CURRICULUM VITAE Lynette Ann Denny SEPTEMBER 2016

I am a gynaecologic oncologist and have been working in the field of cervical cancer prevention in collaboration with colleagues from Columbia University, New York since 1995. We began our collaboration with the intention of evaluating alternative strategies for the prevention of cervical cancer in low resource settings. Cervical cancer was and remains the commonest cancer diagnosed among women living in poor countries due to the failure to either initiate or sustain cytology based screening programmes. Our group pioneered the search for alternative protocols for the prevention of cervical cancer in community-based research sites in townships just outside of Cape Town. Since that time I have had extensive research experience in cervical cancer prevention, including three cross-sectional studies comparing different screening tests (such as HPV testing and VIA to cytology), a prophylactic HPV vaccine trial in HIV positive women and a therapeutic vaccine trial in HPV negative women. We are currently engaged in a NCI funded trial evaluating screen and treat with the HPV Xpert Cepheid HPV test. I am still fully functional as a clinician in all aspects of gynaecology oncology and have recently been appointed the director of the SA Medical Research Council Gynaecological Cancer Research Centre, with 5 years of guaranteed funding.

Khayelitsha Cervical Cancer Screening Project (KCCSP)

Young Women’s Study

This study recruited 3000 HIV negative women aged 18 – 34 years of age. In this cross-sectional study we performed cytology and HPV DNA testing in order to evaluate a] the prevalence of high-risk HPV infection in young women (we already have these data on women aged 35 – 65 years) in this community and b] the types of HPV among HPV positive women.

HIV intervention sub-study (HISS): Inclusion of voluntary HIV counselling and testing services as part of a cervical cancer screening project

This sub study was designed to evaluate the impact of intensive one-on-one counselling on the uptake of HIV testing and on behavioural changes in women recruited to the study.

Other aspects of cervical cancer screening being studied in the Khayelitsha project include:

 Evaluation of the barriers to screening and the factors that facilitate compliance with a screening programme  Referral patterns of women enrolled in a screening programme  Horizontal versus vertical integration of cervical screening programmes at primary care level  A cost-benefit analysis has been conducted, using mathematical modelling and assumptions based on the performance characteristics of the four different screening tests. This is being performed by Dr Susan Goldie, Harvard School of Public Health, Boston, USA in

collaboration with the principal investigators. The manuscript of this work is published (see references)  Hidden costs of Screening e.g. costs associated with tracking defaulters, treating associated co-morbid conditions. This work is in progress and is being conducted through Dr Goldie’s research fellow who has spent time (August/September 2004) embedded in the project.  Comparison of conventional versus liquid-based cytology has been incorporated as into the study design of Phase 3 of the Khayelitsha study and is now published (see references)  HPV DNA typing by PCR (polymerase chain reaction) of all HPV DNA positive women identified in all three phases of the study (see references).

Evaluation of the AMPLICOR Human Papilloma Virus (HPV) Test for the Detection of High Risk Human Papillomavirus in Cervical Samples collected in Preservcyt solution and Surepath Media Principal Investigator: Professor Lynette Denny Co- investigators: Dr Rosalind Boa, Ms Ayesha Sasman (University of Cape Town) This study was funded by Roche Molecular Systems and the Cape Town site was asked to contribute 600 cases to a trial involving an additional 21 000 women recruited from sites in the USA. The data collected from this trial was used for an FDA application for the registration of the AMPLICOR test for HPV DNA testing in the USA. The study recruited 600 women attending the Groote Schuur Hospital colposcopy clinic with abnormal cervical cytology. All women who gave consent had a specimen taken from the cervix for later testing with AMPLICOR as well as routine colposcopy and histology. The women will be followed for a period of 7 years thereafter. The study is complete.

African Women with Cervical Cancer (AWACC) Study: Principal Investigator: Professor Lynette Denny Co-investigator: Dr Rosalind Boa

This study recruited 567 women with invasive cervical cancer from Nigeria, Ghana and South Funded by GlaxoSmithKline. All biopsy specimens were typed for HPV at a central laboratory. The data was published in the International Journal of Cancer in 2014 (Int J Cancer 2014;134:1389-98).

Title: Evaluation of the safety and immunogenicity of GlaxoSmithKline Biologicals’ HPV Vaccine 580299 in adult Human immunodeficiency virus infected female subjects.

