04/2019 Part IX APPLIANCES (APPROVED LIST OF APPLIANCES - NOTES) Part IX - Appliances (Approved list of Appliances - Notes) APPROVED LIST OF APPLIANCES See Part II, Clause 8A (page 9) The price listed in respect of an appliance specified in the following list is the basic price (see Part II, Clause 8) on which payment will be calculated pursuant to Part I, Clause 5B in respect of the dispensing of appliances. NOTES 1. Definition - The appliances that may be supplied against orders on Forms FP10 are listed below and must conform with the specifications shown. See Part 1, Clause 2 (page 4). These specifications include published official standards ie BP, BPC or relevant British, European or International Standards or the Drug Tariff Technical Specification. It should be emphasized that any appliance must conform with the entry in this part of the Tariff as well as the official standard or technical specification quoted therein. Other dressings and appliances which may be necessary will normally be provided through the Hospital Services. 2. Sealed Packets - These are those which are "tamper-evident" - sealed with an easily detachable device that prevents removal of the contents without the seal being broken. Additionally in the case of sterile products: once a sealed package has been opened it should not be possible to re-seal it easily. Where an appliance, other than a bandage, required by the Tariff to be supplied in a sealed packet is ordered of a quantity or weight not listed in the Tariff, the quantity ordered should be made up as nearly as possible with the smallest numbers of sealed packets available for the purpose. Where the quantity ordered is less than the smallest quantity/weight, supply the smallest pack. The quantity of material in each packet supplied should be recorded on the prescription form. 3. Quality - Any medical device placed on the market in the UK has to be CE marked by the manufacturer in accordance with the safety, quality and performance requirements laid down in the EC Medical Devices Directive 93/42/EC and the Medical Devices Regulations 2002 (as amended) which transposes the Directive in to UK law. Manufacturers are required to obtain certification from a third party conformity assessment organisation known as a notified body for all but the lowest risk devices. Notified bodies have been designated as competent to undertake this function by the regulatory authority in the registering member state. In the UK this is the Medicines and Healthcare products Regulatory Agency (MHRA). There are a number of conformity assessment routes that the manufacturers can adopt to demonstrate conformity with the requirements of the Directive but the majority use a quality system approach. More details of the CE marking process can be obtained from the MHRA, 151 Buckingham Palace Road, Victoria, London. SW1W 9SZ or from their website http:// www.mhra.gov.uk/Howweregulate/Devices/index.htm. 4. Weights - All weights specified in the Tariff in respect of appliances are exclusive of wrappings and packing material. 5. Invoice price - This is the price chargeable for the appliance to the contractor by the manufacturer, wholesaler or supplier. 6. Technical Specifications - Numbered Technical Specifications for the items in Part IXA/B/C were published separately in 1981 in loose leaf volume and a revised list was published in 1983. The Specifications will not be reprinted annually but individual specifications will be introduced, amended and reprinted, or withdrawn, as necessary. A list of Technical Specifications is given on page 282. CE marked Devices have shown that they meet the essential requirements of the Medical Devices Regulations 2002 and there is no further need for them to demonstrate compliance with other specifications, such as those referred to above. 7.     NEW ZEALAND DATA SHEET 1. OVESTIN PESSARY (0.5mg) 2. QUALITATIVE AND QUANTITATIVE COMPOSITION Each pessary contains 0.5 mg Estriol. For the full list of excipients, see section 6.1. 3. PHARMACEUTICAL FORM Pessary 0.5 mg - white, torpedo formed pessary. One pessary (2.5 g weight) contains 0.5 mg oestriol. Length 26.5 mm; largest diameter 14 mm. 4. CLINICAL PARTICULARS 4.1 Therapeutic indications Atrophy of the lower urogenital tract related to oestrogen deficiency, notably • for the treatment of vaginal complaints such as dyspareunia, dryness and itching. • for the prevention of recurrent infections of the vagina and lower urinary tract. • in the management of micturition complaints (such as frequency and dysuria) and mild urinary incontinence. Pre- and postoperative therapy in postmenopausal women undergoing vaginal surgery. A diagnostic aid in case of a doubtful atrophic cervical smear. 4.2 Dose and method of administration Dosage OVESTIN is an oestrogen-only product that may be given to women with or without a uterus. Atrophy of the lower urogenital tract 1 pessary per day for the first weeks, followed by a gradual reduction, based on relief of symptoms, until a maintenance dosage (e.g. 1 pessary twice a week) is reached. Pre- and post-operative therapy in postmenopausal women undergoing vaginal surgery 1 pessary per day in the 2 weeks before surgery; 1 pessary twice a week in the 2 weeks after surgery. A diagnostic aid in case of a doubtful atrophic cervical smear 1 pessary on alternate days in the week before taking the next smear. Administration OVESTIN pessaries should be inserted intravaginally before retiring at night.

