Box 30377 Lansing, MI 48909-7877 (517) 364-8400 or (800) 562-6197 INSTRUCTIONS FOR DIRECTORY USE: Members of Physicians Health Plan (PHP) must select a participating Primary Care Physician. Primary Care Physicians include General Practice, Family Practice, Internal Medicine, Pediatrics and some Nurse Practitioners. These physicians/practitioners are listed under the primary care section in this directory. Please select ONE Primary Care Physician for EACH family member. The Primary Care Physician names appear in section one of this directory. Selection of a Primary Care Physician is made by the PHP member. However, if you need assistance in selecting a Primary Care Physician, such as one close to your home or place of employment, the PHP Customer Service Department can suggest several Primary Care Physicians for your consideration. Please call the Customer Service Department located on the back of your member ID card. PHP does not require a referral for PHP members seeking treatment from primary care physicians or specialists within the PHP network. It is important to note that while PHP does not require a referral to physicians/practitioners within our network, there are certain specialists who may require a referral from the member’s Primary Care Physician. PHP will support the specialist in requiring a referral as they deem appropriate. Physicians/practitioners that provide services solely in a hospital-based setting are not listed in this directory. This would include, but is not limited to, specialties such as anesthesiology and emergency medicine. This directory is provided to assist you with your physician/practitioner/provider selection. For a complete description of services and benefits, please refer to your Certificate of Coverage. You are responsible for verifying the participation status of the physician/practitioner, hospital or other provider prior to receiving Health Services. You must show your ID card every time you request Health Coverage. If you fail to verify the participation status, and that failure results in non-compliance with required PHP procedures, services may be denied. In the event that specific Health Services cannot be provided by or through a Network physician/practitioner/provider, health services must be authorized in advance through a referral as designated by PHP. From time to time, the participation status of a physician/practitioner or other provider may change. You can verify the participation status through PHP's website at www.phpmichigan.com or by calling the number on the back of the member ID card. If necessary, Customer Service can provide assistance in referring you to physicians/practitioners or other providers who participate with PHP. You may contact PHP’s Network Services Department at (517) 364-8312 or (800) 562-6197 for additional information regarding physician/practitioner or other provider credentials (including Medical School, Residency, and Board Certification completion). Information may also be found on PHP’s website at http://www.phpmichigan.com. This includes primary hospital privileges, office hours and whether they are accepting new patients. Accreditation information for facilities is also included. This website houses the most current information. A board certified physician (MD or DO) has completed an approved educational training program (residency training) and an evaluation process including an examination designed to assess the knowledge, skills and experience necessary to provide quality patient care in that specialty. There are two certifying umbrella organizations: the American Board of Medical Specialties (http://www.abms.org/)and the Bureau of Osteopathic Specialists and Boards of Certification (http://www.aoa_net.org/Certification/certification.htm). PHP reimburses physicians/practitioners and other providers a fee for each service rendered. If you would like to understand more about the payment methods used for reimbursement, please call the Network Services Department at (517) 364-8312 or (800) 562-6197.
