I am very conscious and proud of the honour that has been conferred on me by my election as the frrst Chairman of this new body, a body which has set itself radical and far-reaching ideals.

I pledge to do everything that I can to build better health and personal social services. I have a tremendous commitment to improvement of services and the vision to achieve real progress on issues like, hospital waiting lists care and long-stay accommodation of the elderly, services for people with a disability, mental handicap, homelessness, drug addiction and Ald. Ivor Call ely, T.O. community care services. Certain existing service delays are unacceptable. ERHA Chairperson I will be working with the ERHA to make fundamental differences and to achieve signifrcant and noticeable change in service provision over the coming years.

The Eastern Regional Health Authority is now the statutory body with overall responsibility for the health and personal social services for the 1.3 million people who live in Dublin, Kildare and Wicklow.

The work ahead of the ERHA is both wide-ranging and complex. Its responsibilities include the strategic planning, commissioning, funding, evaluation and monitoring of services. It will also act as an advocate for Cllr. Jim Reilly clients and patients, with the Department of Health and Children. The new ERHA Vice-Chairperson structures provide a forum whereby the voluntary and statutory agencies can work together in a new partnership.

As Chairman, my main objective during my year of office will be to help to create a climate for the ERHA in which it can evolve and develop. My hope is that it will be the kind of organisation in which it is possible for the relevant constituent bodies to ensure that everyone who needs the health or personal social services that we are charged to provide will frnd that they are treated efficiently, with courtesy, as speedily as possible, and, as far as possible, in their own locality.

The Minister for Health and Children, Mr. Micheal Martin TO., in launching the ERHA on Establishment Day, challenged us to take a radical approach, starting not with existing systems, but with patient need. He asked us to question apparently chronic problems and not to accept the usual answers. This interrogative approach will set us all a headline that can only result in searching and extensive efforts to improve anything that needs to be improved and to make what is already good even better.

Whatever we attempt, we do so with the knowledge that we have the support of an excellent infrastructure, a sound workforce, and dedicated personnel for whom the concept of care and service is ingrained.

From March 1, 2000, the Eastern Regional Health Authority became the statutory body with responsibility fo r health and personal social services for the 1.3 million people who live in Dublin, Kildare and Wicklow.

The ERHA's responsibilities include the strategic planning of services, commissioning of services and fund ing services through service agreements with three new Area Health Boards, the voluntary hospitals and other voluntary agencies.

It is also charged with monitoring and evaluating the services provided by these agencies.

Three new Area Health Boards - the Northern Area Health Board, the East Coast Area Health Board, and the South Western Area Health Board - have responsibility to delive r within their own areas, the services previously provided by the Eastern Health Board.

The three new Area Health Boards, the Voluntary Hospitals and the Voluntary Intellectual Disability Agencies previously funded by the Department of Health and Children, will receive their funding through the ERHA on foot of agreed service pl ans and service agreements.

The new arrangements have been introduced because the previous organisational structures were no longer suitable for the vastly increased population of the Eastern region, and the scale and complexities of the health and social service issues, in the region.

I am delighted with the opportunity to participate in such a major re­ development of the structures for the delivery of health care in the eastern region. To achieve this, it is vital that everybody involved in the services work closely together and that those working at the coalface are provided with the necessary supports and an environment in which excellence can ~ourish .

The Eastern Health Board and the voluntary agencies have done excellent work over the years in providing health and personal social services for a very large and rapidly expanding population with complex problems and needs. They have provided an efficient and effective service at a time of rapid social change.

However, the health services in the eastern region have been historically fragmented. The new structures provide us with an opportunity to build on current achievements and provide a service which will be in a position to respond rapidly to changing needs.

When people are sick and vulnerable, they are least able to negotiate their way around the services, and it is my objective in the new structures to ensure that patients and clients will be able to access the services and find their way from one sector to another easily and without difficulty.

The ERHA is responsible for ensuring that the services provided in the region are effective and represent best value for money. It is also in a position to act as an advocate for the development of a first-class co-ordinated health service for the people of the eastern region.

