Directory of Supporting People Services 2011 1 The Supporting People mission To work in partnership with Local Government, Probation, Health, voluntary sector organisations, housing associations, support agencies and service users to help vulnerable people to live more independently and maintain their tenancies. To give people the choice to receive support in their own home or to move into supported accommodation. To ensure the services provided are of high quality, are cost effective and meet the needs of service users. 2 What is supporting people? Supporting People is a working partnership of local government, service users and support agencies, which offers vulnerable people the opportunity to improve their quality of life. Who is the programme aimed at? At the moment in Sandwell we have provision for the following groups: People with mental health problems People with learning disabilities Frail elderly people Older people with support needs Young families Young people at risk People with alcohol problems Homeless families Single homeless people Women at risk of domestic violence Offenders or people at risk of offending Hearing impaired and deaf people People with HIV and AIDS Who do I contact? Other useful contacts. 4 Support for Young People (includes young families) Support is provided in a variety of ways for young people who may be homeless or need to leave home for the first time. Most schemes that provide accommodation can be applied to directly. Every effort is made to help people with particular problems and this may include finding the best place in Sandwell to help them. “Foyers” offer structured programmes of education and training as well as independent living skills. A number of schemes are able to help young people move from a scheme with others to their own property, and continue to offer support if needed. There are also services for young parents. 5 Fry Housing Service Name: Chapter House Service Type: Accommodation based Address: Phoenix Street, West Bromwich, West Midlands, B70 0AJ Contact number: 0121 558 5739 Email: [email protected] Website: www.midlandheart.org.uk Service Description: Supported accommodation for young single homeless people, ex-offenders and those deemed at risk of offending, aged 18+. Maximum stay up to two years, with the emphasis on resettling people who use the service, back into the community. 6 Bromford Support Service Name: Sandwell Young Families Floating Support Service Type: The service supports young families living in the Sandwell area to develop skills needed to live independently and maintain their tenancies, thereby reducing the risk of tenancy failure. Address: Bromford Support, Flat 15 Hobart Road, Tipton, DY4 9LZ Contact number: 0121 522 2734 Email: [email protected] Website: www.bromfordgroup.co.uk Service Description: Each service user will have their own floating support worker who will provide weekly keywork sessions in the service users’ home. Referrals to the scheme will be vulnerable young families aged 16 – 29, who live in the Sandwell area, have identifiable support needs, and are willing to engage and commit to the support offered. We have a multi-agency approach with our services, and facilitate regular steering group meetings. The main agencies working with the service are Sandwell’s Community Care Team, Sandwell Homes, and Sandwell’s Primary Care Trust (mainly health visitors and midwives). The support can be provided for up to two years, however generally the support is only needed for 9 – 12 months. Accessing the service: Referrals are accepted from any agency or landlord. Self-referrals are also accepted. To apply please contact the above number for a referral form. 7 Bromford Support Service Name: Hobart Road Young Families Supported Housing Service Type: The service offers supported accommodation for nine young families in the Sandwell area, supporting young families to develop the skills needed to live independently, thereby reducing the risk of future tenancy failure or repeat homelessness. There is a support worker on site during normal working hours, providing weekly key-work sessions to each young family. The service is funded by Sandwell Supporting People to work with the nine young families living at the scheme. Referrals will be vulnerable young families aged 16 – 29, who are in housing need, have identifiable support needs, and must be willing to engage and commit to the support offered. We have a multi-agency approach with our services, and facilitate regular steering group meetings. Accessing the service: Referrals to the scheme are via Sandwell’s Community Care Team or Sandwell’s Singe Referral Panel. 8 Midland Heart Service Name: Gilbert Road Service Type: Young single homeless Address: 100 Gilbert Road Capehill Smethwick West Midlands B66 4PZ Contact number: 0121-555-6500 Email: [email protected] [email protected] Website: http://www.midlandheart.org.uk/ Service Description: Gilbert Road provides semi-supported housing for 17-25 year olds who are ready to practice living independently. We provide 14, one bed flats with kitchen, living room and bathroom. They are part furnished with carpets, curtains, cooker and fridge. They have their own separate entrance, access to communal garden and the use of the communal laundry room. Our young people at Gilbert Road initially have a six-month assured short-hold tenancy agreement, which can be extended as appropriate. Tenants are responsible for their own gas, electricity, water and council tax bills. 9 Applications All referrals are taken through the Single Referral Panel, an accommodation provider for young people who have completed a resettlement programme and require semi support accommodation. Customers should be aged 18–25 years. All applications will be assessed individually, but if you use alcohol, drugs or other substances you need to be engaging with a professional agency on a programme of recovery. Our Support Worker can assist with a range of support services including support with managing your money, budgeting skills, accessing education, training, employment, and leisure. Staff will support and advise you in claiming the correct benefits to enable you to live at the scheme; these will include housing and Council Tax benefit. Our staff team consists of the Projects Leader, Team Leader and 1 Project Worker. Our working hours are normally Monday to Friday between 8am – 4pm. 11 Midland Heart Service Name: Murdock Road Service Type: Young single homeless Address: 1/3 Murdock Road Black Patch Smethwick West Midlands B66 2LS Contact number: 0121-555-8444 Email: [email protected] Website: www.midlandheart.org.uk Service Description: We aim to provide safe and secure accommodation for 16 young people (16-25 years of age) in training employment or education. We will provide the necessary support and advice for our residents to achieve independent and successful lives We have sixteen bed-sit rooms, three of which are dedicated to young people with psychosis and one is accessed through Sandwell’s Early Intervention Team. All rooms are fully furnished (bed, chest of drawers, bedside cabinet, built in wardrobe space, central heating radiator, fridge and chair) each two units have a communal bathroom with toilet, bath and shower facilities Kitchens and lounge are communal and are located on the ground floor at the end of the corridor from the main reception.
