I am honoured to present the HIV/AIDS Service Directory for Hoima Town Council; the first of its kind in the history of our town Council

Hoima Town Council is one of the implementing sites of the Alliance of Mayors and Municipal leaders on HIV/AIDS in Africa Uganda Chapter.

The Alliance of Mayors and Municipal Leaders on HIV/AIDS (AMICAALL) was launched in 1997 in Abdijan.

Hoima Town Council launched its Local AMICAALL Programme in 2002 which ran on interim arrangement until 2004 when this council formed its substative HIV/AIDS Task Force/AMICAALL Local Committees.

We are all aware of problems/challenges HIV/AIDS has caused to the community in one way or another, directly or indirectly.

The adverse effects to mention but a few, are that HIV/AIDS has affected the most productive bracket of the population. This has led to most vulnerable class of people to fending for the bereaved family. Thus grand parents and child headed families have become common in our society.

The challenges brought about by HIV/AIDS require a coordinated effort right from the lowest level of administrative units; say village level up to National and International levels.

Horizontally in Hoima Town, it requires both private and public organisa- tions to put hands together to fight, create awareness and prevent the disease.

The World Bank Consultant Dr. Julie Dyer, the National Coordinator AMI- CAALL Dr. John Mugisa have enabled Hoima Town Council to link up and network the community, in order to put a mechanism of creating awareness to the causes, prevention and contol of the scourge given the unique behaviours of urbanisaiton.

1 Information being an important tool to encounter the challenges of HIV/ AIDS, Hoima Town Council has come up with the service directory, which has been compiled with the help of AMICAALL Uganda Chapter funded by UNDP.

The service directory contains the major organisations which have been carefully identified and sorted out because of their involvement in the struggle against the scourge.

The Directory will provide the necessary information, knowledge under- standing and assistance available on HIV/AIDS that the public needs to share.

In this document there is a collection of information on HIV/AIDS related services that are available in Hoima Town Council to enhance information sharing and positive living.

2 Profile of Hoima Town Council

Hoima Town Council is located in in Western Uganda, about 200km North West of city. It is the Headquarters of the district. It is made up of 3 words viz Central, Northern and Western It is boarded by other Local Governments namely Buhanika Sub County in the East but it should be noticed that surrounded mainly…by Busiisi Sub County in almost all directions. The Town Council has a total area of 20.95 sqkm (2.095Hq) and a population of 36150 and population growth rate of 2.8%. The population per ward is as below,

District population officer

The population is predominantly female than male due to urban migra- tion with a ratio of 54.8 : 45.2. many of the families have rented for the wives and children live in town, in order to access relatively better social and economic facilities available in town. The youth also constitutes a big part of the population mentioned above the reason being the same. Due to scramble for accommodation slums gradually are emerging especially in Kiryatete and Kiganda in central and northern ward respectively.

POLITICAL AND ADMINISTRATIVE DIVISION

The political head of the Town Council is Chairperson L.C III elected though universal adult suffrage. The council comprises of fourteen elect- ed representatives two from each ward. The Town/urban Council is the

3 political organ of the town and planning authority as spelt out in the constitution of 1995.

The council has one standing committee of Finance/Social services. The Administrative head of the Council is the Town Clerk who is respon- sible for the technical implementation of council resolutions and guidance on legal matters. He is assisted by various technical heads of depart- ments and sections. Hoima Town council is made of three (3) Wards and 38 villages (L.C. I) the wards include Central Ward, Northern Ward, and Western Ward.

4 The Central Ward is most developed and has most Government offices and the central business district (CBD) compared to Northern and West- ern Wards.

Again Central Ward is most populated with many households while West- ern Ward has a few household and least populated with many households while Western Ward has a few household and least populated and it has headquarters of Church of Uganda and while Northern Ward has head- quarters for roman Catholic Church and its namely peri-urban with some agriculture taking place i.e Kyarwabuyamba and Bujumbura villages.

The central ward is mainly commercial ( central business district) area and this has led to infrastructural development in terms of hospitals, schools, markets, and recreation centres in this particular ward com- pared to other 2 wards. The Northern ward and Western are mainly peri-urban and River Bigajuka passes through both Wards. The town Council has few salient physical features notably Bakumira hill which has the installation masts for MTN and UTL boosting the telecom- munication in the District.

The Town council receives bimodal rainfall, which favours small-scale agriculture especially in peri-urban areas. However it should be noted that people in Town depend on formal employment and informal sector. Most people in Town are hit by abject poverty since they are engaged in small- scale agriculture and petty business.

COUNCIL DEPARTMENTS The Town Council is made up of 7 departments Health, Finance, Educa- tion, Administration, Engineering, Audit, and Urban Council. All these related to one another in terms of technical support when providing serv- ices as provided in second schedule part 3 of Local Government Act 1997. However, it should be noted that after restructuring exercise of Local governments the Departments may increase.

In a nut shell most people hit by poverty are those found in the central area due to unemployment and under employment rates. Most of the other wards are populated with orphans, people with disabilities, wid- ows, elderly, women and other vulnerable groups.

