Bucks County The licenses of David W. Faust, F.D., funeral director license number FD012933L, and funeral supervisor license number FS012933L, of Doylestown, Bucks County, were revoked and he was ordered to pay civil penalties totaling $90,000, based upon his convictions of crimes of moral turpitude that were related to the profession of funeral directing. Delaware County Theodore F. Hawkins, Jr., F.D, license nos. FD012105L and FS012105L, of Chester, Delaware County, was ordered to pay a $10,000 civil penalty and to cease and desist from practicing as a funeral director and funeral supervisor without current licenses, and for violation of a lawful disciplinary order of the State Board of Funeral Directors. Montgomery County Timothy Leslie Walley, Jr. of Harleysville, Montgomery County had his massage therapy license denied based on not being able to practice massage therapy with reasonable skill and safety by reason of mental or physical illness or condition or physiological or psychological dependence upon alcohol, hallucinogenic or narcotic drugs or other drugs which tend to impair judgment or coordination.
