This proposal was developed in response to a Request for Proposal by the City of Black River Falls for completion of a National Register of Historic Places nomination for the Black River Falls Commercial District. Jean Guarino, Ph.D. (consultant) has twenty-one years of experience in the documentation of cultural resources, which includes the preparation of National Register nominations for individual buildings and districts. She will serve as the sole preparer of the nomination, which will be completed within six months of the start date. All work will be developed in conformance to the requirements detailed in the manual found on the Wisconsin Historical Society (WHS) website.

II. SCOPE OF WORK

Task 1: Site Visit and Archival Research Consultant will make a site visit to the City of Black River Falls to take photographs of individual buildings and streetscapes in the commercial district and determine which buildings within its boundaries are "contributing" and "non-contributing" to the district. Consultant will meet with municipal planning staff, if appropriate, to discuss any documentation that may be on file in their office, such as building permits, historic city plans and maps. Research on the historical and architectural evolution of the commercial district within the context of Black River Falls will be undertaken at a variety of repositories, including the Jackson County Historical Society and the Black River Falls Public Library. Resources to be reviewed will include, but are not limited to, Sanborn Fire Insurance maps, atlas maps, historic photographs, contemporary newspaper articles, and local and county history books.

Task 2: Development of Narrative Description (Section 7) Consultant will develop a Narrative Description for the Black River Falls Commercial District, which will focus on its current appearance and date any physical changes over time. The location, boundaries, and general layout of the district will be discussed in detail, including its general lot sizes and building setbacks.

The Narrative Description will provide an overall description of the buildings situated within the district boundaries, discussing them in terms of their architectural styles and materials. Integrity issues will also be discussed, noting the number of demolitions and any new construction that may have occurred within the district, as well as typical alterations experienced by its commercial buildings, such as window, door, and storefront changes.

The Narrative Description section will include a table listing all buildings and structures within the commercial district, arranged by street address. All resources listed will include the circa date of construction, architect name (if known), architectural style, number of stories, materials, and contributing/non-contributing status.

2 GUARINO HISTORIC RESOURCES DOCUMENTATION l OAK PARK, ILLINOIS I 708.386.1142 l GUARINOL@GMAILCOM Task 3: Development of Narrative Statement of Significance (Section 8) The Narrative Statement of Significance will establish that the Black River Falls Commercial District is eligible for National Register listing under Criterion A for Commerce and possibly also for Criterion C for Architecture. Based on the research found, consultant will develop two to three scholarly essays.

The first essay will discuss the historical and architectural evolution of the commercial district within the larger history of the City of Black River Falls. A second essay will discuss the architecture of the commercial district, showing that its buildings exemplify distinctive characteristics of several architectural styles from the late nineteenth- through the early twentieth-centuries. Representative examples of each style within the district will be highlighted. If the architects of some of the buildings are identified, a third essay will provide information on their professional work in Wisconsin and elsewhere, if found.

Task 4: Compilation of Nomination and Editing In addition to researching and writing the narrative essays contained in Sections 7 and 8, consultant will complete all other portions of the nomination form, including the development of a photo key, a web summary, and a site map. Historic photographs of commercial buildings/streetscapes within the district will be scanned and inserted at the end of the nomination report. Consultant will edit the nomination based on comments received from WHS staff following submittal of the nomination package, which will include the following:

• One paper copy of the National Register of Historic Places Inventory Nomination Form (19-900) and one electronic copy saved onto a disc for the WHS.

• One paper copy of a 200-300 word summary of the importance of the district and one electronic copy saved onto a disc for the WHS.

• Two commercially printed sets of digitally produced images for submittal to the WHS.

• Archival discs with digital images will be prepared for the WHS (JPEG format) and the National Park Service (TIFF format).

Task 5: Provision of Mailing Labels Consultant will provide a complete list of all current property owners as listed in the land or tax records after the nomination is scheduled for the Review Board meeting. Three full sets of mailing labels on Avery 5160 or Avery 6161 will be submitted to the City of Black River Falls.

Task 6: Power point Presentation at Public Meeting Consultant will participate in an informational meeting hosted by the City of Black River Falls prior to the State Historic Preservation Review Board meeting. A formal presentation of the historic district will be given at that time.

The presentation will be given at one of the quarterly meeting of the Historic Preservation Review Board.

Task 8: Uploading text and photos Consultant will upload photos and information found on buildings within the district boundaries to the Wisconsin Historic Preservation Database.

LaSalle Downtown Historic District — National Register District Nomination, 2017 Dr. Guarino was hired by the City of LaSalle to prepare a National Register district nomination for the LaSalle Downtown Historic District, which was nominated under Criterion A for commercial development and under Criterion C for architecture. Work involved site visits to photograph a total of 112 buildings that dated from the 1870s through the 1920s. Guarino undertook primary and secondary source research, wrote all aspects of the nomination, developed a table listing all buildings and their contributing/non-contributing status, and gave a 20-minute power point presentation to the Illinois Historic Sites Advisory Council in October 2017.

