This is a replacement post building on the new alliance of skin care across Oxfordshire & Berkshire. Located within the Thames Valley. Oxford’s service is a recognised centre of excellence with the region having one of the highest incidences of skin cancer in the UK. We have state of the art surgical, diagnostic and laboratory facilities in a purpose built unit with plans for additional investment in the estate to cope with increasing demand. We have excellent on-site links with plastic surgery and with a centralised head and neck skin cancer service. The successful applicant will also support the current delivery of the busy & innovative Dermatology services at the Royal Berkshire Hospital whilst contributing to development of skin care provision in the locality. 5. PRINCIPAL ACTIVITIES:

The post-holder’s duties will be at the Churchill Hospital, Oxford (7 PA) and Royal Berkshire Hospital (3 PA), but it is a condition of the appointment that the post-holder will be willing to work in any of the Trusts’ locations.

Clinical The post-holder will use most of his/her time in clinical activities directly relating to Dermatology. The total NHS work load is 10 PAs each week. The post-holder’s duties will be based at the Department of Dermatology, Churchill Hospital, Oxford (7PA) & Townlands Hospital, Henley-on- Thames (3PA), but part of this post may require delivery clinical activity at an alternative clinic eg Royal Berkshire Hospital, Reading or West Berkshire Community Hospital, Newbury. Their presence may from time to time be required in other parts of the Trusts. Clinical care may involve collaboration with and/or supervision of an intermediate dermatology service. The post-holder will also be required to maintain excellent links with primary care at a personal and administrative level. New substantive consultant appointees will be offered mentoring within the Department of Dermatology and by a more senior colleague from another speciality. The Trust also has a Future Leaders Training Programme available to new and junior consultants.

Junior medical support and training: The Dermatology services participate in the Oxford Training Rotation, which consists of specialist training posts shared between several hospitals (The Churchill & John Radcliffe Hospitals, Oxford, the Horton Hospital, Banbury and the Royal Berkshire Hospital, Reading). At any one time, we have at least 6 registrars based in Oxford & Reading each spend approximately 24 months based in each department.

Out-patient clinics: The clinical work plan (see below) will typically involve PA sessions in out- patient clinic settings and PA sessions in supporting activity. The Dermatology Service in Oxford sees more than 48,000 patients per year, in addition to this the RBH service sees more than 26,000 patients per year including new referrals, follow-up consultations and clinical procedures including laser and surgery. Clinics at both sites normally consist of lists staffed by a consultant with a specialist or research registrar and ST1-2. Nurse led clinics and surgical lists are well established and consultant supervised. There is a well-established email advice service In Oxford providing clinical advice by consultants with a 24-48 hour response time. We would like to develop this for Reading as well.

Ward rounds & in-patient cover: All in-patients are admitted on the Churchill site where possible or over at the . Reading patients are admitted to the RBH. 4

There are no acute general medicine on-call duties, but the dermatology consultants at Oxford take part in supervising the non-resident registrar on-call rota currently on a 1 in 10 basis. The Churchill Hospital has a resident general medical hospital-at-night ST1-2 rota shared between departments.

All consultants are expected to participate in the direction of the department’s clinical audit programme, which will normally be carried out with the aid of the registrars.

Teaching/Research Oxford University Hospitals NHS Foundation Trust is a teaching hospital trust, and the post-holder will be required to participate in programmes for teaching clinical students, training junior doctors and in clinical examinations. It is expected that this will be an integral part of everyday clinical activity. The post-holder’s contribution to teaching, training and research will be included in the regular job plan review.

Clinical Governance The post-holder will participate in all clinical governance activities, including clinical audit, clinical effectiveness, risk management, quality improvement activities as required by the Trust, and external accrediting bodies.

Personal and Professional Development The post-holder will be required to keep himself/herself fully up-to-date with their relevant area of practice and to be able to demonstrate this to the satisfaction of the Trust. Professional or study leave will be granted at the discretion of the Trust, in line with the prevailing Terms and Conditions of Service, to support appropriate study, postgraduate training activities, relevant CME courses and other appropriate personal development needs as part of revalidation.

Management The post-holder will be required to work within the Trust's management policies and procedures, both statutory and internal, accepting that the resources available to the Trust are finite and that all changes in clinical practice or workload, or developments requiring additional resources must have prior agreement with the Trust. He/she will undertake the administrative duties associated with the care of his/her patients, and the running of his/her clinical service under the direction of the lead clinician and/or directorate chair.

