1 The Supporting People mission

To work in partnership with Local Government, Probation,

Health, voluntary sector organisations, housing associations, support agencies and service users to help vulnerable people to live more independently and maintain their tenancies.

To give people the choice to receive support in their own home or to move into supported accommodation.

To ensure the services provided are of high quality, are cost effective and meet the needs of service users.

2 What is supporting people?

Supporting People is a working partnership of local government, service users and support agencies, which offers vulnerable people the opportunity to improve their quality of life.

4 Support for Young People (includes young families)

Support is provided in a variety of ways for young people who may be homeless or need to leave home for the first time. Most schemes that provide accommodation can be applied to directly.

Every effort is made to help people with particular problems and this may include finding the best place in Sandwell to help them.

“Foyers” offer structured programmes of education and training as well as independent living skills. A number of schemes are able to help young people move from a scheme with others to their own property, and continue to offer support if needed.

There are also services for young parents.

Service Description: Supported accommodation for young single homeless people, ex-offenders and those deemed at risk of offending, aged 18+. Maximum stay up to two years, with the emphasis on resettling people who use the service, back into the community.

Service Type: The service supports young families living in the Sandwell area to develop skills needed to live independently and maintain their tenancies, thereby reducing the risk of tenancy failure.

Service Description: Each service user will have their own floating support worker who will provide weekly keywork sessions in the service users’ home. Referrals to the scheme will be vulnerable young families aged 16 – 29, who live in the Sandwell area, have identifiable support needs, and are willing to engage and commit to the support offered.

We have a multi-agency approach with our services, and facilitate regular steering group meetings. The main agencies working with the service are Sandwell’s Community Care Team, Sandwell Homes, and Sandwell’s Primary Care Trust (mainly health visitors and midwives).

The support can be provided for up to two years, however generally the support is only needed for 9 – 12 months.

Accessing the service: Referrals are accepted from any agency or landlord. Self-referrals are also accepted. To apply please contact the above number for a referral form.

7 Bromford Support

Service Type: The service offers supported accommodation for nine young families in the Sandwell area, supporting young families to develop the skills needed to live independently, thereby reducing the risk of future tenancy failure or repeat homelessness.

Service Description: The service offers supported accommodation for nine young families in the Sandwell area, supporting young families to develop the skills needed to live independently, thereby reducing the risk of future tenancy failure or repeat homelessness. There is a support worker on site during normal working hours, providing weekly key-work sessions to each young family. The service is funded by Sandwell Supporting People to work with the nine young families living at the scheme. Referrals will be vulnerable young families aged 16 – 29, who are in housing need, have identifiable support needs, and must be willing to engage and commit to the support offered. We have a multi-agency approach with our services, and facilitate regular steering group meetings. The main, agencies working with this scheme are Sandwell’s Community Care Team, Sandwell Homes, Sure Start, and Sandwell’s Primary Care Trust (health visitors and midwives).

8 Midland Heart

Service Description:

Gilbert Road provides semi-supported housing for 17-25 year olds who are ready to practice living independently.

We provide 14, one bed flats with kitchen, living room and bathroom. They are part furnished with carpets, curtains, cooker and fridge. They have their own separate entrance, access to communal garden and the use of the communal laundry room.

Our young people at Gilbert Road initially have a six-month assured short-hold tenancy agreement, which can be extended as appropriate. Tenants are responsible for their own gas, electricity, water and council tax bills.

All referrals are taken through the Single Referral Panel, an accommodation provider for young people who have completed a resettlement programme and require semi support accommodation.

Service Description: Grange Road provides accommodation (nine self-contained flats) for single young people who are homeless, imminently homeless and have support needs. Customers should be aged 18–25 years. All applications will be assessed individually, but if you use alcohol, drugs or other substances you need to be engaging with a professional agency on a programme of recovery.

Our Support Worker can assist with a range of support services including support with managing your money, budgeting skills, accessing education, training, employment, and leisure. Staff will support and advise you in claiming the correct benefits to enable you to live at the scheme; these will include housing and Council Tax benefit.

Our staff team consists of the Projects Leader, Team Leader and 1 Project Worker. Our working hours are normally Monday to Friday between 8am – 4pm.