Commercial and Industrial Survey in Bloomington, Illinois, 2017-18 Dr. Guarino was hired by the City of Bloom.ington to undertake a survey of historic commercial and industrial buildings strung out along four nineteenth-century railroad lines. The goal was to evaluate these buildings for preservation planning purposes. Specifically, the intention was to identify the potential of commercial and industrial properties future local and/or federal landmark designation potential. Guarino photographed and developed survey data forms for 76 buildings and produced a report that included several historical context essays that involved intensive research. The report identified three potential National Register landmark districts and thirteen buildings that are potentially eligible for individual landmark status at the local level.

National Historic Landmark nomination, 2014-15 Dr. Guarino was hired by the Organization of American Historians to develop a National Historic Landmark nomination for the Illinois and Michigan Canal, a 97-mile waterway built 1836-48. The nomination involved intensive primary and secondary research to develop several lengthy, scholarly, footnoted essays. Included in the nomination were 150 canal-related buildings and structures located in different historic downtowns along its length, most of which were built in the late nineteenth-century. All resources were photographed and listed by address in a table that identified their contributing/non-contributing status.

Principal, Guarino Historic Resources Documentation • 1998 to present Jean Guarino has 21 years of experience in documenting cultural resources. Projects include the development of Historic American Buildings Survey/Historic American Engineering Report (HABS/HAER) documentation, National Register of Historic Places reports, and Architectural Survey Work. Clients include municipalities, architecture firms, developers, engineering firms, and non-profit organizations. Representative projects are listed below. A11 projects include site visits, digital photography, primary and secondary source research, as well as the development of architectural descriptions and integrity assessments, and historic context essays. Her professional background exceeds the U.S. Secretary of the Interiors Professional Qualifications Standards in Architectural History.

Dr. Guarino's project work includes the successful completion of thirteen National Register nominations, which include one residential district and one commercial district. She has also authored historic and architectural reports for 16 houses in the communities of Winnetka and Kenilworth, Illinois, which together represent a wide range of architectural styles. She has participated in residential building surveys for the communities of Downers Grove and River Forest, Illinois.

In 2012, we were selected by the Wisconsin Historical Society to complete the Fuldner Heritage Fund's inaugural project, completion of a National Register Multiple Property Documentation Form covering all known existing Centric Bams in the state. Not a nomination in its own right, our report will serve as a basis for evaluating National Register eligibility and Bert & Mary Cunningham Round Barn, Viroqua establish registration requirements for centric barns to be nominated in the future.

Since 2002, the staff of Legacy Architecture has frrf conducted 16 architectural and historical intensive surveys in urban, suburban, small town, and rural = ■ communities across Wisconsin, documenting over dàks. Historic Tax Credit Applications

Since 2012, our staff has successfully completed the three-part applications for state and federal historic tax credits for income producing buildings for numerous projects across the State of Wisconsin, securing $15.5 million in federal and state historic tax credits for our clients since 2013.

The Part 1 - Evaluation of Significance Application Eagle Paper & Flouring Mill, Kaukauna includes a written description of the buildings physical appearance outlining major features on the exterior and interior, a statement of significance summarizing the historical and architectural signifIcance of the building, and extensive photographic documentation.

A Part 2 - Description of Rehabilitation Application outlines the rehabilitation work to be undertaken on the exterior and interior, how it complies with the Secretary of the Interiors Standards, and photography illustrating the condition of the building.

The Part 3 — Request for Certification of Completed Work Application is submitted after the project is Green Bay YMCA, Green Bay complete and documents that the work was performed as proposed.

Section 106 Reviews

A referendum passed in the fall of 2016 for renovations and additions to the Florence County Middle/High School building. Because more than one acre of land would be disturbed and the school was documented in the Wisconsin Historical Society's Architecture & History Inventory (AHI), the Wisconsin Department of Natural Resources (WDNR) required that the property be evaluated for its eligibility for listing in the National Florence High School, Florence, WI Register of Historic Places as a part of its permitting review. The WDNR required a report that evaluated the project 's potential to adversely impact the resource, defined site limits and character relative to the areas impacted by project-related activities, and made recommendations for avoidance. Legacy Architecture visited the site, gathered data regarding the historic significance and integrity of the building, and prepared a Report of Investigations which was submitted to and approved by the WDNR.

