HODGKIN LYMPHOMA TREATMENT REGIMENS (Part 1 of 5) Clinical Trials: The National Comprehensive Cancer Network recommends cancer patient participation in clinical trials as the gold standard for treatment. Cancer therapy selection, dosing, administration, and the management of related adverse events can be a complex process that should be handled by an experienced health care team. Clinicians must choose and verify treatment options based on the individual patient; drug dose modifications and supportive care interventions should be administered accordingly. The cancer treatment regimens below may include both U.S. Food and Drug Administration-approved and unapproved indications/regimens. These regimens are provided only to supplement the latest treatment strategies. These Guidelines are a work in progress that may be refined as often as new significant data become available. The NCCN Guidelines® are a consensus statement of its authors regarding their views of currently accepted approaches to treatment. Any clinician seeking to apply or consult any NCCN Guidelines® is expected to use independent medical judgment in the context of individual clinical circumstances to determine any patient’s care or treatment. Primary Treatment Stage IA, IIA Favorable (No Bulky Disease, <3 Sites of Disease, ESR <50, and No E-lesions) REGIMEN DOSING Doxorubicin + Bleomycin + Days 1 and 15: Doxorubicin 25mg/m2 IV push + bleomycin 10units/m2 IV push + Vinblastine + Dacarbazine vinblastine 6mg/m2 IV over 5–10 minutes + dacarbazine 375mg/m2 IV over (ABVD) (Category 1)2-5 60 minutes. -

These warnings include: - Vincristine should only be administered by the intravenous (IV) route. Administration of vincristine by any other route can be fatal. - Syringes containing this product should be labeled “Warning - for IV use only.” - Extemporaneously prepared syringes containing this product must be packaged in an overwrap which is labeled “Do not remove covering until moment of injection. For IV use only - fatal if given by other routes.” - Contraindication of vincristine in patients with demyelinating Charcot-Marie-Tooth syndrome. - Potential risk of acute shortness of breath when vincristine is coadministered with mitomycin-C and GI toxicities including necrosis with administration of vincristine. HODGKIN LYMPHOMA TREATMENT REGIMENS (Part 1 of 2) The selection, dosing, and administration of anticancer agents and the management of associated toxicities are complex. Drug dose modifications and schedule and initiation of supportive care interventions are often necessary because of expected toxicities and because of individual patient variability, prior treatment, and comorbidities. Thus, the optimal delivery of anticancer agents requires a healthcare delivery team experienced in the use of such agents and the management of associated toxicities in patients with cancer. The cancer treatment regimens below may include both FDA-approved and unapproved uses/regimens and are provided as references only to the latest treatment strategies. Clinicians must choose and verify treatment options based on the individual patient. NOTE: GREY SHADED BOXES CONTAIN UPDATED REGIMENS. Moraes, Hemant Lata, Ebru Bedir, Muhammad Maqbool, and Kent Cushman INTRODUCTION Podophyllotoxin is the starting material for the semi-synthesis of the anti-cancer drugs etoposide, teniposide and etopophos. These compounds have been used for the treatment of lung and testicular cancers as well as certain leukemias. It is also the precursor to a new derivative CPH 82 that is being tested for rheumatoid arthritis in Europe, and it is the precursor to other derivatives used for the treatment of psoriasis and malaria. Several podophyllotoxin preparations are on the market for dermatological use to treat genital warts. Since the total synthesis of podophyllotoxin is an expensive process, availability of the compound from natural re- newable resources is an important issue for pharmaceutical companies that manufacture these drugs. Currently, the commercial source of podophyllotoxin is the rhizomes and roots of Podophyllum emodi Wall. (syn. P. hexandrum Royle), Berberidaceae, an endangered species from the Himalayas. In recent stud- ies, we concluded that the leaf blades of the North American mayapple (P. peltatum L.) may serve as an alter- native source of podophyllotoxin production. Since leaves are renewable organs that store lignans as glucopyranosides, podophyllotoxin can be obtained by conversion of podophyllotoxin 4-O-β-D-glucopyranoside into the aglycone using our buffer extraction procedure. This extraction procedure of P. peltatum leaves yields podophyllotoxin in amounts similar to the ethanol extraction of P. -

Box 3708, Nepal *Author for Correspondence ABSTRACT A tree belonging to Ziziphus (Rhamnaceae) from Timal region of Kavrepalanchok district, Central Nepal is illustrated and described as a new species Ziziphus budhensis KR. Bhattarai & Pathak, a narrow endemic from central Nepal. This species did not match with the Ziziphus species reported from this region. Although the new species is similar to Z. montana W.W.Sm. from Yunan, China with trinerved leaves lacking conspicuous secondary veins, it differs tree habit, shape and size of leaves, length of peduncle, carpel, and shape and size of fruits and seeds as well as distribution range. Detailed taxonomic description, illustration, photographs, ecology and its economic and religious value are discussed. Keywords: Ziziphus, Rhamnaceae, Timal, New Species, Endemic, Nepal INTRODUCTION Ziziphus has about 100 species reported from warm-temperate and subtropical regions throughout the world (Mabberley 2008), among them 17 species are from India (Bhandari and Bhansali, 2000), 12 species from China (Zao, 2007), seven species from Bhutan (Grierson and Long, 1991) and six species are already reported from Nepal (NHPL, 2011). The unidentified species belongs to genus Ziziphus and family Rhamnaceae. Due to its economical importance local people from the Timal region of Kavreplanchok district brought material to the National Herbarium and Plant Laboratories (KATH) for identification. -