Service Description: We aim to provide safe and secure accommodation for 16 young people (16-25 years of age) in training employment or education. We will provide the necessary support and advice for our residents to achieve independent and successful lives

We have sixteen bed-sit rooms, three of which are dedicated to young people with psychosis and one is accessed through Sandwell’s Early Intervention Team.

All rooms are fully furnished (bed, chest of drawers, bedside cabinet, built in wardrobe space, central heating radiator, fridge and chair) each two units have a communal bathroom with toilet, bath and shower facilities

Kitchens and lounge are communal and are located on the ground floor at the end of the corridor from the main reception.

Early Intervention Service Murdock Road Project engages with Early Intervention Service to provide respite and housing related support to young people with low and first episodes of mental health related illnesses. The service will aim to support young people in crisis and seek to prevent hospital admission.

Service Description: Rolfe House provides accommodation for single young people who are homeless, imminently homeless and have support needs. Customers should be aged 16 – 25 years.

All applications will be assessed individually, but if you use alcohol, drugs or other substances you need to be engaging with a professional agency on a programme of recovery. The scheme consists of 24 rooms. There is access to a communal TV lounge, a training room, large IT suite with internet connection and laundry facilities. You have en-suite facilities in your room and you will share a kitchen with one other person.

Our support workers can assist with a range of support services including support with managing your money, budgeting skills, accessing education, training, employment, and leisure. Staff will support and advise you in claiming the correct benefits to enable you to live at the scheme; these will include housing and Council Tax benefit.

Our staff team consists of a Project Leader, Team Leader, four Day Support Workers and two Night Support Workers. Rolfe House Foyer is open and staffed 24 hours a day, 365 days a year.

17 Trident Reach the People Charity

Service Description: Trident Reach is a charity that is dedicated to supporting people within the community. Trident Reach Young People services are currently based across the East and West Midlands in locations such as Sandwell, Birmingham, Worcester and Derbyshire. Our service provision includes mother and baby services, move on accommodation, specialist services for young people who are leaving care and accessing youth offending services.

Trident Reach’s Sandwell Young Peoples Services are funded by Supporting People to provide support to 82 young people aged 16-25 across Sandwell. The support is delivered in a personalised way that is life enhancing and that reflects our passion for people. We are able to provide support to young people within the community and we also have a resource of accommodation and move on accommodation in the Cradley Heath and Wednesbury areas. The support is individualised to the needs of each person and services can be offered in different areas such as homelessness, sustaining tenancies, developing life skills to sustain independent living such as budgeting, cooking, healthy living, and supports young people into education, training employment and we have also supported young people to start their own businesses.

20 Support for people with Mental Health problems

People with mental health problems are individuals who may be differently affected, will have different needs, and are likely to experience periods of illness and recovery.

These services provide both short and long term support, both in schemes and clusters and to people living in their own homes across Sandwell.

There is also support available specifically for people who are Asian or African Caribbean. Support workers work closely with the Community Mental Health Teams to help provide a service that meets health needs as well as helping people to live independently.

22 Sandwell Women’s Service

Service Name: P3 Sandwell Women’s service

Service Description: It has two distinctly separate functions. Seven rooms are designated for the accommodation of clients with mental health difficulties who require tenancy related support. Women accessing this service have a comprehensive assessment of need and are then involved in the formulation of a support plan designed to address these needs. This plan is then followed and regularly reviewed with the aim of promoting independence in the future.

Access to the remaining three rooms is managed jointly by P3 and Sandwell Crisis Home Treatment team as an alternative to hospital admission. Clients using this facility require access to 24 hour support for a short period of time due to a relapse of their mental health difficulties. During their time at the service their support will be reviewed, directed and delivered jointly by the Crisis Home Treatment team and P3 staff.

Service Description: 33 self contained flats for people with longer term mental health needs. The accommodation is dispersed throughout the borough and applicants are encouraged to choose which area they wish to live in.

The focus is on promoting recovery and maintaining someone to live in the community successfully.

The support can be provided over a longer time, so long as the persons needs remain. The Supported Housing team works flexibly, including evenings, weekends and bank holidays, if there is a valid support task.

Service Description: The Housing Support Service office is based at SHARP Hostel in Smethwick. The service provides housing related support to people with enduring and complex mental health needs; living in the community within the Borough of Sandwell. Visits are made between the hours of 9:00 and 18:00, Monday – Saturday; evening and Sunday visits may be accommodated by arrangement. Service users are encouraged and enabled to maximise their individual potential and maintain a suitable lifestyle within the community. The service is long term and may be provided for as long as required. Referrals are accepted directly from the interested party or through an agency or organisation.