TerX Shredding & Recycling Company purchased the former Brokaw Paper Mill in Brokaw, Wisconsin to use it as a scrap recycling facility and solid waste processing Brokaw Paper Mill historic photo, Brokaw, WI facility. They planned to demolish a portion of the historic mill that was not necessary for their operations. As more than one acre of land would be disturbed and the property was the site of two resources documented in the AHI, the mills complex and a church, the WDNR required that the property be evaluated for its eligibility for listing in the National Register and a Report of Investigations be submitted as a part of its permitting review. Legacy Architecture visited the site, gathered data regarding the historic significance and integrity of the buildings, and prepared a report which was submitted to and approved by the WDNR. The church was demolished sometime during the early 1980s, and the AHI was updated to indicate that. Due to its lack of Sheboygan, WI historic significance and architectural integrity, the mill was found to be not eligible for the National Register.

Through Community Development Block Grant monies from the U.S. Department of Housing & Urban Development, the City of Sheboygan offers low interest loans to help low or moderate income home owners or owners of residential rental property leased by tenants with low or moderate incomes to finance the maintenance and/or rehabilitation of their properties, thereby expanding the supply of adequate, safe housing within the city. We recently conducted an assessment of the National Register listed Port Washington North Breakwater Light for the City of Port Washington as a requirement of the conveyance agreement with the federal government in accordance with the National Historic Lighthouse Preservation Act to ensure proper maintenance and care, advance greater public access and use, and more actively highlight its architectural significance and beauty as a part of the community's harbor landscape. The project included a Preservation and Maintenance Plan so future Port Washington North Breakwater Light, Port Washington preservation work would conform to the Secretary of the Interiors Standards.

In the Fall of 2010, we were hired by the City of De Pere to prepare a Historic Structures Report (HSR) and Economic Feasibility Study for the National Register listed Lockkeeper's House along the Fox River in De Pere, which is owned by the State of Wisconsin's Fox River Navigational System Authority as a first step in planning for the building's future. The HSR documented the developmental history of the building, including its significance, chronology of ownership, construction, alternations, and prior studies and treatment efforts. It gave an architectural description of Ny the exterior and interior conditions of the building and De Pere Lockkeeper's House, De Pere identified character-defining elements and features. A conditions assessment was made of the site, envelope, interiors, structure, plumbing, heating, and electrical systems. Ordinances, codes, and accessibility laws were reviewed. Paint and mortar analysis was performed. Detailed rehabilitation recommendations were made for each façade, room, and feature. Work items were then prioritized, and rehabilitation costs for each potential future use were developed. Maintenance recommendations were made for the interim until the building could become occupied.

In 2009, we were retained by the North Woods County Historical Society to work on an HSR for the for the Upham Mansion, Marshfield Upham Mansion, a historic house museum. In an effort to reduce costs, significant portions of the HSR were performed by a group of volunteers. Our role was limited to participating in and overseeing the Condition Assessment, and authoring the Code & Accessibility Review and the Prioritization & Cost Estimate portions of the HSR. Historic Treatment Programs

We served as the historic treatment specialist for the general contractor on the restoration of the National Register listed Fox River Navigational System Authority's Kaukauna Locks 1, 2, 3, and Drydock in Kaukauna, Wisconsin. Our report documented the nontraditional process undertaken to rehabilitate the three locks, dry dock, and lock shacks along the Lower Kaukauna Fox River Lock #1, Kaukauna Fox River that were constructed between 1878 and 1917. The rehabilitation began in the fall of 2012 and continued thru the fall of 2015. Our report also documented the developmental history of the locks, described the existing components, and gave a chronology of their alterations. We developed a preservation plan describing the review process undertaken, the use of the Secretary of the Interiors Standards, the choice of an appropriate treatment, and the applicable rehabilitation standards and guidelines. Finally, we identified character-defining elements and gave a description of the existing conditions of the elements, along with detailed rehabilitation recommendations for each. A final draft of the report documented the competition of the rehabilitation work Kaukauna Fox River Lock #2, Kaukauna that was undertaken.

We served as the historic treatment specialist for the general contractor in their award winning work to stabilize the exterior of eight of the National Register listed Lockkeeper's Houses along the Fox River, owned by the State of Wisconsin's Fox River Navigational System Authority. Our objective was to develop a Historic Treatment Program for the entire project describing in detail the materials, methods, and equipment to be used for the work and addressing appropriate approaches to roof replacement, tuckpointing, repair of decayed wood, and protection of historic fabric. We also acted as a liaison to the Wisconsin Historical Society to maintain the historic Rapide Croche Lockkeeper's House, Kaukauna integrity of the sites and conform to the Secretary of the Interiors Standards.

We are currently serving as the historic treatment specialist for the general contractor on the restoration of the city-owned, National Register listed Riviera Building in Lake Geneva, Wisconsin.

