Dear Continued Supporter,

The Home Care & Hospice Foundation of Pike County is asking for your support. Your contribution will help Pike County Residents experience a healthy, fulfilling and meaningful life.

The purpose of the Foundation is to fill the gap that occurs between the Pike County Health Department, Home Health & Hospice and their expenses, due to; staff education and training, maintenance, and equipment for patients, and overall patient care. The overall objective of the Foundation is to provide the highest quality of Hospice and Home Health care to area citizens; regardless of their ability to pay.

Since 2006, each fall we have been campaigning for annual donations to support our Foundation. The Annual Giving Campaign has been sent to previous donors in hopes they will continue their support for another year. Enclosed is our 2017 Annual Report to show you, our supporters, evidence that your donations are being utilized to GIVE BACK to the community.

“Words cannot adequately express my appreciation to Pike County Hospice for helping with the care of my Mother, Ruth Brown. Mom was diagnosed with cancer in the bone in August of 2012. It was decided to undergo surgery to allow ambulation, lending to quality of life for as long as possible. The surgery was successful and rehab was short. Mom was back on her feet in two weeks walking independently at age 91. Radiation treatments followed by cancer medication ultimately lead us to a disappointing report on October 2, 2012. Treatment was unsuccessful and the cancer had spread. My Mother said, at that time, “God has had my life all mapped out, from start to finish, and the timing of my last breath is in his hands.” “God will have His way!” I have to say that God had His way by allowing Pike County Hospice to become involved in the care of Mom’s remainder of life. I had been involved in Geriatric Health Care for about 15 years but caring for my own Mother was different. I was faced with the emotional decisions to be made as well as the physical aspects of the situation. I have to say that I was blessed with wonderful family support but they were a distance away. My Mother and I both needed help. Hospice was the extending hand of God’s grace, giving us just what we need when we needed it. Whether it was nursing care, counsel, emotional support – they were there – at a beckon call. I truly don’t know how we would have made it through those weeks without the help of Hospice. The whole Hospice staff prepared Mom (as well as our family) for that “last breath” that our loved one took on January 10, 2013. When the end came Pike County Hospice was still there to bring closure to the experience that I (we) needed. On behalf of myself and the family of Ruth Brown I have to say thank you so much. I commend your staff for their professionalism, but, more importantly, to a family who might be facing a similar experience, Pike County Hospice loves and cares and they are there for you.” – Karyl Adam

The Home Care & Hospice Foundation was created to meet, as necessary, the gap between the agency’s income and costs, staff education, and training, equipment purchases and maintenance, facility improvements and fundraising costs, with an overall objective to provide high quality hospice and home care to every area citizen regardless of ability to pay.

The Foundation Board is made up of area citizens and staff, who study needs and opportunities, plan fundraising, and set goals and time tables for implementation of activities. The Foundation is a not-for-profit organization and operates on fundraisers and donations.

How to Donate Enable Pike County Home Health & Hospice to provide hospice care to patients and families regardless of their ability to pay. We are a not-for-profit 501(c)3, charitable organization. Any donations received are issued to assist patients and families served by Pike County Home Health & Hospice. Donations are also tax deductible and are accepted in many sorts including appreciated assets such as securities or real estate.