Detailed Title: A phase I/II, partially-blind, randomized, controlled study to assess the safety and immunogenicity of GlaxoSmithKline Biologicals’ HPV-16/18 L1 VLP AS04 vaccine administered intramuscularly according to a three-dose schedule (0,1,6-month) in human immunodeficiency virus (HIV)-infected female subjects aged 18-25years.

This was the first study of the safety and immunogenicity of the HPV vaccine against types 16 and 18 performed in HIV Positive women. The study recruited 30 HIV negative women (all of whom were vaccinated) and 120 HIV positive women in WHO stage 1 disease. These women were randomised to receive vaccine or placebo (60 in each arm). Women were followed for 18 months to evaluate safety and immunogenicity. Trial completed December 2011 and manuscript published in Vaccine 2013;31(48):5745-53

A comprehensive analysis of the molecular changes occurring during the development of cervical carcinoma may have a significant impact on our understanding and treatment of the disease. To address the underlying molecular events related to cervical cancer our specific aims are: (a) to identify transcriptional profiles and genes associated with the progression of disease (use microarray analysis to identify profiles and genes) and (b) to characterize genes as potential markers, and genes that have functional significance in the development of the cancer (quantitative RT-PCR and immunohistochemisty to confirm microarray data and RNA technology for functional studies).

New initiatives taken in convening this course involved introducing broader concepts of Women’s Health. These include organising and introducing lectures/seminars on:  Understanding Gender (African Gender Institute, University of Cape Town)  Two hour seminar on Adolescent Sexuality and Contraception (Planned Parenthood Association of SA)  Two hour seminar on Adult Sexuality (Dr Eve Wasserman)  Two hour seminar on Rape (Rape Crisis Cape Town)  Two hour seminar on Domestic Violence (NICRO)  The Forensic Examination of Women who are Raped (Professor L Martin/ Dr Sonata Walraven)

At the end of each block the students underwent an examination consisting of an OSCE and a one and a half hour written examination. One of the OSCE stations was devoted to feed-back from the students regarding the course and this information was invaluable in the constant adjustment of the programme.

In addition to the examination, the students were required to perform a literature review on any subject related to Women’s Health. This was performed in groups. One such project, on Female Genital Mutilation, was published by the SAMJ in 1999.

Other responsibilities included supervising problem students, including providing extra tuition and practice for written examinations, particularly for students writing in English as a second language.

Design Of New Curricula For Semesters 6, 7/8 And 9/10 MBChB

Principle designer of the gynaecology blocks in semesters 6, 7/8 and 9/10 in 2004. The timetables and objectives of these courses are available on request. The three different courses

Clinical teaching includes daily teaching ward rounds, pre-operative rounds, post-operative rounds, working ward rounds and formal grand teaching rounds. All 5 clinics are attended by students on a daily basis.

Post-Graduate Teaching

Supervision of training of Gynaecology Oncology Senior Registrar/Sub-specialist

The Gynaecology Oncology Unit of the Department of Obstetrics & Gynaecology was recognised for sub-speciality training by the Royal College of Obstetrics & Gynaecology until 2000. It was recognised as a sub-speciality training unit by the Health Professionals Council of South Africa in 2007. The training programme is 2 years and only registrars who have obtained Part 2 specialist exams are eligible. Gynaecological Oncology has been recognised as a sub- speciality of Obstetrics and Gynaecology by the Health Professionals Council of South Africa and we have been registered as a sub-speciality training unit since 2007. We have two training numbers as of January 2010.

Supervision of the senior registrar entails teaching/training in general gynaecological and cancer surgery, management of all gynaecological cancers and complications, colposcopy, chemotherapy, radiation therapy, gynaecological pathology, management of terminally ill patients including provision of support to family. Training also includes management of the ward, providing a consultative service to other wards and hospitals, clinics and students. An important aspect of the supervision of the senior registrar is teaching a multi-disciplinary approach to cancer management.

In addition, the fellow is required to undertake a research project and to write this up for a MMed dissertation and to pass an exit examination.

I generated the idea to begin a campaign against sexual harassment at both the Faculty of Health Sciences at UCT and Groote Schuur Hospital. The campaign represents a collaborative