Conditions Assessments

We conducted a Conditions Assessment in the summer of 2011 for the Neenah Historical Society to assess the current condition of its Octagon House Museum, identify potential areas of concem or defect, and develop estimates for its preservation and continued use. The report included studies of the general building/facilities (including the site, exterior of the building, and interior of the building), climate control and environment Neenah Historical Society, Neenah (including plumbing, heating, ventilating and air conditioning, lighting and electrical, and fire alarm and security systems), collections and collections policies, exhibitions, storage, and emergency preparedness.

In the Summer of 2010, we conducted a similar report of the Israel Stowell Temperance House in Delavan for the Delavan Historical Society. The goal of the project was to determine the building elements that needed to be rehabilitated in order to allow for the future use of the site, establish priorities of repairs, and to provide cost estimates for a rehabilitation project. The assessment Israel Stowell Temperance House, Delavan included descriptions of building materials, elements, and systems and specifícally addressed the site, envelope, interiors, structure, and mechanical and electrical systems.

During the fall/winter of 2007, we performed an Assessment for the Sheboygan County Historical Research Center, a historic house museum and research archive. Feasibility Studies

In the Fall of 2010, we were hired by the City of De Pere to prepare an Economic Feasibility Study and Historic Structures Report for the National Register listed Lockkeeper's House along the Fox River in De Pere, which is owned by the State of Wisconsin's Fox River Navigational System Authority. The Economic Feasibility Study involved consultations with key stakeholders to provide future use options for the house, De Pere Lockkeeper's House, De Pere which included an active recreational shelter, an institutional office, and heritage tourism lodging. Rehabilitation costs for each use were developed, and the economic viability of each use was determined. Schematic plans and elevations were developed for each use as a means to attract investors and prospective tenants. Finally, a realistic timeline was developed for architectural plans, securing rehabilitation funds, and construction completion.

We performed a Feasibility Study for the Renovation Oregon High School, Oregon and Reuse of the Oregon High School for the Village of Oregon's Historic Preservation Commission in 2003 in an effort to save the building from demolition and lay a foundation for the successful renovation and reuse of the building. The report included studies of the building exterior (including tuckpointing, window & door replacement, skylight replacement, roof replacement, and removal of later building additions) and the building interior (including the structural integrity, plumbing systems, mechanical systems & alternative energy applications, electrical systems, and the removal of asbestos). Since then, the building has been successfully rehabilitated as the headquarters for a nationwide urban housing developer. Background Experience & Capa bilities

Historic Preservation Plan City of Madison, Wisconsin

In 2017, Legacy Architecture was retained by the City of Madison to begin a two-year-long process to create Madison's first city-wide Historic Preservation Plan. To conduct the project, Legacy Architecture led a team of consultants including C.E. Planning Studio of Wauwatosa, for their urban planning and public participation expertise, and local historians Jason Tish and Timothy Heggland.

We began the project by attending the City's Racial Equity & Social Justice Initiative training and assisted City staff in developing a public engagement strategy to involve partners, identify community values, and generally guide the Historic Preservation Plan process in an equitable and inclusive manner.

Throughout early and mid-2018, we assisted city staff in facilitating a series of public meetings to educate and engage the public and city staff, seek input on the community's values, and solicit information on the influence of women and multi-cultural communities such as photographs, oral histories, and other primary source material. Based on public feedback, we developed Community Value Statements to direct the Historic Preservation Plan.

We conducted public meetings to identify existing and potential Implementation Tools and to develop and prioritize Goals and Objectives during late 2018.

We are currently developing a draft Implementation Strategy, identifying short-, mid-, and long-term action strategies, responsible parties, timing and deadlines, estimated cost, proposed source of funding, and public- private collaborations as well as how the plan is to be used, amended, and updated in the future. We will conduct future public meeting to provide information on and seek input from the public and city staff on the Implementation Strategy; then we will develop and draft the Historic Preservation Plan.

Later this year, we will conduct a final public meeting to provide information on and seek input from the public and city staff on the Historic Preservation Plan. We will then meet with the Landmarks Commission to present the draft Historic Preservation Plan and gather additional feedback before editing a final version of the plan to present for adoption by the City's Common Council. Historic Preservation Ordinance Historic District Standards for Review City of Madison, Wisconsin

In 2017, Legacy Architecture was retained by the City of Madison to begin a two-year-long process to create Madison's first city-wide Historic Preservation Plan, including updates to the five local Historic Districts' Standards for Review sections of the city's Historic Preservation Ordinance. To conduct this portion of the project, Legacy Architecture led a team of consultants including local historian Jason Tisch.

We began that year by familiarizing ourselves with each of the five local historic districts, including gathering and reviewing the original designation documents, touring the districts, and photographing the successes and challenges that each district presents. We then researched and reviewed case studies of other communities Standards for Review.

