The knowledge to action framework organizes this bench to bedside process through three phases including: research, translation, and institutionalization without being specific to one disease or condition. The overall goal of this research is to bridge gaps in the translational process from assay development to disease detection through a mixed methods approach. A literature review identifies gaps associated with intestinal permeability and autism spectrum disorders. Mining social media related to autism and GI symptoms captures self-reported or observed data, identifies patterns and themes within the data, and works to translate that knowledge into healthcare applications. -

ABSTRACT Gluten-sensitive enteropathy, called Celiac disease (CD), is one of the most frequent autoimmune diseases, occurring in 1% people worldwide, upon gliadin ingestion. Currently, the only treatment available for CD individual is a strict life-long gluten-free diet. Chicken egg yolk immunoglobulin Y (IgY) is produced and examined for its efficacy in vitro, ex vivo, and in vivo to prevent enteric absorption of gliadin. This antibody was also used to develop sensitive and rapid detection kits for gluten. The extracted toxic gliadin was immunized into chickens inducing humoral immune response to produced gliadin-specific IgY antibodies. -

Characterized by an adverse reaction to gluten protein, celiac disease manifests in the small bowel and results in inflammation and increased permeability of the gut barrier. This is followed by an immune response mounted against not only gluten, but also tissue transglutaminase 2 (TG2), a gluten-reactive enzyme secreted by intestinal epithelial cells. Despite the growing number of individuals affected by this disease, the only reliable intervention strategy available is lifelong adherence to a gluten-free diet. Novel strategies for treating or preventing celiac disease include synthetic pharmaceuticals to modify the gut barrier, parabiotic infection to reduce inflammatory cytokine release and administration of a synthetic polymer that binds gluten proteins and prevents their digestion and absorption. -

Despite its prevalence, the only treatment currently available for celiac disease is complete avoidance of gluten proteins in the diet. Ongoing clinical trials have focused on targeting the immune response or gluten proteins through methods such as immunosuppression, enhanced protein degradation and protein sequestration. Recent studies suggest that polyphenols may elicit protective effects within the celiac disease milieu by disrupting the enzymatic hydrolysis of gluten proteins, sequestering gluten proteins from recognition by critical receptors in pathogenesis and exerting anti-inflammatory effects on the system as a whole. This review highlights mechanisms by which polyphenols can protect against celiac disease, takes a critical look at recent works and outlines future applications for this potential treatment method. We have previously shown that the BL-7010 copolymer poly(hydroxyethyl methacrylate-co-styrene sulfonate) (P(HEMA-co-SS)) binds with higher efficiency to gliadin than to other proteins present in the small intestine, ameliorating gliadin-induced pathology in the HLA-HCD4/DQ8 model of gluten sensitivity. The aim of this study was to investigate the efficiency of two batches of BL-7010 to interact with gliadin, essential vitamins and digestive enzymes not previously tested, and to assess the ability of the copolymer to reduce gluten-associated pathology using the NOD-DQ8 mouse model, which exhibits more significant small intestinal damage when challenged with gluten than HCD4/DQ8 mice. -

The rise in incidence is also due to a real increase of this immune-based disorder, independent of disease detection. The reasons for this true rise in recent decades are unknown but may be related to environmental factors that may promote loss of tolerance to dietary gluten. Strategies to reduce the development of celiac disease have not been proven successful in randomized trials, but the quantity of early-life gluten exposure has been a major focus of prevention efforts. The criteria for the diagnosis of celiac disease are changing, but in adults, diagnosis still depends on the presence of duodenal villous atrophy while the patient is on a gluten-containing diet, along with findings from serology analysis. Although guidelines in the United States continue to mandate a biopsy at all ages, some children receive a diagnosis of celiac disease without a biopsy.