The statutory and voluntary agencies have been generous in their welcome for and their commitment to the new structures and they have already contributed much to their development. I am looking forward to working with Members and with all the agencies to forge a new partnership for excellence which will, we hope, provide major benefits for the more vulnerable in our society. r-:' ~~ VOLUNTARY HOSPITALS WILL RETAIN THEIR OPERATIONAL AUTONOMY

Introducing the new arrangements the former Minister for Health & Children, Mr. Brian Cowen, TD. emphasised "lest there be any doubt remaining on the matter that there is no intention on the part of the Department or the Government to" take over" the voluntary hospitals, by way of the new structures or by any other means.

The Health Strategy, published in 1994 makes it clear that the independent identity of the voluntary providers would be fully respected in the new structures and the new Act delivers on that promise.

The ERHA is specifically required by legislation to have regard to the right of the voluntary bodies to manage their own affairs in accordance with their independent, ethos and traditions.

The Act states specifically that nothing in it shall be construed as prejudicing the performance by the new Adelaide and Meath Hospital Dublin incorporating The National Children's Hospital of its functions under its charter.

The new structures will however, bring about a major change in the funding arrangements for the voluntary hospitals and agencies in the Eastern region.

Until now, the major voluntary service providers tended to operate outside the health board system, funded directly by the Department of Health.

Under the new structures, service agreements between the ERHA and the major voluntary providers means that the voluntary sector will be formally tied into the overall services in the region for the first time.

This will allow the voluntary hospitals and agencies to retain their operational autonomy, while making them fully accountable for the public funds that they receive.

TERMS AND CONDITIONS OF EMPLOYMENT OF EASTERN HEALTH BOARD STAFF WILL BE FULLY PROTECTED

The new Act has a specific provision ensuring that the terms and conditions of employment of the Eastern Health Board staff transferred to the new Authority will be fully protected.

Every person who is a member of the staff of the Eastern Health Board immediately before March I transferred to and became a member of the staff of the ERHA on establishment day.

The ERHA then assigns staff of Area Health Boards, where appropriate. THE MEMBERSHIP OF THE ERHA

The overall membership of the ERHA is 55, consisting of 30 local public representatives nominated by the local authorities in the region, 13 registered health professionals, nine representatives of the voluntary sector, and three nominees of the Minister.

Of the 30 local public representatives, ten have been appointed by Dublin Corporation, and four by each of the Councils of Fingal, South Dublin, Dun Laoghaire/Rathdown, Kildare and Wicklow.

Of the registered health professionals, nine have been elected by the practising medical professionals in the area, one by the practising dentists, one by practising nurses other than psychiatric nurses, one by practising psychiatric nurses, and one by registered pharmacists in the area.

The nine elected by the medical profession include provision for two consultants in general hospitals, one consultant psychiatrist, two GPs and one doctor with special knowledge or experience in preventive medicine.

Of the nine representatives of voluntary service providers, three have been nominated by the representatives of the voluntary hospitals in the region, three by representatives of the voluntary intellectual disability services in the region, and three by representatives of other voluntary service providers.

MEETINGS

The Eastern Regional Health authority will hol1 at least six meetings each year, and such other meetings as may be necessary for the performance of its functions.

The Area Health Boards will hold at least nine meetings each year and such other meetings as may be necessary for the performance of their functions.

BUDGET

The budgets for the year 2000, totalling £ 1.5 billion for the three Area Health Boards and the Voluntary Hospitals and Voluntary Agencies have been determined by the Department of Health & Children.

From the I st March 2000, the ERHA will be responsible for monitoring and evaluating the spending of this money.

From the year 200 I onwards the ERHA will directly fund the three Area Health Boards and the 36 voluntary agencies listed in the second scheduling of the Health (Eastern Regional Health Authority) Act 1999. The other agencies will be funded through the Area Health Boards. ERHA WILL EXAMINE HEALTH NEEDS OF PEOPLE OF DUBLIN, WICKLOW AND KILDARE For the first time in the history of the health services in the region, the ERHA will have a remit to examine the health needs of the population as a whole, ranging over all the health services: primary care, acute hospital care, and community care, in both the voluntary and statutory sector.