Throughout 2018 and early 2019, we facilitated over 18 public meetings to discuss the issues with the existing ordinance Standards for Review and proposed ordinance revisions for review standards for each district. Meeting outcomes and findings were shared with City staff and made available to the public on the city project website for the public to evaluate and gauge progress.

We have recently finished analyzing community input and drafting recommended revisions to the Standards for Review. In the coming months, we will meet with the Ad Hoc Landmarks Ordinance Review Committee to present the draft recommendations, gather feedback, and edit the recommendations before meeting with the full Landmarks Commission to unveil the draft recommendations, gather additional feedback, and make a final version of the recommended revisions to the Standards for Review.

Final results will be presented to the Common Council for consideration of adoption later this year. Background Experience & Capabilities

Lorenzen Holdings, LLC & Petes Garage Green Bay, Wisconsin

We oversaw the adaptive reuse of the historic International Harvester Company buildings in Green Bay's popular Broadway-Walnut Historic District.

The 1936 one-story portion of the building historically housed a truck sales and service garage for International Harvester. During the 1980s, the interior was subdivided and completely covered with carpeting, drywall, and suspended acoustic tile ceilings. The building's original architectural elements were re- exposed to make way for Petes Garage, a 9,000 square foot bicycle and kayak shop, named after the owners' grandfather, with concrete floors and crisp new retail display fixtures and cabinetry. A mezzanine event and lounge space overlooks the sales floor from between lofty curved bowstring trusses that were unearthed during the renovation. The center of the sales floor is washed in daylight from two reconstructed skylights.

The 1913 three-story portion of the building historically housed an agricultural machinery warehouse and office space for International Harvester. The upper levels were vacant for decades. In order to stabilize the building, the leaking roof was addressed immediately, and the roof structure was reinforced. Plywood panels on the exterior of the windows were removed. Broken panes of glass were replaced, and the historic steel windows were restored. Code issues were then corrected by inserting accessible toilet rooms on each floor and building a small addition on the rear of the building for an accessible entrance, secondary exit stair, and elevator shaft. The building now houses a women's clothing boutique , restored historic office space, and a fitness studio.

101 West Main Street Waukesha, Wisconsin

The Old Waukesha County Courthouse is listed in the National Register and is comprised of a 1885 jailhouse, 1893 courthouse, and a 1938 addition that connected the two. The Waukesha County Museum has been housed in the building since it was established in 1913. After a new courthouse was constructed in 1959, the museum shared the facility with other offices until 1993. Since that time, the museum has remained the sole tenant, leaving large portions of the building vacant. We worked with a developer to rehabilitate the landmark into a mix of uses that will allow for continued operation of the museum on the first floor and basement levels and will develop the remainder of the complex into 30 private market-rate apartments, office suites, and a banquet and events facility. We completed both the architectural design and historic tax credit applications.

We listed the S&R Cheese 111 11 11- - Company in the National Register of Historic Places for its statewide significance in the cheese making industry and also served as the architect for the facility's historic photo extensive rehabilitation into a functioning artisan cheesemaking facility. The first phase was completed in 2017 and included extensive exterior restoration, paint removal, tuckpointing, re-roofing, and improved shipping and receiving docks as well as interior moisture and structural repairs and extensive refrigeration and equipment upgrades. Remaining interior renovations will be completed in 2018 and include an experiential center for guest and customers, and new locker rooms, break room, and wellness center for team members.

Doneffs Schuette Building Manitowoc, Wisconsin

We are currently assisting the owner to rehabilitate this vacant 1902 downtown department store in the Eighth Street Historic District. We are completing both the architectural design and historic tax credit applications. The plan for this three-story, 50,000 square foot building will include retail and restaurant spaces on the first floor and apartments on the second and third floors. Major design challenges included meeting current building code requirements for egress and fire safety in this large, old building as well as making the numerous floor levels accessible. We worked to maintain the historic elements historic photo such as the hardwood floors, ornate grand staircase, and tin ceilings. Construction will begin in spring of 2018.

Mary Nohl Art Environment Fox Point, Wisconsin

Located on the Lake Michigan shore, the delightfully embellished home and sculpture garden of artist Mary Nohl is a magical art environment. Over sixty sculptures of concrete, glass, and stone populate the yard. Colorful panels of creatures and figures in profile adorn the homes exterior, while the interior contains thousands of works from a lifetime of tireless experimentation with nearly every conceivable media. In 1996, Mary began before the process of gifting her entire collection to the Kohler Foundation to ensure its preservation. By the time of her death in 2001 at the age of 87, knowledge of Nohl's extraordinary creation had traveled far beyond Wisconsin, and she had achieved international recognition, both as an artist and visionary. The property was listed in the National Register of Historic Places in 2005 for the national significance of Nohl and her art environment.

