Miranda Kavanagh Director of Evidence Evidence Report – Lessons from historical dam incidents iii Executive summary This report aims to help those responsible for the safety of reservoirs. These include engineers appointed under current legislation, personnel who visit reservoirs in the course of their duties, staff who operate and monitor reservoirs, and enforcement authority engineers. The scope of the report is limited to water-retaining structures: some types of waste impoundments, such as tailings dams, may suffer similar types of malfunction, but these are not included. Although the focus of the report is on incidents at dams in Great Britain, reference is made to a few international incidents. International experience is particularly helpful for those types of dam that are not commonly found in Great Britain. The report begins with an introduction in Section 1. The background to the subject is briefly outlined, the value of the national incident database is demonstrated and the need for post-incident reporting and investigation is emphasised. In the next two sections, general, technical and regulatory lessons from dam incidents are outlined. Section 2 gives a historical overview of the subject which shows how serious incidents have improved our understanding of dam behaviour and the hazards posed by these structures. This section should not only be of interest to dam engineers but should also help those reservoir owners with limited technical knowledge to develop a basic grasp of the more significant aspects of the subject. Section 3 shows the close links between historical incidents and failures and the development of reservoir safety legislation and guidance. Section 4 looks at how incidents have been managed, including the role of owners and panel engineers. The significance of drawdown rates and other provisions for dam incidents such as evacuation planning are presented. Some examples of incident management are described. Section 5 begins with an overview of serious incidents and a classification and brief analysis of the modes of failure. This is followed by descriptions of over thirty major incidents and summaries of seventy other incidents. Maintaining reservoir safety has considerable importance for the public in a country such as Great Britain where a number of dams pose a high hazard, being located upstream of heavily populated and industrialised areas. Thus, although the probability of failure of a dam is generally low, the consequences of failure could be great. As most reservoirs constitute a low probability/high consequence scenario, careful management of these risks is essential. Fortunately, few catastrophic failures have occurred in Great Britain and since 1925 there has been no loss of life due to dam disasters. Table 1.1 lists dam failures that caused loss of life in Great Britain. All the dams are embankments except Eigiau which was concrete and failed due to an inadequate foundation. Since 1925, there have been failures involving breaching of embankments and also many ‘near misses’ and other serious incidents (Wright, 1994). Much can be learned from these failures, particularly those such as Baldwin Hills, Malpasset, Teton and Vaiont which have been the subject of detailed investigation and substantial literature. A useful start to such a study is given by Jansen in his book Dams and public safety (Jansen, 1980) which includes illuminating accounts of the failures of Machhu II, Teton, Frias, Baldwin Hills, Vaiont, Babii Yar, Malpasset, and Vega de Tera as well as of many other failures.

    Paddlesteamers, Postcards and Holidays Past SITE VISIT – LYNTON The Valley of Rocks Hotel was built in 1807 by Lynton businessman William Litson. It was as elaborately decorative inside as it was outside. Litson had made a fortune through buying Exmoor wool and having it spun locally before selling it to weavers in Barnstaple. By the 1790s, however, the the spinning trade had been mechanised, and it was no longer a profitable enterprise for Litson. Diversifying, he built the Globe Inn as a hotel, and furnished the adjoining cottages for visitors. The Valley of Rocks Hotel followed. Litson's guests included the Marchioness of Bute, and Mr Coutts the banker. It also boasted landscaped gardens with a fine view of the Bristol Channel – the perfect place for Victorian visitors to promenade in the healthy sea air. At the start of the nineteenth century, access to Lynton was not easy. An 1825 Guide to All the Watering and Sea Bathing Places said: "A few years ago this place [Lynton & Lynmouth] was known only as a fishing creek: the roads to it were impassable and the only place of public accommodation was a miserable ale house." HOTEL WARS All that changed when William Sanford of Somerset's Nynehead Court built himself a summer residence at Lynton and set about improving the roads. By 1830, too, the first steamer carrying passengers up and down the Bristol Channel was stopping off at Lynmouth and rowing visitors ashore. Suddenly Lynton and Lynmouth were very fashionable places to visit, and local businessmen were keen to keep it that way.

This earthquake devastatingly took the lives of 185 people and caused widespread damage across Christchurch and the Canterbury region. Since the February earthquake there has been 15,832 quakes in the Canterbury region. The impact of the earthquakes has resulted in ongoing social, material and political change which has shaped how everyday life is experienced. While the Christchurch earthquakes have been investigated in relation to a number of different angles and agendas, to date there has been a notable absence on how older people in Christchurch are experiencing post-disaster recovery. This PhD research attends to this omission and by drawing upon geographical scholarship on disasters and ageing to better understand the everyday experiences of post-disaster recovery for older people. This thesis identifies a lack of geographical attention to the emotional, affective and embodied experience of disaster.

Ngan-Tillard Delft University of Technology July 2019 Teodora Barbuntoiu | Geological considerations in dams failure | AESB3400 Bachelor Thesis Abstract Dams and reservoirs pose safety concerns to society worldwide. In case of a disaster, the water impounded in the reservoir escapes and destroys everything in its path. Reasons for failure range from geology, hydrology and seismicity, to design problems, lack of maintenance and poor field investigation. Prior cases show that various dams gave away mainly due to geological causes, so there is a particular interest to see how the local terrain features could influence the longevity of the structure. Three historical case studies are discussed in order to emphasize the impact of geology regarding dam failure. The Saint Francis Dam is a prime example of poor site investigation, where the lack of knowledge on the foundation rock led to the rupture of the gravity dam. The Malpasset Dam gave away predominantly due to underestimated effects of the uplift, nevertheless, the geologists were unaware of an active fault system and the mechanical properties of the rock mass. The Baldwin Hills Reservoir comes with a more thorough site investigation, yet still, due to earth movements, the water from the reservoir infiltrated through the embankment. Therefore, geological features at the site need to be included in the design options of the dam in order to ensure a safe, feasible and economical project.

We appear to have an issue with Knotweed near to Anglezarke reservoir and car park. Is there anything we can do about this? Response from the Open Space Strategy Officer: The knotweed would be for the landowner to treat or the members of the parish council once they have the training with the permission of the landowner. Two people from Rivington PC are booked on the upcoming course. 2. Additionally, there is a big issue with anti-social behaviour, dangerous driving and apparent drink/drug taking at viewpoint. The residents living near to viewpoint have expressed particular concern over the speed of the drivers on the country road. Ideas suggested include: Could we put up CCTV & would this be sufficient evidence for prosecution? Could we request an occasional police presence to discourage them? Response from Public Protection: The Councils’ powers are not appropriate for dangerous driving and drug taking, these are direct offences which the Police powers are more appropriate. We do not have the power to identify offenders, we would reply on intel of partners such as the police or members of the public to identify in order for us to consider any appropriate action.