7 3.0. FACTS ON HIV AND AIDS

What is HIV/AIDS? • HIV-Human Immuno-deficiency Virus. The virus destroys the body defense system, attacking the body soldiers and killing them and leaving the person vulnerable to all sorts of diseases (e.g. TB, Pneumonia, skin Infections and diarrhoeal diseases)

• AIDS: Refers to acquired immuno deficiency syndrome

• ACQUIRED. The virus is acquired through someone else. some of the ways we can acquire the disease include:

• Having unprotected sex with an HIV infected Person • Mother to child transmission during pregnancy, delivery or by breast feeding • Blood transfusion with HIV infected blood • Sharing sharp objects, which contain HIV Infected blood e.g. blades and needles.

Immune. A protection against disease causing micro-organisms. Deficiency. Loss of this protection Syndrome. A group of signs or symptoms that together define AIDS

A person progresses from being HIV positive to developing AIDS.A person with AIDS also gets what are called opportunistic infections. These are usually caused by some viruses or other bacteria that our bodies have but can control from harming us. Our Immune system keeps these microbes in check.

You cannot be infected by HIV/AIDS by: • Shaking hands with an HIV positive person • Eating food from the same plate, sharing a cup, cutlery or cookery with HIV positive person • By hugging an HIV/AIDS positive Person • An insect bite e.g. Mosquito or any • Using the same bathroom or latrine with an HIV positive Person • Sharing a bed or sitting next to an HIV positive person • Working with an HIV positive person • By witchcraft

Living positively with HIV and AIDS Get regular counselling from a professional counsellor who will help you to come with terms with your status

Take care of your health by: • Getting treatment for any sign or symptoms of infection or ill health as soon a they occur, by visiting a health care clinic or hospital listed in this service directory

9 • Eating a healthy and affordable balanced diet every day and having fruits and vegetables whenever you feel hungry. A balanced diet consists of proteins and fats (e.g. meat, milk, fish, cheese, nuts, ovocados). These are body building foods. A balanced diet also includes carbohydrates (e.g. porridge, matooke, potatoes, rice, cassava, pasta and bread). These are energy giving foods for the body. It also includes, vitamins, which come from foods such as spinach, carrots, onions, garlic and cabbages. These foods protect the body against infections and gives it energy. • Excising your body regularly by walking, cycling and swimming. Consult a doctor who will advise you if this is possible or not and also on the type of exercise you should do. • Getting enough sleep at night or during the day depending on your job schedule, and resting every time you feel tired • Do not take alcohol, smoke cigarettes or take harmful drugs like opium • Join people living with HIV/AIDS support group, where you can find help and understanding from people who are having similar feelings and experiences like you. The names, contacts and details of such groups are included in this service directory. • Practice safe or protected sex. Use a condom correctly every time you have sex. This will prevent you from being re-infected, which will breakdown your body’s defence system further. It will also prevent you from spreading HIV/AIDS to another person. This service directory will tell you where you can get the condoms from if you are sure of how to use them properly, you can ask a health care worker or doctor. • You can also choose to abstain from having sex • If you feel strong and healthy, continue, to do your usual work • Learn to accept your status of an HIV positive person, as this is not going to change • Enhance your spiritual life

What is HIV antiretroviral Treatment? This is the main type of treatment for HIV or AIDS. It is not a cure, but it can stop people from becoming ill for many years. The treatment consists of drugs that have to be taken everyday for the rest of someone’s life.

10 HIV is a virus and like other viruses when it attacks a cell in the body it produces new copies; HIV can go and infect other previously healthy cells. It is easy for HIV to spread quickly through the billions of cells in the body if it is not stopped from replicating. Antiretroviral treatment for HIV infection consists of drugs which work against HIV infection itself by slowing down the replication of HIV in the body. The drugs are often referred to as: • Antiretroviral • Anti-HIV drugs • HIV antiviral drugs

Antiretroviral (ARV) treatment can extend the healthy life of someone liv- ing with HIV. If other factors remain the same, and ARV medication helps HIV+ people to survive longer: The introduction of ARV medication can however contribute to reducing prevalence by presenting people with an incentive to be tested (those who know their status are more likely to change their behaviour and so not become infected)

Are ARVs a cure? The current drugs are treatment but not a cure; they can stop the pro- gression of HIV. They let your immune system start to repair itself, but you will still be able to pass HIV-to another person

DEFINITIONS

In the following pages, there is a list of all service providers working with HIV/AIDS in Hoima Town Council. The services provided are grouped as follows:

Care: Provision of non-medical care to Persons Living with or Affected by HIV/AIDS, which may include provision of food, clean water, home visits, blankets, clothing or other items, moral and spiritual support, basic health care and some counseling. Condom Provision: Free provision of condoms.

Counseling: Includes pre- and post-test counseling, counseling for those living with HIV/AIDS and counseling for family and community members affected by HIV/AIDS.

Data Collection: Collection of data about HIV/AIDS and HIV/AIDS service providers, monitoring and evaluation of service providers and programmes.

Education: Includes education about HIV/AIDS; general provision of schooling; vocational training; training in income generat- ing activities; and education of counselors.

Funding: Provision of funds for organizations working with HIV/ AIDS, including community-based organizations.

Sensitization: Activities to help community awareness of HIV/AIDS, avoidance, and prevention of stigmatization.

VCT: Voluntary Counselling and Testing: provision of testing facilities for HIV/AIDS and counseling prior to testing.

Widow/Widowers Support: Support and care specifically for widows and widowers, which may include provision of basic health care, funds, food, accommodation, items for the house, clothes, coun- seling on positive living and/or training in income gener- ating activities.