Due to the harsh lakefront environment, low quality of original construction, recent flooding, and other factors, the property was in desperate need of rehabilitation by 2015. We served as architect of record for the project, after construction, before art restoration which ended up requiring full reconstruction of the extensively deteriorated 1921 portion of the house and screen porch. The project included sitework to minimize the potential for future flooding, selective removal of trees that threatened the house and sculptures, wood siding repair, roof and window replacement, and upgrades to lighting, plumbing, mechanical systems. While not required, the Secretary of the Interior Standards for Rehabilitation guided the project.

Ongoing art conservation efforts were also begun at this time, including the stabilization of outdoor concrete sculptures and full restoration of the exterior decorative painting and art installations.

Meeme House Manitowoc, Wisconsin

The Manitowoc County Historic Society sought our help to restore a 150 year old stage coach inn. The building was moved 15 miles from its original location in the rural Town of Meeme to the society's popular Pinecrest Historical Village. The building will be restored to its 1910 appearance when it served as a hotel, meeting hall, and tavern and adapted for its new use as an interpretive museum and event space. As the building was renovated historic photo extensively over its life, unearthing its original historic elements took major investigation. Our staff had to remove modern elements to discover and find evidence of lost historic details. An addition will be reconstructed to house an educational tum-of-the-century kitchen and modern accessible restrooms.

As one of the last buildings in the downtown Sheboygan Falls National Register District that had not been restored, the owner of this building wished to bring it back to its original appearance based on an archive photo. We measured the existing building, met with the owner to develop a design concept, and prepared color façade restoration elevations. The existing storefront was removed, the masonry cleaned and repaired, and a new historically accurate storefront added.

Stefanie Weill Center for the Performing Arts Sheboygan, Wisconsin

The 1928 Sheboygan Theatre is a very fine and rare example of an "atmospheric" movie theatre. Converted into twin movie theatres in the 1980s, the theater closed in the 1990s and stood vacant for a decade until the community banded together to save and restore this downtown landmark. The "A Night in a Garden of Old Spain" themed interior was restored including its decorative walls, columns, and balconies; omate wrought iron sconces and chandeliers; and night sky ceiling with twinkling star lights. Expansion of the stage, orchestra pit, dressing rooms, and green room and upgraded seating and toilet rooms transformed the venue ShE8Cii(GAN into a cutting-edge 21st century performing arts center. -STEFAN1E H. In addition to the combined professional experience of the project team, we have worked together for the past ten years, successfully completing many large-scale architecture history projects and NRHP nominations. Several highlighted projects demonstrate this expertise including the Lincoln Boulevard Residential Historic District National Register of Historic Places Nomination, the William and Annie McDermott House National Register of Historic Places Nomination, the Holy Ghost Parish District (Dickeyville Grotto) and US Highway 51 Reconnaissance Survey. As UWM-CRM is part of a research-based public institution, we do not charge profit on our outside contracts, resulting in the maximum allocation of research funding towards project directed research. LINCOLN BOULEVARD RESIDENTIAL HISTORIC DISTRICT, NRHP NOMINATION Manitowoc, Wisconsin Between 2014 and 2018, UWM-CRM completed the Section 106 compliance process for a roadway project in the City of Manitowoc. As part of our reconnaissance survey of the project area, UWM- CRM architectural historians identified a potential residential historic district. Following consultation with the State Historic Preservation Office, UWM- CRM prepared a National Resgister ofHistoric Places nomination for the Lincoln Boulevard Residential Historic District as measure to mitigate adverse effects caused by the proposed project activity. The Lincoln Boulevard Residential Historic District is a group of 84 houses centered around a tree-lined boulevard that contains nearly every architectural style popular in Wisconsin between 1895 and 1960. The houses in the district are notable for their use of high-quality materials, and the boulevard is generously landscaped with a variety of mature trees and plantings. As part of the nomination process, UWM-CRM led several public meetings with local property owners; completed site photography for the houses and the landscape; and successfully presented the nomination at a quarterly State Review Board meeting. The Lincoln Boulevard Historic District was listed in the National Register in early 2019. FIRM EXPERIENCE & QUALIFICATIONS -HIGHLIGHTED PROJECTS 7

