Job Summary:

The Analyst IV, Quality Care Integration Coordinator, is assigned to the Health Promotion and Healthcare Quality Improvement (HPHQI) Section of the California Department of Public Health (CDPH), Sexually Transmitted Diseases Control Branch (STDCB). Under the general direction of the HPHQI Section Chief, the Quality Care Integration Coordinator will play a leadership role in helping reduce the incidence and prevalence of chlamydia and other STDs in California by promoting the scale up and integration of evidence‐based STD quality healthcare services into primary care settings statewide. Essential to this overarching role, the Quality Care Integration Coordinator will serve as an expert to the STDCB on federal and statewide changes related to healthcare reform affecting the landscape of healthcare in California, including new mandates, programs, resources and applications of the Affordable Care Act. Additionally, this position will serve as the STDCB liaison to and primary care stakeholder groups, facilitating ongoing communications and collaborations between these groups and all Branch sections. This position will also serve the Branch by tracking new grant announcements applicable to STD prevention and control efforts, coordinating applications to funding opportunities that strategically advance the STDCB’s mission. Light travel within California may be required.

Essential Duties and Responsibilities:

50% Healthcare Service Integration Coordinator Responsible for identifying and leveraging various mechanisms to promote the scale up and integration of evidence‐based STD‐related quality healthcare services statewide, including:  Identifying and leveraging federal program clinical quality improvement requirements, especially within healthcare reform;  Initiating and leveraging partnerships with healthcare organizations, including managed care, Medi‐Cal, federally‐ funded community health centers (CHCs), federally‐qualified health centers (FQHCs), state medical associations, health insurance organizations, and other primary and preventive care stakeholders;  Seeking outside funding opportunities to support these efforts, as applicable.

Responsible for seeking and utilizing strategic opportunities to enhance awareness and obtain buy‐in by targeted healthcare provider groups for STD prevention, treatment, and clinical management recommendations and the integration of best practices. These opportunities may include:  Presenting at key meetings and conferences;  Drafting articles for medical association newsletters and similar outlets;  Participating in discussions with medical directors and administrative decision‐makers at healthcare insurance and delivery organizations to: - Share data identifying gaps and highlighting training or technical assistance needs; - Outline benefits of integrating new quality STD services; - Request participation in demonstration projects for new interventions or needs assessments; - Offer technical assistance, planning, and implementation support, including enhanced support from expert STDCB staff and partner agencies as needed, related to the integration of new provider tools, best practices, quality improvement policies, and guideline recommendations; - Negotiate data sharing agreements for one‐time needs assessments or continuous quality assurance.  Other activities, including innovative means, that are expected to reach large provider groups or influential healthcare decision‐maker targets. Responsible for overseeing Program Collaboration and Service Integration (PCSI) initiatives for the STDCB, including collaborative work across disease areas to integrate services at the client level, especially prevention activities related to STD, HIV/AIDS, and viral hepatitis.

25% Managed Care and Primary Care Liaison Serves as a key STDCB Liaison with California managed care and primary care associations, provider groups, health insurance organizations, and related healthcare delivery stakeholders:

 Develops and enhances partnerships with public and private primary care organizations, building a thorough understanding of their organizational priorities, including competing priorities and conflicting perspectives, and how these affect negotiations related to increasing access to high quality STD services for at‐risk populations.  Serves as the Branch liaison to publicly‐funded healthcare consortia including networks of Federally‐Qualified Health Centers (FQHCs), including 330‐funded Community Health Centers (CHCs), Rural Health Centers (RHCs), and Migrant Health Centers (MHC), as well as Indian Health Service clinics, and primary care clinics operated by local health jurisdictions in California; and collaborates with these partners to promote STD services that are essential for the Branch’s mission.  Serves as a key Branch liaison to the California Primary Care Association, state and local professional medical societies, and other healthcare partners to promote and facilitate increased access to STD clinical services and STD, HIV and viral hepatitis service integration at the client‐level.  Represents the Branch, often independently, to public health professionals, private healthcare delivery organizations, medical associations, and others at meetings, professional conferences, and other forums. Responsible for coordinating and participating in collaborations with other STDCB colleagues and external partners, as applicable, on the planning, development, goal and priority setting, coordination, implementation, oversight and evaluation of initiatives to increase access to STD clinical services in primary care settings statewide.