2017Annual Events 6th Annual Valentine Gala Valentine ChartGala AnnualTitle Profits Our 6th Annual Valentine Gala was held on Saturday, February 11th $45,000.00 at the KC Hall in St. Clement. It was a very successful event profiting $40,000.00 $40,330.71. A Prime Rib Dinner along with the well-known dessert $35,000.00 table was enjoyed by over 350 guests. $30,000.00 $25,000.00 $20,000.00 Profit The funds raised from each Valentine Gala are to help offset the costs $15,000.00 of our new facility that was built in 2011. Since that first event, we $10,000.00 $5,000.00 have been blessed with an enormous amount of support from our $0.00 community members who return each year to help us raise funds to 2012 2013 2014 2015 2016 2017 keep our Foundation operating. Nursing Scholarship Each year we present a $1,500 nursing scholarship to a Pike County resident who is pursuing a nursing career. We understand that we could not function without our nurses, and realize how compassionate of a job it is. We like to encourage residents of our community to pursue their nursing career and return to bring that knowledge and compassion back to our community. This year’s recipient was Jessica Grummel pictured right with Administrator, Chris Deeken. Volunteer Appreciation Dinners Each spring and fall we host a dinner for our Hospice Volunteers. We do this to help show our gratitude and appreciation for all that our volunteers do to help our patients and their families. In 2017 we held an appreciation dinner in April and in December. Both boasted a bountiful dinner, desserts, and gifts for those we appreciate so much.

23rd Annual Hospice Memorial Service This year’s service “A Path of Golden Memories” was held at the Church of Nazarene in Bowling Green on Thursday, October 12th, 2017 at 6:00 p.m. Appetizers, cupcakes and cider were served for those 60+ in attendance. Pike County Hospice staff and volunteers helped with encouraging words, memorial messages, songs, gifts and more. The program was completed with a final memorial video and a closing prayer by Hospice Chaplain Brother Don Amelung. Our next Memorial Service is tentatively scheduled for Thursday, October 11th, 2018.

Thanksgiving Dinners Hospice staff and volunteers worked tirelessly in our kitchen to make more than 45 home made Thanksgiving meals this November. All the trimmings were served; turkey, dressing, cranberry sauce, green beans, rolls, mashed potatoes, gravy, dressing, and pies. These meals were delivered hot and fresh to those Home Health & Hospice patients and their families who were home bound during the Thanksgiving holiday.

Children attending ChartCookies Title & Cocoa with Santa 8th Annual Cookies and Cocoa with Santa 700 Saturday, December 2, 2017 we had 588 children and their families 600 walk through the front doors of our facility, and walk out with gifts, 500 snacks, and a chance to visit with Santa and one of his reindeer. NEW to 2017 Camp Horizon! Friday, December 8th, 2017 we invited the public to our first Camp Horizon event. “Healing is a new dawn away... Bereavement Mini-Camp for our Youth.” This was a time to gather and remember our loved ones who have died. Children and teens learned healthy ways to cope, while enjoying crafts, snacks and more. This mini-camp was hosted by our Hospice Social Worker, Anna Wilson and our Hospice Chaplain, Brother Don Amelung. Camp Horizon was free to the public, with a mission to support the youth of our community in their grieving process. With over 20 in attendance, we are proud to say that this event was a success and we plan to continue it in the future.

2nd Annual Veteran Appreciation Fair & Flag Dedication Ceremony We had double the attendance at this years fair held on Thursday, November 9th, 2017. Our dedication ceremony as a full program with the local VFW, bagpipes, trumpet, guest speaker’s, singers and more. This year we had multiple raffles, more giveaways and were able to offer free breakfast again thanks to the wonderful donations from within our community. Newsletters By the end of 2017 our monthly newsletters have reached a circulation of over 800 recipients. Our E-News is received by more than 120 people. This newsletter is free to the public, and can also be picked up in the lobby of our office. A collaboration of the Foundation and the agency, it is stocked full of recent news, updates, reports, spotlights and much more! 2 Compassionate Giving

This Foundation could not exist without the compassionate giving of our community. Without that sup- port, we would not be able to GIVE BACK in the ways that we do. From patient care to events, we are always trying to find new creative, compassionate ways to “repay” these donations. This Foundation is supported with the help of the Annual Giving Campaign and Memorial Donations. Chart Title

Memorial Donations Annual Giving CampaignDonations THE ANNUAL GIVING CAMPAIGN is our way of raising funds to assist our home health and hospice patients. These donations help our patients within these programs. Each year, information on the Foundation, as well as donation forms and other materials are sent to those who have made donations previously. This is our way of offering the opportunity to continue the compassionate giving.

