MSUM/UWEC Phonics Survey Version I
Introduction
Reading is an essential, lifelong skill. In order to understand which skills must be taught and practiced, it is important to understand with which specific reading skills readers are struggling. This is a comprehensive phonics assessment tool to identify students’ areas of strengths and weaknesses with reading different types of letter combinations. Students’ results on this phonics survey allow the instructor or interventionist to determine which phonics skills require intervention. Readers’ skill deficits guide instruction along the phonics hierarchy. Students’ should master lower-level phonics skills before moving on to more complex ones. Students may have memorized commonly used words without understanding how specific letter combinations make specific sounds. In order to attain a true understanding of a student’s phonics skills, the student’s performance reading pseudowords should also be assessed. This phonics survey includes both real words and pseudowords. This comprehensive phonics survey is matched with a brief phonics skills screener. The screener should be used first to help determine an appropriate phonics skill to start with on the comprehensive phonics survey. This entire assessment system will assist with informing instructional decisions based on student progress.

Directions
Materials needed: student copy, assessor copy, stopwatch, pen or pencil
Check that the student has adequate vision. Review the phonics survey pages prior to administration to ensure you know the proper pronunciations of all the words, especially the pseudoword pages. Have a stopwatch ready to allow the student to read the desired page for one minute. Begin with easier skills and work toward having the student progress through the skill hierarchy. Give the student copy to the student and have the assessor copy at hand, along with a stopwatch and writing utensil. Describe the task to the student as instructed below. Allow the student to read for one minute unless the child has reached the early stopping criteria. Follow along on the assessor copy as the student reads. Circle words that the student sounds out. Write a slash through words the student reads incorrectly. If possible, write in the words the student says instead. Do not mark words the student reads correctly. If a student is stuck on a word for more than 5 seconds, mark the word with a slash on the assessor copy and instruct the student to try the next word. Stop the student after one minute and mark after the last word he or she read with a bracket ( ] ). Early stopping criteria: stop the child from reading if she or he has read one row of words incorrectly, and reverse to an easier skill if you have not already assessed it.

Describing the task - say to the student: “Start at the top and read across the page from left to right (point). Some words may be easy for you and others may be difficult. Please do your best reading. Keep reading until I tell you to stop. Do you have any questions? ** Begin.” (Start timing when the student begins reading the first word.)
**If the student has any questions, answer them to the best of your ability. If the student asks if it is a test, tell them that it is a way to help you figure out how to help the student learn. If the student asks about the numbers on the side, tell them that you’re using the numbers but the student does not need to pay attention to them.
Recommendation: Have the student use a ruler or a blank piece of paper to help them keep track of where they are on the page, or you may suggest the student use his or her finger to follow along.
Scoring and Interpretation
Each reading skill is scored from 0 to 96+ in the student’s ability to read a type of word. Scores of 80 to 100+ words read correct (WRC) per minute with 95% accuracy indicates mastery of 1- to 3-syllable words. Scores of 65 to 85+ words read correct per minute with 95% accuracy indicates master of 3+ syllable words. Lower scores indicate areas in need of instruction, feedback, and practice. If students have low reading accuracy, explicit instruction in the specific skill area may be beneficial. If students have high reading accuracy but still score lower than the mastery level, reading strategies to develop fluency is suggested. Instruction should begin with the lowest number skill not yet mastered (1-49). If the student has not yet mastered the first skill in this inventory (VC, VCe), the student’s pre-reading skills such as letter-sound fluency, letter- naming fluency, and phonological awareness may need to be assessed. Follow along on the assessor sheet as the student reads from his or her sheet. If a student says the sounds in a word separately, prompt the student once to read the word all-together. Mark errors with a line through the word and, if possible, write in the word the student said instead. Circle words that the student read correctly, but had to sound out to pronounce correctly. Do NOT mark words that the student reads correctly and fluently. You may stop the student from reading once the minute is up or he or she has read one row of words incorrectly (unless you suspect the student has gaps in his or her skills, then you may want to test up to the skills expected to be mastered by the grade the student is in). Subtract the number of words crossed out in each row from six and write this number on the blank to the right of the row. Add the student’s score for each row completed in order to calculate words read correct per minute per skill area. Calculate the student’s reading accuracy per skill area by dividing the student’s total words read correct per minute by the total words read in one minute, and multiplying by 100. On the following page is an example of a score sheet that suggests that the student would benefit from explcit instruction in the CVC phonics skill, particularly with the short /u/ and /a/ sounds, assuming the student had mastered the previous phonics skills in the skills hierarchy. The student would likely also benefit from opportunities to build fluency incorporated with immediate feedback on his or her accuracy (e.g., immediately after an error say “That word is XXX, what word?”).