Many people with a learning disability live independently in the community. Housing-related support can enable people to develop the skills to do this successfully and to lead active lives.

35 Support for Older People

In this section you will find a range of services that provide floating support, visiting people where they live now, or sheltered accommodation with support for Extra Care needs.

These services provide both short and long term support.

Support is offered to help you manage finances, stay healthy and other needs.

Often people are good neighbours, and may organise social activities. If you also have a care need this can be arranged separately and the scheme will help if you have any problems getting your care.

Service Type: Tipton Extra Care Village is a purpose built extra care scheme providing accommodation, support and domiciliary care services for over 55’s across three sites – Millennium Forge, Swallow Fields and Exon Court, all in the Tipton area.

Service Description:

37 We provide domiciliary care services in service users homes 24 hours a day. The service is provided for people over the age of 55 or living with physical disabilities, sensory loss, including difficulty with sight and hearing and for older people with learning disabilities, mental health problems including dementia.

The aim of our service is to provide help with health and social care needs in order for people to retain/regain their independence, the overall aim is that the service will be flexible enough to prevent you from having to move out of your home into residential care.

Tipton Extra Care Village recognises that each person is unique with their own likes, dislikes, references, history and make up and therefore understands that care and support will need to be flexible in order to reflect this to ensure that support with personal care reflects your unique needs.

Tipton Extra Care Village understands the importance of maintaining independence and therefore encourages involvement in all aspects of decision making;

We are committed to not only sustaining your quality of life but enhancing it.

Each of our services has a member of staff on duty 24 hours a day, the numbers of staff on duty at any one time will depend upon the level of care required within each scheme. Most of your care will be provided at regular times that you have selected when completing your care and support plan.

What is extra care housing? You may be thinking of moving, or have to leave your home, because of life changes or health problems. A move to extra care housing means you can remain independent, but also get the care and support you need. It gives you the security and privacy of a home of your own, a range of facilities on site, with 24-hour care and support services on hand.

How much will I have to pay? The amount you will have to pay will depend on your individual situation and income, and the scheme that you are moving into. Each scheme will charge a different amount depending on the size of the accommodation and the services provided. Costs could include rent or mortgage payments, service charges and payments for care and support. You may qualify for help with some or all of these costs. It’s important that you get advice before you move in and claim all the benefits you are entitled to.

Manifoldia Grange is centrally located in the West Bromwich area of Sandwell, less than half a mile from all local amenities. Service Description: Action on Hearing Loss’ focus, is to work with people who are deaf, hard of hearing or deaf blind. People using our services often have additional or complex needs which include learning disability, mental health, physical disability, autistic spectrum disorders, dementia, age related frailty, cognitive impairment.

Policies Action on Hearing Loss (previously RNID) is proud of policies in place. These are for the staff and service users alike. Policies/procedures in place cover equal opportunities, complaints, referral and admission to services, SU’s support and care planning, protection of vulnerable adults, risk assessments and more.

Action on Hearing Loss provides varied levels of support based on individual needs. We promote a quality lifestyle that upholds privacy, dignity, choice and independence and enhances personal fulfilment.

We take a person centred approach to the provision of support. We have excellent links with external agencies which includes housing associations, colleges and employment services. Action on Hearing Loss is a deaf charity. Support provided is governed by Supporting People. Regular assessments/audits are held by both Supporting People and internally to ensure our standards remain high. Staff receive appropriate training which includes signing to Level 2 British Sign Language.

Service Description: The Hostel provides furnished shared and single rooms for males and females over the age of 18. Service Description:

Sandwell Integrated resettlement Support (SIRS) offers voluntary support to all young people leaving custody and those on statutory court orders with accommodation issues.

SIRS visits young people in custodial establishments to compile a resettlement plan with them to prepare for their release. The resettlement plan includes planning appropriate accommodation and support to maintain this accommodation, support offered to parents & carers, identify peer influences and address them, help seeking education, training and/or work, access to constructive leisure time pursuits and the development of life skills e.g. budgeting, cooking and domestic management. The service also addresses any other issues identified e.g. accessing benefits, completing forms etc.
