To get detailed information about your health care coverage, please see your Evidence of Coverage (EOC). You will have to choose one of our network providers listed in this directory to be your Primary Care Provider (PCP). Generally, you must get your health care services from your PCP. Family Practice, Internal Medicine, General Medicine and Pediatric physicians serve as PCP’s for FHCP Medicare members. You will need a referral from your Primary Care Physician to see most specialty providers listed in this directory. However, there are certain specialists and services listed in the directory under the category of “Direct Access” that do not require a referral from your PCP. There are also certain services that you can access without a referral from your PCP. The providers of these services are also listed in the “Direct Access” section of the directory. The network providers listed in this directory have agreed to provide you with your health care services. You may go to any of our network providers listed in this directory; however, some services may require a referral. If you have been going to one network provider, you are not required to continue to go to that same provider. In some cases, you may get covered services from non-network providers. For example, if you need a specialty service or care that cannot be rendered by a FHCP Medicare network provider, FHCP Medicare will make arrangements for you to be seen by a provider that is not in our network. Also, emergency or urgent care inside or outside of our service area can be provided by providers that are not in our network. If you have any questions about whether we will pay for any medical service or care that you are considering, you have the right to ask us whether we will cover it before you get the service or care. If you are enrolled in FHCP Medicare Rx Plus POS, then you may choose to see any contracted HMO provider OR Medicare participating provider or facility, without a referral. If you receive a bill from a non-plan provider, please do not pay the bill. You should forward the bill to Claims for processing at the address listed below: FHCP Medicare Attn: Claims Department P.O. Box 10348 Daytona Beach, FL 32120-0348 Emergency care and post-stabilization services can always be obtained in or out of the service area from the nearest available provider. You do not have to obtain emergency or post-stabilization care from a FHCP Medicare network provider. When inside the service area you must use plan providers for urgent care. When out of the service area urgent care may be obtained from the first available provider. In addition, when out of the service area you can obtain dialysis treatment from any qualified dialysis provider. Our emergency and post-stabilization services can be obtained from any hospital, urgent care center, or physician’s office. Our network hospitals that provide emergency services inside our service area are listed in this directory. 3 | Page It is important to understand that unless you are enrolled in FHCP Medicare Rx Plus POS, if you obtain routine care from out-of-network providers neither Medicare nor FHCP Medicare will be responsible for the costs. If you are enrolled in FHCP Medicare Rx Plus POS, with the exception of emergencies or urgent care, it may cost more to get care from out-of-network providers. You must use network providers except in emergency or urgent care situations or for out-of-area renal dialysis or other services. If you obtain routine care from out-of-network providers, neither Medicare nor FHCP Medicare will be responsible for the costs. Just wanted to let you know that I’ve quit my cor- porate job. I couldn’t stand another day in the cubbyhole culture. I’m joining some other friends who feel the same way and we’re planning a cross-country trip to experience what the real America is like. We’ll be traveling very slowly down the back roads for the next few months, and we plan to meet up with folks from all walks of life. We will be documenting our travels as we go and our plan is to eventually produce a feature-length documentary of our adventure.” Sounds sorta like Kerouac or Kuralt, doesn’t it? But read on: “We’ll begin our trip from Seattle in August and end it in Boston sometime this fall. We’re financing this through our own savings and by plastic, but we are looking for sponsors, too. By the way, we will be traveling by Segway Human Transporter (HT).” The letter above may be imaginary but the trip across America that Hunter Weeks, Josh Caldwell, Pat Armstrong, Alon Waisman, and Gannon Weeks are making is very real.