MEMORIAL DONATIONS are made by persons who want their donation to be made in memory of a lost loved one. These donations typically come from funeral visitations, general mail, or in the form of the trees that make up our memorial border surrounding our office building. In 2017 a new memorial was debuted at the 6th Annual Valentine Gala, where donations were accepted and in return, a picture of a loved one was displayed by candlelight at the well attended event. We have since continued this tradi- tion, and it grows each year.

“I have been a recipient of the services more than once from Pike Co Home Health, as well as my late husband, Darrell Mercer. Darrell also received Hospice care. The services they provided were so important in helping me regain my strength after lengthy hospital stays. When Darrell received home health, it not only helped him (and it helped him a lot), but it helped me, too, physically and emotionally. Above that, this staff gave so much more -- respect, interest in our daily lives, great care, kindness, dedication, and professionalism. I will be forever grateful and highly recommend Pike County Home Health and Hospice. 3 Compassionate Giving 65% of Donations Received: Given Back to Our Community

In 2017, of all the funds that were donated to our cause, 65% of that was given directly back to our community. This was done so through many ways:

- Patient Medical Needs: For those items that are not covered by Medicare/Medicaid, some things are still needed by a patient and are covered by the Foundation when a patient is unable to afford them. 35% of Donations Received: Operational Expenses

We can’t have a steady Foundation without the typical operating expenses. In 2017, of all the funds that were donated to our cause, 35% of that was used for operational expenses. These expenses include, but are not limited to, print materials, office supplies, postage, and professional fees.

Bereavement Support Did you know Pike County Hospice is the only hospice provider in our area that is faith-based, and that offers a community bereavement program? Our Foundation assists in the operational costs of any Bereavement Support needed for our patients’ families. Following the loss of a loved one, there is no doubt that many struggle with continuing their life with this new, heavy feeling of grief. This is a burden on many, and our Hospice program does its best to ensure that you have the support and encouragement you need by offering personal support from our Social Worker for up to 13 months after the loss, as well as a Bereavement Support Group.

Our Hospice Social Worker, Anna Wilson, LCSW (pictured right) helps the patient and family during the end of life process, as well as after, to understand that grief is normal, and that the emotions felt are also normal. This is done on a one-on-one basis personally for the patient’s family.

The Bereavement Support Group meets on the first Monday of each month in our Community Conference Room at 6 p.m. It is lead by our Hospice Chaplain, Brother Don Amelung (pictured right). He understands that everyone must deal with grief and bereavement, but also believes you do not have to do it alone. Pike County Hospice In 1993 Pike County Hospice was established, and since then we have been caring for the terminally ill within our community with pride. Hospice is a specialized type of care for those facing a life-limiting illness, their families and their caregivers. This special care addresses the patient’s physical, emotional, social and spiritual needs. It takes place in the patient’s home or in a home-like setting and also assists the patient’s family caregivers. Hospice care concentrates on managing a patient’s pain and other symptoms so that the patient may live as comfortable as possible and make the most of the time that remains. Pike County Caring for our Hospice Communities since 1993 Become a Pike County Hospice Volunteer Hospice volunteers come from all walks of life and bring comfort and companionship to individuals with life-limiting illnesses. Serving people within your community, Pike County Hospice Volunteers provide much needed support to our patients, their families and caregivers. Those who are interested may volunteer as much or as little time as possible and will be given a training program that will provide guidance, knowledge and skills which will prepare the volunteer to offer invaluable end-of-life care. Pike County Home Health In 1969, 21 years after the grand opening of our Public Health Department, Pike County Home Health Services were officially offered to our community. Home Health is care provided in a home or residence during a time of acute onset or worsening of a condition that requires a nurse or therapist (physical, occupational, or speech).

Home health care is a wide range of health care services that can be given in your home for an illness or injury. Home health care is usually less expensive (most covered with Medicare and Medicaid) , more convenient, and just as effective as care you get in a hospital or skilled nursing facility.
