Locate the Block ID Request Form on the SBC CLEC Web Site Complete Block ID Request Form and submit Block ID Request Form
Section I: Locate the Block ID Request Form
Introduction The Block ID Request Form is used to establish the CLEC identity to the SBC Information Security Department.
Following the successful completion and submission of the CLEC Block ID Form, the CLEC will receive new Security Identification enabling the CLEC to connect to the E9-1-1 database in order to send and retrieve E9- 1-1 data.

NOTE: All companies that send or receive data from the SBC DBMS must have a NENA (National Emergency Number Association) Identifier: A national Company ID in E9-1-1 service allows the PSAP to identify the switching carrier for the caller, and to determine the 24 x 7 number of the Company for emergency contact needs. A NENA ID Supports tracking, completeness, and accuracy in 9-1-1 data record processing by both the 9-1-1 service providing Company and the data record source.