Risk Management The management of risk is the responsibility of everyone and will be achieved within a progressive, honest and open environment.

Dermatology The challenge in 2021 is to develop and evolve the service further to match the ever increasing demand i.e. ~12-15% increase in 2 week wait referrals year on year 2009-2019 whilst adapting to the challenges and changes brought about by the pandemic. The Oxford Department of Dermatology is a busy regional centre for dermatology. We provide an excellent skin cancer and general dermatology service to the patients of Oxfordshire and beyond. We train all the registrars in the Oxford Rotation at some stage.

We have trained GPwSIs to develop a commissioned low risk tumour service in the community and train all our GP registrars in basic surgical techniques. We have trained Australian and New Zealand registrars in surgical fellowships in the past and have offered surgical sabbaticals to a number of consultants from UK, Europe and Russia. Oxford was one of only two UK skin surgery units who were awarded Department of Health funding for a post CCT fellowship in skin surgery for 2009/2010.

There are daily consultant-led skin cancer screening clinics (up to 34 patients: 11x 2 week wait new, 13x routine new and 10x follow-up patients with support from a dermatology StR and an ST1-2 GP trainee or FY1) with parallel minor ops clinics allowing for a one-stop service (2-3 lists with 2-3 booked patients and one-stop cases with a consultant, registrar, fellow and/or GP assisting). This is under review currently as the make-up of clinics evolves with the pandemic and active triage of referrals.

The unit performs on average 300 day case operations per month of varying complexity from simple excisions to complex flap repairs and grafts. All procedural work is consultant supervised. Surgical outcomes are regularly audited and all cancer cases are reviewed weekly with regard to diagnosis and adequacy of excision. All common tumours have an agreed management protocol for Oxfordshire, which has resulted in a robust and trusting relationship with our surgical and oncology colleagues. We meet weekly with the plastic surgeons in parallel clinics in the Churchill & Horton Hospital, Banbury and at the weekly MDT clinics.

We have 2 dedicated skin cancer nurse specialists. Mohs surgery serves both the local population and as far as a field as Somerset and Northampton. The case mix is half ocular (William Perkins; also DFSP) and half non-ocular (Richard Turner & Sarah Felton) - on average ~300 cases per year in total.

Laser work is primarily pulsed dye and Nd:YAG laser with a general anaesthetic list once per month. These are supervised by Dr Wali +/- Dr Felton. 12

The photodynamic therapy (PDT) service is onsite with weekly nurse-led clinics.

General dermatology patients are seen in out-patient clinics at the our main unit on the Churchill site and also at The Horton Hospital. We are supported by a highly experienced dermatology nursing team, skin cancer nurse specialists, biologic nurse prescribers and a dedicated pharmacist. We have good working relationships with our colleagues supporting a number of specialist interests and clinics eg oral medicine, rheumatology.

Facilities The Department of Dermatology is a stand alone building annexed to the main hospital via a link corridor and is adjacent to the new Oxford Cancer Centre. Head & Neck surgery and Skin Cancer Plastic Surgery are in a dedicated unit next to the department on the Churchill site allowing for an integrated service in conjunction with the internationally recognised Melanoma Oncology Unit and Haematology where an increasing number of bone marrow allografts are performed. Additional bowel and pancreas transplants are also performed at the Churchill. We are fortunate to have a full range of surgical and medical facilities on site.

Theatre suite We have a dedicated state of the art skin surgery suite which includes:  4 dedicated skin surgical theatres all with integrated oxygen and suction facilities.  3 bed recovery area - with integrated oxygen and suction facilities.  Office area

Minor-op room A separate biopsy rooms are used for minor diagnostic and vulval biopsies in out-patients.

Post-op dressing room All post-op wounds are seen in this dedicated area.

Wound care suite All patients with leg ulcers or chronic wounds are reviewed in this recently renovated dedicated 2 bed area.

Phototherapy suite We have narrow-band UVB and PUVA machine as well as hand and foot machines. We are also fortunate to have a phototesting monochromator.

Photography studio We are very fortunate to have Medical Illustration run a photographic studio in the department 5 days a week.

Patch test room We have a dedicated latex free room and patch test studio.

Research facilities Laboratory: The first floor of the dermatology unit includes a dedicated laboratory. All Mohs’ sections are fully processed here. Microscope rooms: There are two separate microscopes: one triple headed microscope in the laboratory and a double headed Nikon microscope is in the seminar room for slide reporting and meetings. Seminar Room: Digital projection is available for histopathology cases in the weekly pathology review meeting. This enables clinical, dermoscopic and histopathological correlation to be available for all suspicious pigmented tumours that are excised.

