AIG Emergency Rapid Response Project Monthly Report – December, 2006

1. Community and Government Relations:

The Emergency Response teams (ERTs) have continued to focus on assessments and trainings through the month of December. Visibility and awareness of government/humanitarian/earthquake-affected communities to team programs and services has been raised through brochures, hourly radio public service announcements and newspaper advertisements. Team linkages with communities through first responder trainings and the above mentioned awareness material has increased incoming requests for assessments, training and emergency assistance.

The tent-shelter construction support to the Camp Management Organization (CMO), on request of the Earthquake Rehabilitation and Reconstruction Authority (ERRA) continued, as Rapid Build Teams completed the work in Chattar Kalas camp and Agro Tech Industry camp, located in city. The teams will now move to Agro Tech and Mera Tanolia camps. 240 tent shelters have been built through December.

There continued to be strong linkages between humanitarian organizations and district government through the Rural Relief and NFI working group. ERTs have mobilized high altitude assessment and distribution teams on request of IOM, ERRA and District Commissioner, for the verification of the situation in Pre-Winter Shelter Security Survey identified vulnerable communities.

Disaster Management, and BLS training of Development Authority Muzaffarabad (DAM) workforce, staff of national and international NGOs and trainings in at-risk communities is also ongoing.

2. Current Activities:

(i) Emergency Intervention

Interventions included: medical evacuations, emergency screenings, road traffic accidents, landslides, and fire.

Due to heavy snow, the road up the valley was blocked and 4 member GTZ team with guide and four-wheel vehicle were Kaghan trapped in fresh snow. Operational Manager (NWFP) Bashir Ahmed witnessed a RTA and mobilized Muzaffarabad ERT. The team reached the location in 25 minutes and searched for victims in the van. Ops manager had already transported 14 casualties to Gari Habibullah field hospital. The field hospital was without medical facilities, doctors or medicine, Gari Road Traffic as they were in process of closing operations. Muzaffarabad ERT 23-Nov Muzaffarabad Mansehra Gari Habibullah Habibullah Accident reached the hospital and proceeded with emergency care. Two casualties had fractures in the mouth and hip dislocation. All had lacerated wounds. After immediate care and stabilization of wounds, all patients were shifted to Mansehra Civil Hospital. Two serious patients were referred to Abbottabad. There were no deaths from the incident.

A Toyota pick up coming from Jabori to Shinkiari felll from the road Road Traffic due to break failure. Naseem and his 4 year old son fell 70 feet from 28-Nov Siran valley Mansehra Gugarrmang Dadar Accident the road. After revival from unconsciousness, first aid treatment was given and patients taken to the hospital. There is full recovery.

While ERT was on survey, a young man requested help for his 2-Dec Bagh Bagh Dharay Badiar Medical Evacuation mother as she had just fallen and had a backbone injury. First aid and evacuation to DHQ hospital Bagh.

An outbreak of acute diarrhea and pneumonia claimed the lives of 6 infants. Two days prior to the outbreak, Muzaffarabad ERT had held a training in Khanian village. Community members Ali Asghar and Neelum Maskeen contacted the ERT and requested assistance. and valley, Emergency Medical Neelum ERTs were also mobilized. 90 children in the community 4-Dec Jhelum Muzaffarabad Heerkotli Khanian Screening and were screened, 36 infants were treated on the spot and 7 severe valley, Evacuation cases were transported to AIMS Hospital, Muzaffarabad. ERT Muzaffarabad coordinated with District Health and Merlin for continued medical support. IOM mobilized UNICEF blankets, warm clothing and hygiene kits and carried out the distribution for 470 families in Khanian. All evacuated and treated infants had full recovery.

2 Vehicle rolled down off the road due to landsliding. ERT stabilized Battagram 5-Dec Battagram Pashora Wasma Vehicle in Landslide driver and passenger, who were both without serious injury. In valley coordination with the police, the vehicle was recovered.

At 11:00pm Bagh ERT received information that two cars had slipped and were on the roof of a private house down slope from the Road Traffic 7-Dec Bagh Bagh Dharay Sherpur road. There were no serious injuries. First aid and evacuation were Accident given to the passengers. The vehicles were recovered from the dangerous position in coordination with the army.

