Naugatuck RiverLevee & Floodwall Investigation & Repair Project, Waterbury/Watertown, CT: Directed the evaluation of current watershed flood potential 60 years after original system design and subsequent construction of watershed storage structures. Evaluated the effectiveness of upper watershed flood control dams to reduce peak flood flows, and developed a HEC-RAS hydraulic model to analyze flood elevations for reduced flows and changed channel hydraulics associated with dam removal. Documentation submitted to CT DEP and the USACE for review and approval.

Designed and undertook design/build construction of repairs to comply with U.S. Army Corps Local Protection Project maintenance requirements.

On-Call Contract, Connecticut Department of Environmental & Energy Protection: Directing engineering services to inspect, investigate and design repairs to state owned dams. Have investigated several High Hazard dams, numerous Significant Hazard dams, have provided design/build construction of repairs to a High Hazard dam in Middlefield, CT, and have investigated dam de-commissioning options for several former water company dams. Construction of repairs have been completed in the Wyassup Lake Dam and are currently underway on the Beseck Lake Dam.

North Branch Flood Investigations: Directed the assessment of debris impacts on flooding along the North Branch of the Park River in Hartford for the City of Hartford. Assessed options to minimize potential blocking of flood conduit debris control structure.

Island Brook Flood Control Investigation: Directed the development of an Environmental Impact Evaluation for design of a flood control project on the Island Brook in Bridgeport, CT. The Island Brook suffers from extensive flood damage in both its upland and tidally influenced reaches. The EIE evaluated the effectiveness of proposed flood control improvements, anticipated impacts on the natural and manmade environment and the overall cost effectiveness of the proposed project.

Saccarappa (Slaters) Pond Dam, Oxford, MA: Directed the engineering investigations, design of repairs and permitting for this Significant Hazard municipally owned former mill dam. Designed new concrete spillway, leakage control using PVC liner, and toe drains for this stone masonry earth embankment dam. Permitting involved the coordination of historical studies since this dam was located in an area of archaeological significance as defined by the MA Historical Commission.

South Easton Pond Dam, Newport, RI: Directed the engineering investigation, design of repairs, permitting and construction administration for this High Hazard water supply dam. This unique dam with ring dike of over 5000 feet in length nearly failed from wave induced embankment overtopping and erosion during a nor’easter in 2006. Design entailed substantially strengthening the dike and designing it to withstand wave attack through use of articulated concrete erosion protection. Major

challenge was the need to keep water supply in production during construction.

Huntington ParkPond Dams, Redding, CT: Directed the engineering investigation, design of repairs, permitting and construction administration for this High Hazard dam. Our geotechnical investigation of the 25 foot high embankment lead to designs of slope drainage and stability improvements, entirely new concrete spillway and drawdown structure and access roadway improvements.

Slacks Reservoir Dam, Smithfield, RI: Directed the engineering investigation and design of repairs for this High Hazard earthen dam. This 150 year old dam forms a several hundred acre lake used for recreation. Undertook a soil boring program, geotechnical analyses of seepage and stability and developed a repair program to address internal erosion and stability inadequacies. Negotiated with regulatory agencies to allow a phased implementation of repairs. Developed an Emergency Action Plan as part of permit negotiation process.

Asa Pond Dam and Indian Run Dam, South Kingston, RI: Directed the design of repairs, permitting and construction inspection for two former earth embankment mill dams. Included design of new concrete spillway, low level outlet, embankment stability improvements, seepage control and public safety amenities.

WoonsocketFalls Dam Local Protection Project, Woonsocket, RI: Fuss & O’Neill project team dam safety consultant for the condition evaluation of this Army Corps of Engineers local protection project flood control dam on the Blackstone River.

Connecticut River Levee Local Protection Project, West Springfield, MA: Directed the development of an Operation and Maintenance Plan for the City of West Springfield Flood Control Dike System along the Connecticut and Westfield rivers. This involves the inspection and mapping of over 7 miles of earthen dikes and closures which provide protection to substantial areas of development within the flood plains of these rivers.

Palmer Brook Dam, Becket, MA: Directed the geotechnical evaluation of this earthen embankment high hazard dam which included a soil boring program, seepage and stability analyses. In addition the embankment toe drains and low level outlet were camera inspected to asses their current condition. Provide annual inspections and on-call emergency inspection services.

Blackstone Heritage Park, Uxbridge, MA: Designed, obtained permits and provided construction administration services, for the historical restoration of the system of gates along the Blackstone Heritage Park in Uxbridge, Massachusetts for the Massachusetts Department of Conservation & Recreation.