Skin Cancer Skin Cancer Governance The department produces regular audits of performance, which are discussed at the MDT or at the monthly departmental governance meeting.

MDT & SSMDT The weekly MDT & SSMDT meetings take place in the state of the art facility £109 million Oxford Cancer Centre. The West Thames Specialist Skin MDT is video-conferenced to Reading and Swindon from Oxford.

Combined Oncology, Plastic Surgery and Dermatology Clinic Once monthly, following the MDT, a combined clinic occurs to review the patients with complex disease.

General +/- paediatric dermatology clinics run daily with consultant lists of 8 new and 6 follow- up patients ± supervising registrar lists. New to follow-up ratio is ~1:1.

Transplant Clinics Oxford has a large transplant population ie kidney, heart, bowel / pancreas and bone marrow, who are reviewed in the weekly skin transplant clinic.

Skin Cancer Research Oxford is one of the country’s 5 Biomedical Research Centres. The Skin Cancer Clinical Trials Unit is next door to the Department of Dermatology in the Cancer Centre. Its location ensures good communication between clinicians and researchers. The department is actively involved in melanoma clinical trials.

The Royal Berkshire Hospital was rated as an ‘Outstanding’ Hospital in its January 2018 CQC report, and was ranked as the number one place to complete foundation training in 2017.

The Royal Berkshire Hospital achieved Foundation Trust status on 1st June 2006. It was the first Foundation Trust in the South Central Strategic Health Authority. The Trust has a successful track record of delivering high quality acute medical and surgical services for 500,000 patients across West Berkshire, an area extending from Newbury in the west to Henley-on-Thames in the east, and including Wokingham and parts of Hampshire to the south and parts of Oxfordshire to the north. The Trust is one of the largest general hospital Trusts in the country and is Reading’s biggest employer.

The hospital provides all those services normally associated with a very large District General Hospital. Trust Management The Trust is under the leadership of Graham Sims, Chair, Steve McManus, Chief Executive and Lindsey Barker, Medical Director. Successfully achieving Foundation Trust status was an important landmark for the Trust and recognised the commitment of its management team and clinicians to maintain the high quality of its service delivery.

The Trust has in place a Clinically Led structure placing the decision making responsibilities with Clinicians supported by management teams. As an organisation we do not stand still and our success is because of the strong clinical leadership and the ‘can do’ attitude of the staff. There are 3 Care Groups (Urgent, Planned and Networked Care).

The Appointment This is a new 10 PA shared post for a Consultant Dermatologist at the Royal Berkshire NHS Foundation Trust (6PA) & Oxford University Hospitals NHS Foundation Trust (4PA).

Our service covers a large area encompassing both East and West Berkshire and parts of Oxfordshire and Hampshire. We are lucky to work in very beautiful area of the country including Henley and West Berkshire.

We pride ourselves on being a very friendly department and have gone from strength to strength in the last few years in terms of expanding our department, services and non-consultant grade workforce and it is now time to expand our consultant numbers.

The successful applicant will support the current delivery of the Dermatology service, including contribution to the development of these services at the Royal Berkshire NHS Foundation Trust. Subspecialty interests of successful candidates can also be accommodated.

The appointee will join the existing team to provide specialist care to this growing population of patients, and raise departmental profiles through teaching and education.

The aim of the Royal Berkshire NHS Foundation Trust Dermatology service is to become a leader in the field of dermatological management and the successful applicant must therefore have the pre-requisite procedural and leadership skills as well as team work, drive and ambition to succeed in achieving these goals.

The Dermatology Offices are situated in the main part of the hospital. All consultants share a large open plan office.

Oxford University Hospitals NHS Foundation Trust ______Clinic facilities are provided in the main outpatients department and include 2 fully equipped minor ops theatre and treatment rooms for PDT.

The department works around a hub and spoke model and peripheral clinics include:  Bracknell Health Space, Bracknell, equipped with outpatient facilities and a skin surgery theatre.  Townlands Hospital, Henley, new facilities including a skin surgery theatre.  West Berkshire Community Hospital, Thatcham with facilities for skin surgery and phototherapy.