It will carry out its functions in the context of the objectives of the Health Strategy: maximising health and social gain, addressing inequalities in health status, tackling the main causes of premature death, promoting good health, preventing disease, treating the sick and helping families and individuals to deal with personal social problems. In its allocation of resources, it will be charged with responsibility for deciding priorities on the basis of those parameters and thus ensuring efficiency, effectiveness and value for money.

The Eastern Regional Health Authority will present an unprecedented opportunity to develop a comprehensive, cohesive and integrated health system in the eastern region. Its objective is to deliver real benefits to the people in the long term, by bringing about a re­ orientation of the ser vices, so that all services are integrated around the patient, and seamless in the perception of the user.

The Authority will not be directly involved in the delivery of services. Service delivery will be the responsibility of the three Area Health Boards, the voluntary hospitals and the voluntary agencies. The Northern Area Health Board, the South-Western Area Health Board and the East Coast Area Health Boards will each have responsibility for the delivery within their own areas of the services previously being provided by the Eastern Health Board. They will also plan and co-ordinate all services within their areas, in co-operation with the local voluntary services providers.

The Members of the three Area Health Boards are appointed from the membership of the Eastern Regional Health Authority, creating a link between the planning and strategic work at the centre and the realities of service delivery at area level.

The precise number of Members of each Board is as follows: Northern Area Health Board 19 Members. South Western Area Health Board 21 Members East Coast Area Health Board 15 Members.

The Authority when appointing a member of an Area Health Board fixes the Member's period of membership, which shall not exceed five years.

The legislation provides that each Area Health Board will have its own Area Chief Executive and the team will be led by the Regional Chief Executive who will be the accountable person for the operation of the Authority as a whole.

The Area Health Boards will hold at least nine meetings each year and such other meetings as may be necessary for the performance of their functions.

Since there is just one representative each on the ERHA of general nurses, psychiatric nurses, dentists and pharmacists it will not be pOSSible to have a representative from each of these professions on each Area Health Board.

However, where these professions are not directly represented on an Area Health Board the legislation provides that their representatives may attend and speak, but not vote, at the meetings of the Area Health Boards on which they are not directly represented.

The functions of the Area Health Boards are as follows:

To provide, or arrange for the provision of health and personal social services specified in arrangements agreed with the ERHA

To plan and co-ordinate the provision of services in co-operation with persons providing services in the area and with such other persons as it may see fit.

To advise the ERHA on the provision of services generally.

Area Health Boards will carry out their functions subject to any general directions which may be given by the ERHA and will co-operate with the ERHA and the other Area Health Boards in the co-ordination of services in such manner as the Authority may determine from time to time.

Where Area Health Boards make arrangements with people for the provision of services, it shall put in place systems, procedures and practices to enable it to monitor and evaluate the services so provided.

The South Western Area Health Board covers Dublin city south of the River Liffey (except for the Pembroke electoral area) South Dublin County, the county Kildare and the local electoral area of Baltinglass in Co. A Shared Services Centre has been established in Dr. Steevens' Hospital to provide a wide range of professional. technical and information support services to the ERHA and the three Area Health Boards.

Within Shaping a Healthier Future. the government proposes that the health services should be centred on and driven by the need to simplify organisational arrangements and achieve value for money.

It is envisaged that the new Shared Services Centre will enable expertise and overheads to be shared. will encourage innovation and development and allow its customers to concentrate on their core business.

The majority of staff in the Eastern Health Board who were previously engaged in providing centralised processing services are continuing to provide these services to the ERHA and the three Area Health Boards through the Shared Services Centre. which is located at Dr. Steevens' Hospital.

The staff are employees of the ERHA and assigned to work in the Shared Services Centre, which itself is within an Area Health Board - the East Coast Area Board.

The SSC reports to a Board consisting of the Regional Chief Executive Officer. the three Area Chief Executive Officers and the Director of the Shared Services Centre.

The services to be provided to the ERHA and the three Area Health Boards include Employee Services. Finance Services. Management and Information Technology Services. Engineering. Planning and Development Services, Estate and Facilities Management Services and other corporate services.