Willett Pond Dam, Norwood, MA: Directed the engineering investigation of this High Hazard earth embankment dam owned by a non- profit watershed management organization. Hydrologic & hydraulic analysis was undertaken to assess options for providing adequate spillway capacity. A soil boring program and geotechnical seepage and stability analyses have been undertaken to define alternatives for bringing the dam to current safety standards. Developed flood inundation mapping for Emergency Action Plan use.

Phase I Inspections of Dams for Private and Municipal Dam Owners: Investigated and/or directed the investigation of over 60 dams in Massachusetts to assist the dam owners in complying with mandated Phase I Inspection requirements since 2004. Mr. Cullen has 28 years of experience in geotechnical engineering, solid waste engineering, environmental engineering, and site design engineering. Glasgo Pond Dam, Griswold, CT: Performed a dam safety investigation of a high hazard earth embankment dam. Issues with the dam include apparent instability of the near vertical downstream stone masonry face of the dam, inadequate spillway capacity for the design storm event, and needed repairs to the low level outlet gate valves, weir boards and accumulated debris. A soil boring program was performed and the downstream face of the dam is being monitored for movement. A dive inspection was performed to assess the condition of the low level outlet. A report was prepared outlining the repairs and updates to the dam.

Willett Pond Dam & Dike, Norwood/Walpole, MA: Project Manager for a Phase II Investigation of a high hazard dam. The dam is a 900-foot long, 25-foot high earth embankment dam with a concrete core wall. There is also a 1,900-foot long, 14-foot high dike associated with the dam. The investigation included hydraulic and hydrologic analyses, soil borings, and stability and seepage analyses.Several alternatives were conceptualized for repair of the dam to bring it into compliance with current dam safety standards. A major issue includes the concrete spillway flow capacity, which is much smaller than that required to safely convey the design storm. dike, and a major secondary road on the dam crest that is deteriorating. Abridge over the spillway was recently replaced by the State without consulting the dam owner, resulting in new bridge over an inadequate spillway. Currently we are working with the owner to evaluate the repair options and refine cost estimates for the various alternatives.

Niskayuna Dam, Becket, MA: Coordinated subsurface investigation program for a failed earthen dam. The dam impounded water for recreational purposes and failed during heavy rains in October 2005. Performed seepage analysis for use in designing a replacement dam structure.

Outdoor World Dam, Sturbridge, MA: Performed borings at a failed earthen dam. Performed seepage analyses to evaluate the existing embankment and a proposed rebuilt embankment to increase the factor of safety against future failure.

Phase I Dam Inspections, MA: Performed numerous Phase I dam inspections for private and public entities to meet the Massachusetts Dam Safety regulations. Inspected dams, assessed hazard rating and condition, assessed adequacy of previous design work, and prepared reports for the Massachusetts Department of Conservation and Recreation Office of Dam Safety.

Dam Jurisdictional and Ownership Determinations, MA: Oversight of hundreds of dam jurisdictional determinations and dam ownership research for the Massachusetts Department of Conservation and Recreation Office of Dam Safety. Each dam was documented by performing site inspections to confirm locations, estimate structural height and storage capacity, and taking photographs. Ownership determinations were made by visiting the sites to confirm locations, talk to owners, visit assessors offices and deed registries to research ownership.

StanChem Dam Denil Fish Ladder Berlin, CT: Project Manager for the design and preparation of construction drawings and specifications of a concrete Denil fish ladder around a dam on the Mattabassett River in Berlin, Connecticut. Designed an integral fish counting/viewing structure suitable for filming the fish through an acrylic window built into the fish ladder wall. Designed wood baffles for control of the flow velocities through the concrete fish ladder, while maintaining access to the dam outlet structure. Currently providing construction assistance and oversight.

Oxoboxo Lake Dam, Montville, CT: Project Manager for evaluation of seepage through the dam via an abandoned low-level outlet. Prepared grouting plan for mitigation of the seepage. Oversaw the first phase of grouting in 2008. Prepared a report and recommendations for completion of the grouting program.

Lake Beseck Dam, Middlefield, CT: Project Manager for evaluation of significant seepage through a Connecticut DEP-owned dam. Planned and implemented emergency repairs on a previously constructed seepage cut-off system. Currently preparing final design and permit documents for permanent upgrades to the dam.

Springborn Dam, Enfield, CT: Project Manager for dam removal feasibility study at an abandoned Connecticut DEP-owned dam. Currently planning exploration and evaluation of the dam impoundment to address issues related to dam removal such as effect on adjacent bridge and building structures, sediment transport and disposal, groundwater impact, and fish accessibility upstream of the dam. Sediment sampling and quality analysis was performed revealing contaminated sediment. Currently assessing the potential risk involved in allowing sediment to move downstream versus removing and landfilling sediment.

