A*STAR Research Attachment Programme Nomination Form Instructions: 1. The nomination form is to be completed and submitted to A*GA. 2. Please ensure that Parts A, B and C are completed. 3. The following documents are to be attached to the nomination form: - Student’s supporting documents, which include a. Curriculum Vitae (CV) b. Copy of the Identity Card or Passport c. Copies of university results/transcript(s) – Results/transcripts should show that at least 2nd Class (Upper) Honours or equivalent is met – If the original results/transcripts are not in English, please also submit a copy of the translated and authorized results/transcripts, i.e. signed or stamped, by an official translation service or by your school/college/university.

Funding Terms:

The Award provides support for living in Singapore, consisting of:  a monthly stipend of S$2,500 for the period that the student is based at the A*STAR RI, for a minimum period of 1 year, up to a maximum period of 2 years;  a one-time settling-in allowance of S$1,000 provided the student spends at least a 1 year at the A*STAR RI;  a one-time airfare grant of S$1,500; and  medical insurance.

I hereby agree to the following responsibilities:

1. The Partner University Supervisor is required to visit the host A*STAR RI on at least one (1) occasion per year when the Student is based at A*STAR RI, for a total period not exceeding two (2) weeks per year in order to maintain an adequate level of direct contact with the Student. Similarly, the A*STAR Co-Supervisor is required to visit the Student at least one (1) occasion per year for a total period not exceeding two (2) weeks per year when the Student is based at the University.

2. A*STAR RI/Consortium shall bear the expenses of its own A*STAR Co-Supervisors for the travel, accommodation and subsistence for the purposes of supervising the respective Student when the Student is based at the Partner University.

3. The Partner University Supervisor and A*STAR Co-Supervisor should scope the project so that it would require at least 1 year of attachment at A*STAR RI to complete.

2. Expenses of its own A*STAR Co-Supervisors for the travel, accommodation and subsistence for the purposes of supervising the respective Student when the Student is based at the Partner University. The A*STAR Co-supervisor is normally expected to visit the student at least once a year.

By submitting this form, RI would have obtained the student’s consent for the following:

The student agrees that A*STAR and its related entities, including its subsidiaries and research institutes (collectively, the “A*STAR Group”), may collect, use and disclose the personal data that he/she has submitted for the intended purposes of: managing all matters pertaining to his/her scholarship/Award and studies; inviting him/her to participate in future studies/surveys; contacting him/her to provide information on scholarships, outreach/scholarship events and A*STAR developments; tracking and studying the demographics of our scholarship applicants/recipients; and for policy review and formulation.

The A*STAR Group may also back up or archive his/her personal data in secure, centralized IT systems, and disclose his/her personal data where necessary when collaborating with third party businesses or organisations, engaging third party service providers, for business or administrative purposes, and for any investigation or proceeding. The A*STAR Group may also disclose his/her personal data to Ministries, Statutory Boards and other Singapore Public Agencies. As some third parties may be located outside of Singapore, his/her personal data may be transferred out of Singapore from time to time.
