We experienced many highlights in 2017, such as being chosen Non-Profit of the Year by the Chicopee Chamber of Commerce, recieving four prestigious national and state awards for excellence and quality of stroke care, and seeing our 1000th patient in our Weight Management Program. Our most notable accomplishment, however, was opening our new Emergency Department. This state-of-the-art, 21,460 square foot facility features a new Center for Behavioral Health Services, 40 treatment areas, multi-patient trauma rooms, advance lifesaving equipment and a patient navigation service. As 2017 came to an end, Holyoke Medical Center entered its 125th year of caring for our community. To celebrate this momentous occasion, we held an Anniversary Gala on Sunday, September 16, 2018 at the Log Cabin in Holyoke. The night began with a social hour and champagne toast, followed by a 7 course chef’s dinner & wine pairings. Guests enjoyed music from the O-Tones and bid on silent auction items in between socializing and dancing. At the evening’s close, each guest received a complimentary copy of Holyoke Medical Center: 125 Years of Caring for Our Community created in partnership with The Republican. Other notable accomplishments in 2018 include being selected as one of only 12 national Weight Management Programs to participate in the clinical study of the Elipse Balloon, offering walk-in services in our new Chicopee medical office, and becoming the only hospital in Massachusetts with ISO 9001 Certification. In closing, a lot has changed since our foundation in 1893, but we have not strayed from our commitment to growth and improvement. Hospitals continue to be challenged by the changes in our healthcare system, but we remain dedicated to our mission to serve the Pioneer Valley by providing the best possible care with compassion and love for what we do. The support we receive from the community helps us to keep pace with changing technology and the growing healthcare needs of our community. As we look ahead to the next 125 years, please know that your partnership is vital to the success of our organization. Thank you again for your generosity and designating Holyoke Medical Center as your charity of choice! • 90-Day Notice of Intent to Discontinue Services
June 29, 2020 Via Email Sherman Lohnes, Esq., Director Division of Health Care Facility Licensure and Certification Bureau of Health Care Safety and Quality Department of Public Health 67 Forest Street Marlborough, MA 01752 Re: Holyoke Medical Center, Inc. – Formal 90-Day Essential Service Notice – Closure of Inpatient Obstetrics/Maternity Services and Well Infant Nursery Dear Attorney Lohnes: We write on behalf of Holyoke Medical Center, Inc. (the “Medical Center” or “HMC”). Pursuant to 105 CMR 130.122(B), the Medical Center hereby provides ninety (90) day notice to the Department of Public Health (“Department”) of its decision to discontinue its operation of its Inpatient Obstetrics/Maternity Service and its bassinet Well Infant Nursery service (the “Services”). This letter is in follow-up to the required initial notices that were filed with the appropriate parties on May 29, 2020. We offer the following comments with respect to this matter. The Medical Center has determined that the Services should be closed due to low utilization for the past several years. Without a higher caseload of obstetric/maternity and infant patients, the professional skills and competence of the Medical Center’s physicians and staff have the potential to not be sufficiently preserved or developed, particularly in matters involving complications such as emergency C-sections. In addition, without a critical volume, further investments in the Services are financially not feasible. A hospital cannot maintain clinically thriving and financially sound obstetrical/maternity and nursery services if few patients choose to use them and, instead, obtain care from other providers. Patient choice in the service area is the main driver of these considerations.

These needs were identified in the 2013 Community Health Needs Assessment (CHNA) published and made widely available to the public on August 29, 2013. The 2013 CHNA and this implementation strategy were undertaken to understand and address community health needs, and in accordance with proposed Internal Revenue Service (IRS) regulations pursuant to the Patient Protection and Affordable Care Act of 2010. Final guidance as to the content and format of these documents has not been issued by the IRS. This implementation strategy outlines the significant community health needs described in the CHNA that Holyoke Medical Center plans to address in whole or in part. Holyoke Medical Center may amend this implementation strategy as circumstances warrant. For example, certain needs may become more pronounced and merit enhancements to the described strategic initiatives. Alternately, other organizations in the community may decide to address certain community health needs included here. This plan and its strategies may be refocused to account for such changes in the community landscape. • Your Choice 3-Tier Option— a Tiered Provider Network Plan 2018 Hospital Tiering List
Your Choice 3-Tier Option— A Tiered Provider Network Plan 2018 Hospital Tiering List Your Choice is a Tufts Health Plan tiered provider network comprised of primary care providers, specialists and hospitals. As part of our National Committee for Quality Assurance (NCQA) accreditation and our own quality standards, we are committed to contracting only with high-performing hospitals. Every doctor and every hospital in the Your Choice network has been carefully evaluated, and every provider and hospital in the Your Choice network meets our standard for quality of care. Because not all doctors and hospitals charge the same costs, we’ve factored in cost-efficiency in our evaluation, and we have separated Your Choice providers into three tiers. Provider groups and their primary hospital affiliation are in the same tier to reflect referral patterns that those physicians would make most often.* Below is a list of all the hospitals in Tufts Health Plan’s network and their tiers. In choosing a tier, members must consider how much they are willing to pay out-of-pocket for the doctor and/ or hospital they prefer. If members want the most cost-effective option, they should choose a doctor or hospital from Tier 1. If the doctor or hospital is in Tier 2 or Tier 3, they have the freedom to choose, but they will pay higher out-of-pocket costs. Members pay different levels of copayments, coinsurance, and/or deductibles depending on their plan design and the tier of the provider delivering a covered service or supply. Your plan may make changes to a provider’s tier annually.