Robinson Pond Dam Phase II Investigation, Oxford, MA: Project Manager for a detailed investigation and conceptual design of repairs of a Significant Hazard dam. Issues addressed included an inadequately sized spillway, sinkholes in the dam crest, and seepage through the earthen embankment. A dam breach inundation study was performed to assess downstream hazard potential. Performed a seepage and stability analysis of the dam. Successfully argued for keeping the dam at the Significant Hazard classification when the State considered raising its classification to High Hazard.

Briggsville Dam Removal Feasibility Study, Clarksburg, MA: Project Manager for removal of an existing run of the River dam on the North Hoosic River. Prepared construction plans and specifications for removal of a concrete and stone dam. Grade control weirs and habitat weirs were designed to protect an upstream vehicle bridge and to provide fish passage up river. Calm water pools were designed for allowing resting areas for fish. A wetland restoration plan was prepared for areas that will no longer be inundated with water when the dam is removed.

Green River Dam Removal Study, Greenfield, MA: Project Manager for an on-going feasibility study and preliminary design for the removal of two dams on the Green River. Currently assessing sediment management options based on sediment quality, sediment mobility and redistribution. Implemented sediment sampling program for sediment quality analysis. Effects on aquatic organisms and recreational boating were assessed. Conceptual dam removal plans with cost estimates were prepared summarizing the options for final design and permitting of the removals.

Wyassup Lake Dam, North Stonington, CT: Project Manager for design of repairs to an earthen dam forming Wyassup Lake. The concrete spillway was redesigned and the existing spillway will be removed and replaced in its entirety, including a new low level outlet. Steep embankments will be stabilized by addition of fill material that will also act as a filter for potential seepage. A new access road was designed for maintenance vehicles; a pre- fabricated foot bridge will be installed across the new concrete spillway. Final design plans and specifications will be prepared for use by the

Heritage Park Canal Restoration, Uxbridge, MA: Project Manager for a canal restoration project undertaken for the Massachusetts DCR in Uxbridge, Massachusetts. The project consisted of design, permitting and construction administration of flow control structures, canal embankments and replacement of a bridge used for emergency vehicles. Special provisions had to be made during the construction process to protect breeding Wood Turtles located in the canal.

Hilton Head Island, SC: Project Manager for a geotechnical study for foundation design of a new Control Tower at Hilton Head Airport. Auger cast piles were recommended to support an airport tower in loose sand deposits.

Port Wentworth, GA: Project Manager for a geotechnical study for foundation design involving paper roll handling machinery inside warehouse at Weyerheauser Paper Mill. Issues involved with the project were machine vibrations and the presence of buried concrete skids and timber piles from previous shipbuilding operations.

Port Wentworth, GA: Project Manager for a slope stability study of Savannah River bank for support of a 500,000 pound crane. Steep riverbank slopes complicated by buried debris required a support system for the stability of the slope during reconstruction of a warehouse.

Jasper County, SC: Project Manager for a geotechnical study for foundation design, University of South Carolina Administration Building, Science Building and Library. Portions of the buildings were constructed in areas of soft clays, requiring a surcharge to preconsolidate the clay layer.

Glasgo Pond Dam, Griswold, CT: Performed hydraulic and hydrologic analyses for this high hazard dam owned by the Connecticut Department of Environmental Protection. A hydrologic analysis was completed to assess a range of flood flows from the 100-year event to the Probable Maximum Flood. Hydraulic analyses were performed to assess the spillway design flood for the dam and alternative spillway configurations to safety convey that discharge. Assessment of the spillway design flood included running multiple dam breach analyses to identify impacts to downstream properties.

Trout Pond Dam, Granby, CT: Responsibilities included designing repairs to Trout Pond Dam and an appurtenant diversion structure in Bissell Brook, and the design of a new fish passage in Bissell Brook. Blackstone River & Canal Heritage State Park, Uxbridge, MA: Project Engineer for the inspection and rehabilitation of three historic gate structures at the Blackstone Canal in Uxbridge, Massachusetts. The gate structures were historically used to control the water surface elevation in the Canal. The rehabilitation of the structures, which had to be performed using historically accurate materials, will return the gates to their original operating condition. Engineering services included a field inspection of the gate structures, and preparation of repair plans.

