Dear [INSERT NAME]

Important information – Your wheelchair service provider is changing

I am writing to inform you that from 1 April 2018 a new wheelchair service will be provided across Barking and , Havering and Redbridge. This new service will be run by AJM Healthcare, a specialist provider of mobility equipment.

The wheelchair service you receive will continue uninterrupted. The main change for you to be aware of is that should you need any support, from 1 April you will need to contact AJM Healthcare. Details of the new service, including opening hours and the telephone number for the service and out-of-hours emergency repairs are on the back of this letter.

If you are currently on a waiting list for an assessment, awaiting a delivery or a repair your details will be transferred securely to AJM Healthcare who will contact you soon after 1 April.

Your GP and/or social worker will be informed of these changes.

Our priority is to ensure that you continue to receive an uninterrupted wheelchair service, and so your care will be automatically transferred to AJM Healthcare from 1 April.

Why is this happening?

North East NHS Foundation Trust’s (NELFT) contract to provide wheelchair services is due to expire at the end of March 2018. After a comprehensive procurement exercise, Barking and Dagenham, Havering and Redbridge Clinical Commissioning Groups appointed AJM Healthcare to provide a new wheelchair service across the three boroughs.

AJM Healthcare will provide a full service that includes clinical assessment, clinical support and wheelchair equipment. NELFT and DGT Services Ltd (DGT) will no longer provide wheelchair services in Barking and Dagenham and Havering.

What do you need to do?

To enable your care to be safely transferred from NELFT and DGT to AJM Healthcare, your clinical records will need to be shared. Sharing your records with AJM Healthcare will allow them to:

 offer you uninterrupted care  respond more quickly to requests for help  contact you if there are known problems with your wheelchair  contact you when a review of your wheelchair is due.

Only the records relating to your wheelchair will be shared with AJM Healthcare. If you receive any other services from NELFT or DGT, these records will remain unchanged and will not be shared with anyone apart from your GP.

If you are happy for your records to be shared with AJM Healthcare, you do not need to do anything. Your records will be automatically transferred at the end of March.

How will your records be transferred?

Your electronic records will be transferred securely between:

 NELFT and AJM Healthcare  DGT and AJM Healthcare

The data in your records will be encrypted and sent via secure email to AJM Healthcare where it will be uploaded to their computer systems. Your data and records will not leave the secure NHS email system.

What if you don’t want your records to be transferred?

If you don’t want your records to be transferred to AJM Healthcare, you need to contact NELFT and DGT directly to tell them. If they don’t hear from you by Monday 12 March 2018 your records will be automatically transferred to AJM Healthcare. If you don’t want your records to be transferred, you will need to register your equipment yourself with AJM Healthcare. Contact details for AJM Healthcare, NELFT and DGT are on the back of this letter. Once your data has been transferred to the new provider you are always at liberty to request (provided your identity is proved along with photo ID) information held about only yourself from AJM Healthcare. This is known as a Subject Access Request, (under Section 7 Data Protection Act 1988). All requests MUST be in writing to AJM Healthcare.

If you have any questions or queries about the new wheelchair service, please don’t hesitate to get in touch with AJM Healthcare.

We wish you all the best with your ongoing care.

Yours sincerely
