHSL Eligibility Brochure (7-12) -Signed MSHSL Annual Sports Health Questionnaire (Athletic requirement) -Signed 2014-2015 MSHSL Eligibility Statement (Athletic and Fine Arts requirement) 3. Current physical (Athletic requirement) 4. Concussion Test (May be required) Tests are good for two years. (Athletic requirement) 5. Completed Emergency Medical Card with Ins. Info. (Required for all activities) 6. Completed Student Activity Card (Required for all activities) 7. Paid activity fee (Required for all activities) 2. Academic Requirements You must be receiving passing grades in all classes. A student must attend all classes during the school 3. School attendance day to be eligible to practice and/or participate in a scheduled contest on that same day. Athletics & Fine Arts Grades 7 and 8 fees - $60.00 per activity 4. Activity fees Grades 9-12 fees - $80.00 per activity Individual cap per year: $160.00 Available at the Ridgeview Medical Center in Waconia. Adult annual pass = $45.00 Student annual pass = $25.00 Senior Citizen pass = FREE (residents of ISD 466 and over 62) Volunteer sign-up available. Parents volunteering at six (6) or more scheduled events will receive an annual pass or the Activities 9. Volunteer sign-up Office will waive one student activity fee.