En route to the Northern Areas, the tires of a passenger coach burst and the vehicle fell a height of approx. 2000 ft. 4 passengers were Battagram Road Traffic seriously injured and 20 died on the spot. ERTs assisted police and 8-Dec valley, Allai Kohistan Dassu Keroonala Accident army in the dangerous recovery and identification of bodies. On valley request from District Administration, teams then arranged ambulance transportation for the injured and the dead.

Bagh ERT responded to a call of three patients of serious fever at Sorol Sudhan Gali. The team reached location and gave basic first 13-Dec Bagh Bagh BirPani Sorol Medical Evacuation aid before assisting in the difficult evacuation to PIMA hospital, Bagh.

Bagh ERT received information that a child at IDP camp chattar # 2 IDP Camp Emergency Medical expired due to pneumonia. ERT screened all the children of the 19-Dec Bagh Bagh Dharay Chattar #2 Screening camp and gave report to the concerned agencies (District Health, World Health Organization). Road Traffic 10 year old male child, Naseem, injured in RTA. Siran ERT 21-Dec Siran valley Jabori Jabori Kanoog Accident stabilized fractured clavicle and dressed head injury.

Fire broke out in a wooden shelter and destroyed the shelter, all assets, including eight hundred thousands rupees cash and left 25-Dec Muzaffarabad Chinari Chinari Fire three people with 2nd degree burns. First Aid and burn treatment given to the casualties. Long term treatment prescription also given by ERT Medic.

3 (ii) Training

On 7 December. a 23 member ‘Urban Emergency Workforce’ (UEW) of DAM received disaster management, first aid and BLS training from the Muzaffarabad and Neelum ERTs. The workforce will focus on urban emergencies this winter whereas the ERTs will continue to prioritize rural needs and remain on standby for technical assistance in the urban setting.

Neelum ERT held a first-aid and BLS training for 25 staff of Muzaffarabad-based NGO Kashmir Youth Foundation on 20 December.

On 27 December, Jhelum ERT organized a large training on disaster management and BLS training for 50 staff members from 7 NGOs and UN agencies (NRSP, Merlin, Diachoni, UNHabitat, UNFPA, Oxfam, NRC) and 2 government organizations (District Hospital, Jhelum Welfare Association) working in the Jhelum valley.

ERTs have continued the community-level training programs based on assessment and also call-in requests. A total of 41 trainings were held in December. 6861 community leaders, secondary school children, teachers and government/non-government staff have been identified and trained as first responders in at-risk communities. Training modules include: disaster management, first aid, BLS, mountaineering, survival and fire safety. (Refer to training matrix)

7 (iii) Assessment

DISASTER ASSESSMENT- VILLAGES AT HIGH RISK

To date ERTs have assessed 355 at-risk villages. Assessment work has successfully led to the matching of emergency needs with training modules for the training operations of the ERTs.

Teams have also been mobilized by ERRA, district government and NGOs for assistance in assessment, for re-verification of at-risk communities identified in the Pre-Winter Shelter Security Survey and identification of beneficiaries. In coordination with Church World Service (CWS), Kaghan and Sirin ERTs identified1000 vulnerable families in the valleys (UCs Mahandri, Jabar/Devli) for distribution of shelter kits.

8 3. Future planning

Tent-shelter construction will be complete in Kashmir by 13 January. Approximately total of 340 shelters will be built in Chattar Kalas, Industry, Agro Tech and Mera Tanolia camps, as per Camp Support Team requirement. Rapid Construction teams may shift operations to NWFP, dependent on land issues with the owners of camp site land.

A new ERT will be launched in the Kohistan valley. Following the emergency intervention involving the recovery following the serious RTA on 8 December, the District Commissioner, Kohistan, was very grateful for the assistance received from the ERTs. He requested IOM to sanction a new AIG-funded ERT for the Kohistan valley. Team members are being identified and will be trained in early January. This initiative will be well appreciated by the earthquake-affected District, which is one of the lowest districts in terms of development indicators in .

A survey of the self-settled camps in earthquake affected areas (i.e., families away from their areas of origin who have made camp-like settlements outside of government-supported camps) is planned. Similar to the registration and constraints survey carried out in government-supported camps, this survey will address the immediate needs of the residents in self-settled camps as well as the impediments to return.

Camp support team has not installed the new fire extinguishers in camps, so the planned fire safety training in camps has been postponed until further notice by the camp support team.