10% Federal Program and Healthcare Reform Expertise

Responsible for building and maintaining expertise on federal and statewide initiatives affecting the landscape of healthcare in California, including new mandates, programs, resources and applications of the Affordable Care Act and other healthcare reform efforts, including how these create new opportunities and barriers a) to healthcare access for California residents, and b) to quality assurance and improvement efforts by public health and medical care agencies:

 Monitors national and state‐level health initiatives and program requirements affecting healthcare delivery systems applicable to sexual and reproductive health services, such as the Affordable Care Act, national quality measures, preventive services guidelines, training requirements, and other opportunities that could potentially be leveraged to support expanded access to quality STD clinical care services. - Identifies specific opportunities for the STDCB, partner organizations, and primary and managed care stakeholder groups to leverage for the improvement of STD clinical care. - Keeps apprised of any new healthcare barriers that will need to be circumnavigated.  Develops and maintains working knowledge of funding structures, program design, and service policies of federally qualified health centers and insurance programs and works with leaders within these care structures to increase access to STD screening and management.  Participates in national working groups to exchange expertise and brainstorm creative solutions, and as another means to keep apprised of new federal opportunities and barriers.  Keeps STDCB and key partner organizations educated and informed about federal and statewide program structures, initiatives, opportunities and barriers, and provides recommendations for strategic direction and prioritized action related to these, as requested.

10% Grant Writer and Coordinator

 Tracks federal and foundation grant announcements applicable to STD prevention and control efforts; determines eligibility, timelines, and fit; shares announcements with STDCB staff and strategic partners as applicable.  Coordinates the submission of HPHQI‐related grant applications for funding opportunities that strategically advance the section’s goals a) to scale up proven quality interventions, and b) to build the evidence base for new priority interventions.  Consults with HPHQI, other Branch colleagues, and strategic external partners (as applicable) to determine study/project designs, protocols, backgrounds, justifications, partners; and proposed budgets, and uses information to draft grant applications according to specific grant requirements and stated purposes.  Ensures timely and complete submissions of grant applications and renewals; serves as primary contact for funders.  Oversees IRB application submissions, drafting the proposal content as necessary, if applicable and required by grant application.

5% Branch and Section Administrative Support

 Prepares, drafts, and edits to finalize a variety of program documents that include narratives and program reports; program correspondence; project statistical summaries; memoranda or agreements and contractual scopes of work with project partners.  Represents the STDCB at Branch meetings, professional conferences, meetings with CDPH leaders, and other forums.  Participates on STDCB Management Team.  Responds to state drills, as needed.  Provides program assistance not considered part of the routine assignment, including a variety of educational, professional liaison, communication and other tasks.  Other duties as assigned.

Minimum Qualifications:

 Possession of a Master’s Degree in Public Health, Social Work, Public Policy, or a closely related professional field.

 At least two years of experience working in the field of healthcare service delivery, health policy, healthcare financing, health service evaluation, managed care clinical policy development, or a related field.

 Proficiency using Microsoft Office 2010, including Outlook, Word, Excel, and Access.

 Experience working across disciplines and managing multiple projects simultaneously.

 Experience preparing grant applications and reports.

 Experience drafting and presenting professional talks at conferences, meetings or other public forums.

 Experience establishing and maintaining effective working relationships with professional health personnel, professional health organizations, government officials or similar  Excellent written and verbal communication skills in English.

 Group facilitation, diplomacy, and negotiation skills.

 Ability to reason and problem-solve analytically, logically, and creatively.

 Ability to effectively and efficiently work on multiple tasks and projects, work well under pressure, and to work both independently and collaboratively.

 Ability to work with excellent initiative and with minimum supervision.

 Knowledge of public health, including clinical applications, an understanding of surveillance and epidemiology, primary prevention, and health education.

Preferred Qualifications:

 Experience working within a clinical setting.

 Two years of experience working in a local, state, or federal STD, hepatitis, tuberculosis, teen pregnancy prevention, family planning, or HIV program, with a background in health policy, program planning and coordination, or similar

 Knowledge of facilitators and barriers that affect healthcare providers’ adherence to STD screening, timely treatment and partner service guidelines.

 Knowledge and experience in healthcare systems, including financing mechanisms; approaches to developing, promoting and enforcing clinical policy decisions; and methods of reimbursement for preventive services.

 Understanding of the science behind national guidelines for clinical preventive services and familiarity with methods of monitoring adherence to guidelines, including HEDIS measures.

 Knowledge of healthcare reform proposed mandates and applications.

 Knowledge of the structure of healthcare organizations, including the various forms of managed care, and understanding of how preventive health issues are address in primary care settings.

 Knowledge of Program Collaboration and Service Integration (PCSI) efforts at the national level and how these efforts can be implemented at the state and local levels.

 Understanding of California epidemiology, screening and diagnostic approaches, treatment recommendations, and prevention strategies related to STDs.

 Knowledge of federal and state laws pertaining to state and local public health programs.

 Knowledge of California Department of Public Health functions, policies, and procedures and its participation in public health programs.
