Chief Executive Officer and Board of Directors Message: An “Evolution to Excellence” Table of Contents 2016 was a monumental year for Loudoun Medical Group. LMG’s continued growth and success is a testament to the power of collaboration among physicians and administration toward common goals of quality patient care and financial success. In fact, LMG’s quality scores, as measured by CMS and other payors, are nearly 100%. Chief Executive Officer and Board of Directors Message 1 Furthermore, LMG achieved its best financial position in its history during 2016. LMG has more than doubled in size since the current Management Team was formed in 2011. In addition to Organization Chart 2 growth, 2016 introduced a number of initiatives that support LMG Pods working together as one group for a common purpose, including the LMG Laboratory and the development of the LMG Multi-Specialty Suite on the campus of StoneSprings Hospital in southern Loudoun County. LMG will develop further initiatives in 2017 that 2016 – 2017 Board of Directors Members and Medical Director 3 leverage the size and scope of LMG’s Pods and support common goals while generating revenue. We are proud to note that all growth has been independent of a hospital system or any type of external corporate capital infusion. Practice Locations / Map 4 Reflection on LMG’s accomplishments is appealing and is certainly necessary, but we must always keep our fingers on the pulse of LMG’s preparedness for future challenges. LMG has partnered with CMS and every major commercial payor to build value-based programs that reward high-quality, low-cost care. LMG’s involvement in such programs will only deepen over time. Our focus on providing pre and post-discharge care coordination, reducing avoidable hospital Loudoun Medical Group Shareholders 6-12 readmissions, managing referrals and voluntarily participating in value-based programs are absolutely vital to our long-term success. Loudoun Medical Group Shareholder Track Physicians 13 We welcome value-based medicine as it will reward LMG for the years of investment made in our clinical, operating and information systems and enable us to increasingly capitalize on our strong relationships with managed care partners. LMG looks forward to leading the charge in 2017, while remaining dedicated to the Loudoun Medical Group Clinical Providers 14-18 fundamentals of our mission – providing patients with exceptional care. We will stay focused on growing our information systems, expanding telemedicine, integrating the principles of population health into everyday clinical workflows and completing a number of capital strengthening initiatives including the addition of new specialties Administrative Staff 19 and integration with the rapidly expanding StoneSprings Hospital market. LMG’s dedication to the community remains a core value. The LMG Charitable Foundation donated $86,500 to Charges / Payments / Expenses 20-22 local charities during 2016 as a result of events such as the LMG Pod Fair & Charity Raffle, as well as employee payroll deductions. Specifically, LMG has been a strong supporter of the Loudoun Free Clinic, where many LMG physicians have held leadership and volunteer positions over the years; Loudoun’s Habitat for Humanity; the Office Managers 23-24 coordination of LMG-wide food drives throughout the year to support area food banks in the Loudoun and Alexandria communities; and supporting such organizations as the Loudoun County Fire & Rescue Department, Employee Statistics 25 Loudoun Abused Women’s Shelter (LAWS), Loudoun Therapeutic Riding, and Backpack Buddies, whose mission is to provide nutritious meals for children and youth in the community. These are just a few examples of the dozens of programs that help expand our reach far beyond patient care in the practices.