UNDP is holding Disaster Risk Reduction training of trainers in Abbottabad and Muzaffarabad. Operational Managers and Assistant Operational Managers will attend the two-day trainings and thereby be able to give further technical training to the teams.

ERTs are also planning training of Municipal Corporation Muzaffarabad employees and also a training for the Kashmir media community.

4. IN FOCUS- EMERGENCY RESPONSE TEAM DYNAMICS

Introduction

On 8 October 2005, at 8.50 PST an earthquake measuring 7.6 on the Richter scale occurred in Pakistan, Afghanistan and India. The epicenter of the earthquake was located 19km northeast of Muzaffarabad, along a fault line associated with the Indian subcontinent moving northwards at rate of 40 mm/py into the Eurasian continent. More than 1000 aftershocks were recorded by 27 October ranging from 5.0 to 6.0 on the Richter scale.

This earthquake is cited as the worst natural disaster Pakistan has suffered in terms of the number of lives lost and injuries sustained as well as the devastating impact on the economy and infrastructure in the affected regions. Reported by Masaid Ullah The eastern districts of the North West Frontier Province (NWFP) and Pakistan Field Assessment Officer Administered Kashmir (PAK) suffered the full force of the earthquake. The area Neelum Valley ERT is still under stress from the unexpected emergencies of winter due to vast topographic, economic, social and political upheaval. To cope with these emergencies IOM has launched the Emergency Rapid Response project with the help of American International Group Disaster Relief Fund .There are ten teams working and covering the whole earthquake area.

9 Each Emergency Response Team (ERT) consists of three members: an emergency medical doctor, a mountain search and rescue guide and a field assessment officer. We are the three members of ERT base in Patika, in the Neelum valley of Kashmir. Neelum valley is one of the most affected valleys of the earthquake. The basic responsibility of the team is Assessment, Training and Emergency Intervention.

How do the ERTs Work?

In ERT there are three team members for the above mentioned three main tasks. Every member is a specialist of his field. The mountaineer knows how to cope with emergencies in hard, tough and cold locations and how to plan and execute mountain operations. The medical doctor specializes in training the first responders, providing emergency medical care and ensuring the team is physically fit . The assessment officer is the coordinator of the team. He is responsible to liaise with the community and ensures that the ERT utilizes the training and intervention services in the right place, at the right time and for the right people. . (a) Assessment Planning and Mobilization

The ERT makes a work plan for the upcoming week. This is a flexible plan: adaptable in case of bad weather or some emergency. We prioritize high-risk areas in terms of altitude, accessibility, and community’s plans (like we had to postpone a first aid training due to grass cutting in some villages). The field assessment officer has to contact the right person in the community for data collection and ask the right questions to the right people to match disaster risks with training modules as well as objectively survey communities in terms of general shelter/snow coverage/accessibility situations. He has to introduce his team members and vision of the AIG project.

(b) First Responder Training

The field assessment officer sets dates and times and mobilizes the first responders for trainings in the communities. Before starting the training, the trainers (doctor and mountaineer) are introduced to the participants. Then the trainers begin with the training modules.

In the training of first responders, the emergency hotline number of our field station is stressed. It is conveyed that the ERT is ready to respond in case of any emergency in the Neelum valley. During coordination with organizations working in the area, they are requested to inform us in case of any emergency in their coverage areas.

The trainers are facilitated by the assessment officer. We try to make our trainings as interactive as possible.

(c) Emergency Intervention

The mountaineer is the key person during an emergency intervention, particularly high altitude mountain rescues. In harsh weather and terrain, he guides the team to the location and ensures safe procedures during the action.

Most interventions require immediate on site emergency medical care. The doctor has supplies in the emergency medical kit and makes a team decision as to whether the ERT has the capacity to deal with the situation on site or if the patients need road or heli evacuation to tertiary care units. In these instances, satellite communication is used to contact heli service, or additional support from the head office.

10 The ERTs challenge community stereotypes which label NGO and UN work as a luxurious lifestyle. The field efficiency of the teams is evident through high altitude assessments, first-responder trainings and interventions in the difficult to reach mountains and valleys of the earthquake affected regions. Through the coordination of the professional skills of individual team members, ERTs are able to effectively carry out the dangerous and unpredictable work in the field. Community and government appreciation of the passing on of skills through first responder training and professional response to emergencies encourages our team to continue the work.
