EIV Identity Verification – Failed EIV SSA Identity Test Reports

The Failed EIV SSA Identity Test Reports can be accessed from the Identity Verification Report link. It is important to select the recertification month “ALL” when running the report. This report must be used monthly to clear up any invalid, discrepant or missing information in the TRACS database that was not identified and corrected at the time of recertification.

Any discrepant information must be corrected in the TRACS database. If it is determined that the information in TRACS differs from the information found on the tenant’s current HUD-50059, retransmit the current HUD-50059 to correct the TRACS database.

There will not be any employment or income information in EIV for tenants who fail the Failed EIV SSA Identity Test Reports. So, it is essential that any discrepancies are corrected within 30 days from the date of the report.

Within the EIV System, this report can be accessed from the Identity Verification Report link. Select the recertification month “ALL” and if the report name is underlined, this indicates it is an active link. To obtain the data about the household members who failed the Failed EIV SSA Identity Test Reports, just click on the report name to obtain.

EIV Identity Verification – Failed EIV SSA Identity Test Reports

 Identifies tenants for the selected contract(s)/project(s) that have had their personal identifiers (SSN, last name, date of birth) sent to SSA, but data could not be verified by SSA due to missing or invalid information.  Must be reviewed and acted upon monthly (within 30 days of the report date).

Income data on Tenants Failing EIV SSA Identity Test

 Income data is not available for tenants who failed the EIV SSA Identity test and are listed on the report,  Income data will not be reflected until EIV SSA Identity test is passed.

Correcting Failed Data

1. Before contacting the tenant, confirm accuracy of data entry in TRACS, (e.g. has a number been transposed when entering the SSN? Birth date? Names spelled correctly?). 2. Contact the tenant to confirm their SSN, DOB and/or last name data that needs to be corrected. 3. Obtain the documentation from the tenant to verify any discrepancy in their “personal identifiers”. 4. Once failed data is identified and corrected, management will process a HUD-50059 then transmit through TRACS so that the tenant will be included in the match against SSA and NDNH data. 5. Print and retain a copy of the report in a master file called “Failed SSA Identity Test Reports”. The report will be documented with action taken to resolve invalid or discrepant personal identifiers.

Encourage the tenant to contact the SSA to correct any inaccurate data in their databases if the personal identifiers on the form HUD-50059 and in TRACS are accurate. If SSA records are wrong, only the tenant can request SSA to correct his/her record by completing and submitting formSS-5, Application for a Social Security Card."

Management can assist the tenant in requesting benefit information from SSA, if the tenant requests their assistance in accessing the SSA website or has questions on completing the request.

 Select “If you get Social Security benefits” or “If you get “Supplemental Security Income (SSI) benefits  Select “Request a Proof of Income Letter”

IMPORTANT: Tenants must check the box “All Benefit Information Available” to make sure all benefits received are provided.

2. To request a Proof of Income Letter from SSA’s toll-free number call 1-800-772- 1213.

a. This information is free and the tenant should receive the letter in the mail within 10 days. b. The tenant will provide the Proof of Income Letter to be used in calculating their income. c. A copy of the letter will be retained in the tenant’s file. d. The original returned to the tenant for their records.

The tenant can request SSA to correct his/her record by completing and submitting form SS-5, Application for a Social Security Card, to the local SSA office.

NOTE: If a tenant’s information was corrected at the time of recertification but the EIV data has not yet been updated, this must be noted on the printed report and no further action is required.
