RECOMMENDA nONS:

A. That the City Couucil designate the unnumbered location situated one sidewalk square northerly of and between numbered locations 77H and 78h as shown on Sheet # 17 of Plan D-13788 for the for the installation of the name of Sally Field at 6767 Hollywood Boulevard.

B. Inform the Hollywood Chamber of Commerce of the Council's action on this matter.

C. That this report be adopted prior to the date of the ceremony on May 5, 2014.

FISCAL IMPACT STATEMENT:

No General Fund Impact. All cost paid by permittee.

1. Unnumbered communication dated April 7, 2014, from the Hollywood Historic Trust of the Hollywood Chamber of Commerce, biographical information and excerpts from the minutes of the Chamber's meeting with recommendations. City Council -2- C. D. No. 13

DISCUSSION:

The Walk of Fame Committee of the Hollywood Chamber of Commerce has submitted a request for insertion into the Hollywood Walk of Fame the name of Sally Field. The ceremony is scheduled for Monday, May 5, 2014 at 11:30 a.m.

The communicant's request is in accordance with City Council action of October 18, 1978, under Council File No. 78-3949. Following the Council's action of approval, and upon proper application and payment of the required fee, an installation permit can be secured at 201 N. Figueroa Street, 3rd Floor, Counter 23.

The Walk of Fame Committee of the Hollywood Chamber of Commerce has approved the below listed name for insertion into the Hollywood Walk of Fame:

SALLY FIELD-(Category -MOTION PICTURES) Requested star location to be 6767 Hollywood Boulevard north side of the street in front of the Hollywood Wax Museum. The star for SALLY FIELD will be placed in the second row next to the original row closest to the property. The stars situated in the original row where star is situated are RAP AEL MENDEZ(78h) to the west and EDGAR BERGIN (77H) to the east. According to sheet # 19 plan 13788, SALLY FIELD'S star is next to two blank squares one on the (East) and the other on the (West) and have no designated numbers) in the second row. STAR TO POINT WEST. The dedication ceremony is scheduled for Monday, May 5, 2014 at 11:30 p.m.

SALLY FlELD

Sally Field, a two-time Academy Award winner, most recently starred in Steven Spielberg's critically acclaimed Lincoln as Mary Todd for which she won the Critics award for best supporting actress and was also nominated for a Golden Globe, , Critics Choice, BAFTA and Academy Award. Field previously won for her performances in Robert Benton's , for which she also received a Golden Globe, and 's Norma Rae, for which she received a Golden Globe, along with the New York Film Critics prize, the National Board of Review Award, the Los Angeles Film Critics Award, the National Society of Film Critics honor and Best Actress at the Cannes Film Festival.

Ms. Field has also received Golden Globe nominations for her work in Smokey and the Bandit, Absence of Malice, Kiss Me Goodbye, Steel Magnolias and Forrest Gump. Her many film credits include An Eye for An Eye, Mrs. Doubtfire, Soapdish, Not Without My Daughter, The End, Hooper, Stay Hungry (her first major film role), as well as Punchline and Murphy's Romance, beth of which were produced by her production company, Fogwood . In 2012, Ms. Field appeared as Aunt May in The Amazing Spiderman and will reprise her role in The Amazing Spiderman 2 scheduled for release in 2014.

Born in Pasadena, California and raised in a show business family, Ms. Field began her career in 1964 in the television series Gidget. She went on to star in the The Flying Nun in 1967. She starred in three television series by the age of twenty-five. She received for her title role in the landmark miniseries Sybil and for her performance on ER. She also received Emmy nominations for her role in Showtime's A Cooler Climate and the NBC miniseries A Woman of Independent Means which she co-produced and for which she received a Golden Globe nomination. Ms. Field co-starred in the ABC series drama Brothers & Sisters from 2006 to 2011 and for her role as "Nora Walker," Ms. Field received a Screen Actors Guild Award and an Emmy Award as well as two Golden Globe nominations.

Ms. Field made her directorial debut in 1996 with the ABC telefilm The Christmas Tree which she co-wrote and which starred . She directed an episode of the HBO miniseries From the Earth to the Moon" and in 2000 made her feature fihn directorial debut with Beautiful starring Minnie Driver.

In 2002, Ms. Field made her Broadway debut in Edward Albee's The Goat and in 2004, received rave reviews for her role as "Amanda" in Tennessee Williams' The Glass Menagerie at The Kennedy Center.

Ms. Field has served on the Board of Directors of Vital Voices since 2002. She has served as Mistress of Ceremony at Vital Voices Global Leadership Awards gala held at The Kennedy Center from 2002 through 20011. She also served on the Board of Directors of The Sundance Institute from 1994 to 2010.

Sally has three sons and four grandchildren. ###