HOLY GHOST PAR1SH HISTOR1C DISTRICT (D1CKEYVILLE GROTTO) Dickeyville, Wisconsin

UWM-CRM identified the Holy Ghost Parish Historic District as eligible for inclusion in the National Register of Historic Places in 2016, during a Section 106 survey for an adjacent Wisconsin Department of Transportation roadwork project. The property contains the church, school, rectory, and convent of the Holy Ghost Parish in Grant County, as well as the Holy Ghost Park, which features the religious and patriotic folk art statuary and structures designed and constructed by Father Mathias Wernerus, priest of the Holy Ghost Parish from 1918-1931. The folk art installations were created using concrete forms inset with glass, tiles, and semi-precious stones, as well as found materials like shells, petrified wood, and broken crockery. Most famous of these installations is the Grotto ofChrist the King and Mary, His Mother, commonly referred to as the Dickeyville Grotto. The historic district is significant for its association with artist and creator Father Mathias Wernerus and also as an important regional roadside attraction. It is furthermore significant as an example of a Catholic church and school complex. FIRM EXPERIENCE 8( QUALIFICATIONS -HIGHLIGHTED PROJECTS 8

WILLIAM AND ANNIE MCDERMOTT HOUSE, NRHP NOMINATON Fond du Lac, Wisconsin UWM-CRM architectural historian Gail Klein completed a National Register of Historic Places nomination for the William and Annie McDermott Hosue on behalf of the City of Fond du Lac, Wisconsin. Constructed in 1899 for one of Fond du Lac's leading businessmen and city leaders, the McDermott House is an excellent representative of the Queen Anne style of architecture that was popular in Wisconsin in the late nineteenth and early twentieth centuries. The nomination required in-depth archival research, the creation of floor plan and site plan mapping, detailed photographic documentation of the building's interior and exterior, and a full narrative description of the building's existing appearance, history, integirty, and condition, as well as a complete historical and architectural context. The nominatoin was reviewed and approved by the Wisconsin State Historic Preservation Office, the Wisconsin Historic Review Board, and the National Park Service. As a result, the William and Annie McDermott House was listed in the National Register of Historic Places. FIRM EXPERIENCE & QUALIFICATIONS -HIGHLIGHTED PROJECTS 9

US HIGHWAY 51 RECONNAISSANCE SURVEY Dane County, Wisconsin

UWM-CRM conducted a reconnaissance level survey of historic properties along US Highway 51 and cross roads as part of a large scale corridor study in southeastern Dane County. The survey covered 195 historic properties, the majority of which were located in and around the City of Stoughton. As part of this investigation, UWM-CRM identified a number of properties as potentially eligible for National Register listing. The UWM-CRM team prepared 10 Determination of Eligibility reports for a variety of properties including historic tobacco farms, the Martin Luther Children's Home complex, a one-room school building, and residences exhibiting styles ranging from Italianate to Queen Anne to mid-century Ranch. As part of this investigation, UWM-CRM architectural historians photographed and evaluated all surveyed properties for National Register eligibility, created GIS mapping identifying surveyed properties and historic district boundaries, and created or updated Wisconsin Historic Preservation Database records. Survey methodology followed that outlined in the Wisconsin Historical Society's Architecture History Survey Manual. PROJECT SCOPE, METHODS & DELIVERABLES 10

PROJECT OVERVIEW The proposed project will consist of the preparation of a National Register of Historic Places (NRHP) nomination for the Downtown Commercial Historic District in the City of Black River Falls. The nomination will be prepared in accordance with the procedures outlined by the National Park Service (NPS) and the Wisconsin State Historic Preservation Office (SHPO).

METHODOLOGY The project will be completed in several interconnected phases. At the outset of the project, UWM-CRM's lead architectural historian on the project will attend an introductory public information meeting scheduled by SHPO. Following this meeting, the initial stages of the project will consist of a site visit of all properties located within the proposed historic district in order to view the properties and complete photographic documentation of the buildings and their existing settings following NPS and Wisconsin SHPO standards. Following completion of the fieldwork portion of the project, our in-house GIS specialist will produce mapping showing the historic district boundaries and locations of contributing/noncontributing buildings, as well as required topographic maps with UTM coordinates.

The fieldwork phase of the project will be followed by detailed historical research relating to the development of the Downtown Commercial Historic District and focusing on contributing buildings within the District. Research will also focus on the significance of the District within appropriate architectural and historic contexts.

Archival research will be followed by the preparation of the narrative portions of the nomination as well as the assembly of supplemental documents required by the Wisconsin SHPO. UWM-CRM staff will also update or create WHPD records for each contributing and•noncontributing property as appropriate.