Introduction In the history of the environment as a public problem, industrial disasters have been insufficiently explored.1 Such disasters are nonetheless crucial because their collective interpretation weaves technical and scientific issues with problems of social justice and controversies concerning conflicting “common goods”, by destabilizing the equilibria that has formed between these elements.2 These disequilibria open the way for episodes of normative and cognitive uncertainty, and thereby become windows of opportunity for social critique and social change, especially by opening public debates on rules, institutions, and representations about technical progress. In short, industrial disasters become opportunities for rethinking the types of "compromise" between "orders of worth"—in particular industrial and civic--upon which a society rests.3 Yet there is still little acknowledgment of the social change that industrial disasters can trigger. -

BEFORE THE REACTION: AN INTRODUCTION TO REACH On July 10, 1976, in Seveso, Italy, just north of Milan, a mush- room cloud formed above a small chemical manufacturing plant, unleashing thirty to forty kilograms of TCDD into the atmosphere. TCDD is the most dangerous form of dioxin and a major compo- nent found in Agent Orange; it has an extremely adverse effect on the ecosystem and can cause numerous human diseases.2 Due to the dioxin spill, the human population suffered severe dermato- logic and hepatic repercussions which caused potentially lethal con- ditions, while various wild and domestic animals were either instantly and fatally poisoned by the chemical or slaughtered later as a preventive measure. -

Advance Journal of Food Science and Technology 4(1): 15-21, 2012 ISSN: 2042-4876 © Maxwell Scientific Organization, 2012 Submitted: August 26, 2011 Accepted: October 15, 2011 Published: February 15, 2012 Nutrient Analysis of Some Selected Wild Edible Fruits of Deciduous Forests of India: an Explorative Study towards Non Conventional Bio-Nutrition Ajay Kumar Mahapatra, Satarupa Mishra, Uday C Basak and Pratap C Panda Regional Plant Resource Center, Bhubaneswar, Orissa, India Abstract: Considering the growing need to identify alternative bio-nutritional sources, 15 wild edible fruits consumed in deciduous forest zone of India were evaluated for their nutritive value in order to prioritize edible wild fruits suitable for domestication. The result showed significance of wild fruit species as important source of nutrient for rural poor. The nutritional value of many wild fruits compared well with domesticated popular fruits as mango, banana, guava, papaya, sapota, pomegranate, strawberry etc. in terms of protein, carbohydrate or Vitamin content. The carbohydrate content in wild varieties as Mimusops elengi (18.1%) is found to be at par with mango (17%) and pomegranate (17.1%). High concentration of sugar was noted in Ziziphus rugosa (20.7%) compared to domesticated sapota (21.4%), grapes (16.2%) and pomegranate (16.5%). Protein content in Bridelia tomentosa (3.1%), Carissa spinarum (3.6%) and Polyalthia suberosa (1.9%) was found similar to cultivated fruits, viz., guava (2.5%), banana (1.09%) and lemon (1.1%). Maximum proportion of Ascorbic acid/Vitamin-C content was seen in case of Solanum torvum (37.4 mg/100 g), Terminalia citrina (53.52 mg/100 g), which is higher to banana (8.7 mg/100 g), apple (4.6 mg/100 g), pomegranate (6.1 mg/100 g) and mango (27.7 mg/100 g). -

Within the industrial- ised world, specific concentrations of highly dangerous chemicals may be localised to a certain area, vastly increasing the risk to the local environment. Although highly regulated by legislation such as the Control of Major Accident Hazards Regulations 1999 (COMAH), and agencies such as the Health and Safety Executive (HSE) and the Environment Agency (in England and Wales), or the Scottish Environment Protection Agency (SEPA) (in Scotland), industry has been a common source of environmental contamination. Additionally, as we discuss below, manufacturing of chemicals is not the only industrial process that places the environment at risk. Environmental poisoning can also result from the transport, storage, and secondary uses of the primary product. Small scale releases of toxic chemical substances are common within the industrialised world.1 Their impact is usually limited by effective statutory controls and efficient emergency services, but the combination of system failures can lead to disaster. In limiting the impact of such chemical releases, effective communication is required not only between the operator and emergency serv- ices but also from the emergency services to the public. Without such communication, panic may ensue, leading to emergency and hospital services being overwhelmed and prevented from effec- tively pursuing their roles.