    UK Clinical Guideline for best practice in the use of vaginal pessaries for pelvic organ prolapse March 2021 Developed by members of the UK Clinical Guideline Group for the use of pessaries in vaginal prolapse representing: the United Kingdom Continence Society (UKCS); the Pelvic Obstetric and Gynaecological Physiotherapy (POGP); the British Society of Urogynaecology (BSUG); the Association for Continence Advice (ACA); the Scottish Pelvic Floor Network (SPFN); The Pelvic Floor Society (TPFS); the Royal College of Obstetricians and Gynaecologists (RCOG); the Royal College of Nursing (RCN); and pessary users. Funded by grants awarded by UKCS and the Chartered Society of Physiotherapy (CSP).     Pessary for management of Pelvic Organ Prolapse Cathy Davis Clinical Nurse Specialist Department of Urogynaecology King’s College Hospital, London Definition of Prolapse The descent of one or more of the anterior vaginal wall, posterior vaginal wall, uterus (cervix) or vaginal vault (cuff scar after hysterectomy). The presence of any such sign should be correlated with relevant POP symptoms. (Haylen et al 2016) Risk Factors • Increased intra-abdominal pressure • Chronic cough • Chronic constipation • Weight lifting • Presence of abdominal tumours - fibroids & ovarian cysts • High impact exercise • Age/ Menopause • Obesity Risk Factors contd.... • Smoking • Multiparity • Congenital weakness – rare; due to deficiency in collagen metabolism • Injury to pelvic floor muscles • Iatrogenic/ pelvic surgery - hysterectomy Symptoms of POP • May be asymptomatic – a small amount of prolapse can often be normal • Sensation of a lump or bulge " coming down" - most common • Backache • Heaviness • Dragging or discomfort inside the vagina – often worse on standing /sitting for prolonged periods • Seeing a lump or bulge Symptoms of POP contd.... • Bladder / Urinary symptoms -Frequency -Difficulty initiating voids, low-flow, incomplete bladder emptying -Leakage on certain movements or when lifting heavy objects -Recurrent urinary tract infections • Bowel symptoms -Constipation - Incomplete bowel emptying - May have to digitate to defecate/use aides • Symptoms related to Sex – uncomfortable, lack of sensation Diagnosis of Prolapse • Vaginal examination •

Powel Chair of the Supervisory Committee: Professor Kristi Morgansen Aeronautics & Astronautics While control actuation is well understood to influence the observability of nonlinear dy- namical systems, actuation of nonlinear stochastic systems by process noise has received comparatively little attention in terms of the effects on observability. As noise is present in essentially all physically instantiated systems, complete analysis of observability must account for process noise. We approach the problem of process-noise-induced observability through the use of a tool called the empirical observability Gramian. We demonstrate that the empirical observability Gramian can provide a unified approach to observability anal- ysis, by providing sufficient conditions for weak observability of continuous-time nonlinear systems, local weak observability of discrete-time nonlinear systems, and stochastic observ- ability of continuous-time stochastic linear systems with multiplicative noise. The empirical observability Gramian can be used to extend notions of stochastic observability that depend explicitly on linear systems structure to nonlinear stochastic systems. The different business models have been explained theoretically and exemplified with real business cases from around the world. To complement the conducted primary research, the author has conducted several expert interviews. Then the different models’ ability to contribute to the solution of the waste and pollution difficulties caused by the linear take-make-dispose model employed throughout the industry is assessed. Five categories have been derived to rate the degree of change the business models can add in the development towards a Circular Economy. The results show that only a fully Circular Supply Chain model is able to enhance systemic change at large scale while eliminating waste and pollution through its inherent design. September 2018 in nürnberg und fürth ARCHITEKTUR VORWORT / INHALTSVERZEICHNIS Visionen Nürnberg macht sich fit für die Zukunft. Und die Kultur dient dabei als Zugpferd; gerade in der Entwicklung einer Vision für eine lebenswerte (Kultur-)Stadt 2025 und darüber hinaus. Überall findet sich Aufbruch: der Bau einer neuen Universi- tät in Nürnberg, die Entwicklung zukunftsweisender Materialien in der Energie- wirtschaft, innovative Robotik in der Chirurgie, virtuelle Museumslandschaften, die Entwicklung einer internationalen Ethik des Völkerrechts, die Untersuchung neuer Formen des städtischen Zusammenlebens in europäischen Forschungs- projekten sowie Wohn- und Arbeitsformen, in denen sich die Menschen gegen- seitig unterstützen und inspirieren. Das Schlagwort „Partizipation“, das gerade bei Zukunftsvisionen erhöhte Bedeutung erlangt, hat in Nürnberg seit der Kulturladenbewegung der 70er Jahre eine gewisse Tradition. Signifikant sind dafür auch Formate wie die Stadt(ver)führungen, die auch in der 19. Auflage wieder sachkundigen Bürgern, bekannten Persönlichkeiten, professionellen Stadtführern sowie engagierten Vereinen und Institutionen Raum geben, ihre Visionen auszudrücken und zu vermitteln. Wesentlich ist die Begegnung der Menschen miteinander bei der klassischen Führung und bei partizi- pativen Formen wie der offenen Probe eines Theaterstücks, dem internationalen Frauenfrühstück, dem Jodelworkshop, der Kreation eigener Visionen im Forschungslabor oder der gemeinsamen Erstellung eines Kunstwerks. Die enorme Bandbreite des Themenspektrums ist den vielen Beteiligten zu verdanken, die sich intensiv für diese Art der Wissensvermittlung engagieren. Ihnen gilt mein herzlicher Dank ebenso wie den Mitarbei- tern im Projektbüro des Kulturreferats und den Förderern für ihre Unterstützung: dem ESW – Evangelisches Siedlungswerk, der VR Bank Nürnberg, den Nürnberger Nachrichten sowie dem Funkhaus Nürnberg.