Hamilton Reservoir Dam, Holland, MA: Project Engineer for the preparation of inundation mapping as the result of a failure of the dam, and the preparation of a Phase I Inspection Report. The dam breach analysis and Phase I Inspection was requested by the Massachusetts Department of Conservation & Recreation, Office of Dam Safety following flooding in October 2005. Engineering services included performing a dam breach analysis and preparation of inundation mapping, a hydrologic analysis of the contributing watershed, and the preparation of the Phase I Inspection Report.

Packers Pond Dam, Plainfield, CT: Assessed the hydraulic capacity of the existing spillway and downstream channel. Participated in the development of design plans for an additional spillway to provided adequate capacity to safely convey flood waters to the downstream area. This included a hydraulic analysis of the proposed spillway, an evaluation of the channel stability, and the design of an armored channel lining. Additional engineering services provided for this project included an inspection of the dam, an analysis of downstream flooding due to a failure of the dam, and development of an Emergency Operations Plan.

Oxoboxo Lake Dam, Montville, CT: Performed a hydrologic and hydraulic analysis of the site, an evaluation of improvement options, and an assessment of the environmental impacts to the surrounding area. Completed an Individual Permit for Dam Construction administered by the Connecticut Department of Environmental Protection. The permit was required to complete proposed improvements to the facility.

Smith Pond Dam, Monson, MA: Project Engineer for a Phase I Inspection of Smith Pond Dam and an associated earthen dike. The Inspection was requested by the Massachusetts Department of Conservation & Recreation, Office of Dam Safety following a breach of the dike. Engineering services included a hydrologic analysis of the contributing watershed and an analysis of the hydraulic capacity of the outlet structures and downstream channel. Additional services provided for this project included researching ownership of the abutting properties, and researching information in the Office of Dam Safety files.

Hydraulic & Hydrologic Investigations for Dams: Performed detailed hydraulic and hydrologic analyses for many dams throughout Connecticut, Massachusetts, Rhode Island, and New York. Hydrologic analyses were typically performed for the contributing drainage basins to define flood flow hydrographs and the range of anticipated peak flows at dams and at specific design points on riverine systems. Hydraulic analyses were typically performed in HEC-RAS and were used to assess the potential impacts to downstream riverine systems related to altering outflow patterns from dams as a result of proposed improvements to hydraulic structures, such as spillways. Hartford Levee, Multiple Water Courses, Hartford, CT: Performed wide ranging hydrologic and hydraulics related analyses in support of the levee accreditation process. This included a levee breach analysis, hydrologic analyses to assess changes in watershed development, and hydraulic analyses for various flood conveyance systems. Software used in these analyses included, but was not limited to, HEC-RAS and HEC-HMS. Hartford, CT: Project director for design of improvements to the historic facade on the United Way of the Capital Area building. This building is located in a park-like setting, and is accessible to the general public. Design included extensive use of pavers, masonry sitting walls, and landscaping and period lighting.

East Hartford, CT: Technical oversight of a complex stormwater management master plan for the Rentschler Field mixed-use development, which is anchored by the University of Connecticut's Division I football stadium. The site is located within two watersheds, and is characterized by complex interconnections between them. Plan was reviewed by the Town of East Hartford and multiple state agencies.

Westbrook, CT: Project manager and engineer for 21-unit age restricted housing development. Coordinated survey, site planning, civil engineering, landscape architecture and stormwater management efforts. Designed a subsurface sewage disposal system and obtained permits from the Town of Westbrook and the Connecticut Department of Public Health. Presented at public hearings to obtain land use approvals from town agencies. Performed construction stakeout and construction observation for subsurface sewage disposal system components.

Kensington, CT: Project director for conversion of a large, 4-story manufacturing building into condominiums. Coordinated environmental investigations, survey, site planning, civil engineering, landscape architecture and stormwater management efforts. Cohasee Brook Reservoir Dam, Southbridge, MA: Project engineer for the development of an Emergency Action Plan for the Cohasse Brook Reservoir Dam located in Southbridge MA. Performed dam breach analyses for "Sunny Day" and Spillway Design Flood" conditions utilizing HEC-RAS to route breach flows through Cohasse Brook, downtown Southbridge, and the Quinebuag River. Ms. Sharma is a Geotechnical Engineer with a wide variety of skills and experience. She works as a project engineer for the water resources design team in our Manchester office. University of Massachusetts Solar Power Canopies, Amherst, MA: Performed geotechnical studies for foundation support of solar power panels and canopies. Borings and test pit excavations were performed in varied conditions, including shallow bedrock, steep slopes, parking lots, and on closed landfills. Analyses were performed to determine the most favorable foundation types to suit the conditions encountered, including deep and shallow foundations.