Upon completion, the nomination package will be submitted to the City of Black River Falls and the Wisconsin Historical Society for review. Any requested revisions will be made within the time frame specified by the City and the Wisconsin Historical Society. Following this review, UWM-CRM architectural historians will present the district nomination at the next available Historic Review Board meeting(s) and will attend any follow-up Public Information Meetings scheduled by SHPO. DELIVERABLES Deliverables for the project include three major items: (1) completed NRHP nomination form; (2) supplemental documents required by the National Park Service and the Wisconsin State Historic Preservation Office; and (3) presentation of the completed NRHP nomination before the Wisconsin Historic Review Board. The written NRHP nomination form will be submitted both in hard copy and digitally (as a Microsoft Word document, per SHP0 requirement). Supplemental documents to be submitted with the nomination include the following: > a completed nomination checklist > a 200-300 word summary of the historic district for inclusion on the Wisconsin Historical Society's website > a CD containing digital photos in TIFF format for submittal to the National Park Service > a CD containing digital photos in JPEG format for SHP0 use > a USGS map with labeled UTM points showing the location of the historic district > District maps (GIS-based)

HAAS, PRINCIPAL INVESTIGATOR Jennifer brings nearly twenty years ofexperience with regard to cultural resources, historic preservation, archaeological/burial sites, and historic properties. In terms of administrative and project management experience, Ms. Haas concurrently manages multiple cultural resource projects, averaging six hundred per year, for the transportation, energy, development, federal (military), and utility industries that require compliance with Section 106 of the National Historic Preservation Act (NHPA), National Environmental Policy Act (NEPA), as well as state- level compliance (Wisconsin, Minnesota, Illinois, Michigan, North Dakota, and Illinois). For the utility industry, Haas is the primary point of contact and project manager relative to the on-demand cultural resources services for WEC Energy Group, Madison Gas & Electric, EDUCATION Mid-States Consultants, and Alliant Energy. In addition, Kelly developed a historic context for the Preservation Institute (2016) development of hydroelectric technology and facilities in Wisconsin. The project team also produced archival photodocumentation of the Historic Preservation Consultants hydro plant, its penstocks, and associated resources in three seasons. Seminar, Wisconsin Historical Society UWM-CRM also developed text and visual content for informational (2014— 2017) signage to be installed at the facility.

Local History & Historic Preservation Conference, Wisconsin Historical Society (2014-2017) Historic Districts Effects Assessment - Quincy, Illinois UWM-CRM conducted an effects assessment for a cell tower project Historic Preservation Conference, in the City of Quincy, along the Mississippi River in Adams County, National Trust for Historic Illinois. The project area encompassed four historic districts listed Preservation (2013) in the National Register of Historic Places: the Downtown Quincy Historic District, the East End Historic District, the Northwest Historic District, and the Southside German Historic District. The City of Quincy, due to its location along the Mississippi River, was a bustling travel center in the early-to-mid 1800s, and Quincy's residential districts contain houses that were constructed as early as the 1830s and 1840s. Kelly conducted photographic documentation, archival research, and effects assessment for all NRHP listed resources. PROJECT TEAM 16

JUSTIN MILLER, ARCHITECTURAL HISTORIAN Justin has been working an architectural historian in Wisconsin for over ten years and has been with UWM-CRM since 2014. Justin's historic preservation experience includes reconnaissance and intensive survey work, Determination of Eligibility reports, National Register nominations, and cultural resource compliance projects including consultation and developing Memorandum of Agreements. Justin's responsibilities will include responsibilities will include project management assistance, attending informational meetings, conducting fieldwork and historical research, photography, and report writing and production. GAIL KLEIN, ARCHITECTURAL HISTORIAN Gail has been working as an architectural historian and historic preservation consultant in Wisconsin since 2009 and has been with UWM-CRM since 2014. Gail's historic preservation work has included the preparation of multiple National Register nominations, intensive survey reports, historic tax credit documentation, and Determination of Eligibility reports. She has led historic preservation projects documenting and evaluating a wide range of historic property types, architectural styles, and historic districts. Gail's responsibi lities will include research and fieldwork, photography, and assisting with report writing and production. Gail lives in Stoughton, Wisconsin.

The UWM-CRM team Conference, Wisconsin Historical utilized GIS mapping to locate and identify geographically isolated Society (2009-2017) bridges, culverts, and other resources historically associated with Historic Tax Credits Workshop, the interurban railway. NICK WEBER, GIS SPECIALIST Nick has been working as an archaeologist in Wisconsin and the Midwest since 2004 and has worked with Geographic Information Systems (GIS) since 2009. Nick started working with UWM-CRM in 2014 and has been the program GIS Specialist since 2016. Nick's previous GIS projects and experience cover a wide range of geospatial fields and activities, including geospatial data production, geospatial database maintenance and data curation, predictive modeling, spatial analysis, remote sensing, LiDAR data processing and analysis, as well as spatial web-based mapping and mobile application development.

On survey projects, Nick works closely with the architecture history EDUCATION project team to streamline survey efforts and improve survey data Graduate Certificate in Geographic accuracy. Prior to any field survey work, Nick produces detailed, user- Information Systems, University of friendly survey maps. He also coordinates with the survey team to Wisconsin-Milwaukee utilize hand-held GIS data collection devices, as necessary.