There are weekly dermatology MDT meetings, which are administered by dedicated MDT co- ordinators and attended by plastic surgery, medical and clinical oncologists, oculoplastic and ENT consultants as well as dermatologists. All relevant skin cancer cases are discussed with a histopathologist present. Twice a month we take part in SSMDT meetings with Oxford via video conferencing, followed by an MDT clinic where patients who need a multidisciplinary approach are seen

We have a phototherapy suite. This is staffed by a dedicated nurse and houses 2 TLO1 machines, plus facilities for topical phototherapy. We also have a TLO1 machine at West Berkshire Community Hospital to treat patients living in the West Berkshire locality.

We have close links with our rheumatology colleagues. Every quarter there is a combined dermatology/rheumatology clinic and a vasculitis clinic.

We have no dedicated dermatology beds but any dermatology emergencies are admitted to a general medical ward under the care of the general physician on that day, with dermatology input. We admit few patients and find this system works well.

We provide a specialist referral service for inpatients to assist our colleagues and successful candidates will contribute to the provision of this service.

We also have a Reading Dermatology Research Group, which has been actively involved in research over the last few years.

Non-Consultant Staff 2 StRs on the Oxford Dermatology training rotation, 1 GP VTS trainee, 1 specialty doctor, 2 GPsSIs and 3 dermatology clinical nurse specialists and various support staff in outpatients who perform phototherapy and PDT. We have a dermatology cancer nurse specialist who works closely with the MDT.

The department is also supported by our ‘Clinical Administration Team’ consisting of 10 staff including a booking officer.

Immediate Responsibilities of the Appointee The post holder will share clinical and managerial leadership within the Dermatology service with the present consultants. An office base with secretarial and administrative support will be provided.

This is a new substantive NHS posts in the Department of Dermatology due to expansion of the unit and development of a skin care alliance across Berkshire & Oxfordshire.

Applicants must be on the General Medical Council Specialist Register or within six months of being admitted to the Register for trainees if in a training programme within the UK. In accordance with the regulations, all other categories of doctors must be on the GMC Specialist Register to be considered for a consultant appointment by the Advisory Appointments Committee. The appointee must have a CCT in Dermatology. Wide experience is required in all aspects of Dermatology.

The trust is committed to ongoing training and support of consultants, There is a structured two year programme of induction and leadership development. Every consultant appointed is offered a choice of mentors and expected to meet with them on a regular basis.

Clinical Objectives 1. To provide, with colleagues, Dermatology services in both in-patient and out-patient settings.

2. To contribute to on-call cover 9am-5pm for the Dermatology service.

Management responsibilities 1. Active participation in monthly clinical governance meetings within the department.

2. Participation and involvement in meetings and other business in the Dermatology service

3. The post holders will be expected to participate fully in the teaching of pre-clinical & clinical undergraduate medical students from Oxford University. They will also be expected to contribute to the programmes of F1/F2, Core Trainee and Specialist Registrar teaching.

In addition, attendance is expected at the following: • Oxford Regional Meeting pm on 2nd Tues of month • Monthly pathology meeting 2nd Thursday of month, RBH

4. The successful applicants will be expected to contribute to medical audit and clinical governance and clinical governance meetings take place on alternate months.

5. The successful candidates will be expected to maintain his/her professional development to the level set by the Royal College of Physicians. The department runs an annual appraisal system in line with the National Appraisal Scheme for Consultant Medical Staff (as outlined in MD5/01) and expects every consultant to maintain a personal development portfolio.

6. Main Conditions of Service

1. The post is covered by the national Terms and Conditions of Service of Hospital Medical and Dental Staff ( and Wales) and the Conditions of Service determined by the General Whitley Council for the Health Services (Great Britain), as amended from time to time. As the Trust develops it will review these terms and may vary the national agreements to respond to local conditions and reflect Trust requirements following negotiation with the BMA Local Negotiating Committee.

2. The new consultant will be required to maintain his/her private residence in contact with the public telephone service, and to reside within ten miles by road from the Churchill Hospital unless specific written approval for a greater distance is given by the Medical Director of the Trust.

3. The salary scale is £75,249 – 101,451 with nine thresholds (1 April 2013).

4. The successful candidate must hold full registration and licence to practice with the GMC.

5. The successful candidate must have CCT (or equivalent for non-UK applicants of equivalent status), or be accredited (or equivalent), and be on the GMC Specialist Register in the specialty appropriate for the consultant post at the time of taking up the consultant appointment.

6. The successful candidate is not required to subscribe to a recognised professional defence organisation to fulfil his/her contractual obligations to this Trust, but should ensure he/she has adequate defence cover as appropriate, for example, for private and Category 2 work, and for GMC disciplinary proceedings.
