Ischemic Cardiomyopathy: Symptoms, Causes, & Treatment
Ischemic Cardiomyopathy Ischemic cardiomyopathy is a condition that occurs when the heart muscle is weakened due to insufficient blood flow to the heart's muscle. This inhibits the heart's ability to pump blood and can lead to heart failure. What Is Ischemic Cardiomyopathy? Ischemic cardiomyopathy (IC) is a condition that occurs when the heart muscle is weakened. In this condition, the left ventricle, which is the main heart muscle, is usually enlarged and dilated. This condition can be a result of a heart attack or coronary artery disease, a narrowing of the arteries. These narrowed arteries keep blood from reaching portions of your heart. The weakened heart muscle inhibits your heart’s ability to pump blood and can lead to heart failure. Symptoms of IC include shortness of breath, chest pain, and extreme fatigue. If you have IC symptoms, you should seek medical care immediately. Treatment depends on how much damage has been done to your heart. Medications and surgery are often required. You can improve your long-term outlook by making certain lifestyle changes, such as maintaining a healthy diet and avoiding high-risk behaviors, including smoking. Subject and Methods: We enrolled 302 consecutive patients with inferior-wall AMI who were treated with percutaneous coronary in- tervention (PCI). Patients were categorized into 2 groups based on electrocardiograms before and after PCI: the J group (J waves in in- ferior leads and/or rJ waves in leads V1-V3) and the non-J group (no J wave in any of the 12 leads). We compared patients with high am- plitude (>2 mV) J or rJ waves (big-J group) with the non-J group. The cardiac and all-cause mortality at 6 months and post-PCI ventricular arrhythmic events ≤48 hours after PCI were analyzed. Results: A total of 29 patients (including 19 cardiac death) had died. Although all-cause mortality was significantly higher in the post-PCI J group than in the non-J group (p=0.001, HR=5.38), there was no difference between the groups in cardiac mortality. When compar- ing the post-PCI big-J group with the non-J group, a significant dif- ference was found in all-cause mortality (n=29, p=0.032, HR=5.4) and cardiac mortality (n=19, p=0.011, HR=32.7). -

ElAmm Purpose of review The aim of this study is to summarize the literature describing the pathogenesis, diagnosis and management of cardiomyopathy related to myocarditis. Recent findings Myocarditis has a variety of causes and a heterogeneous clinical presentation with potentially life- threatening complications. About one-third of patients will develop a dilated cardiomyopathy and the pathogenesis is a multiphase, mutlicompartment process that involves immune activation, including innate immune system triggered proinflammatory cytokines and autoantibodies. In recent years, diagnosis has been aided by advancements in cardiac MRI, and in particular T1 and T2 mapping sequences. In certain clinical situations, endomyocardial biopsy (EMB) should be performed, with consideration of left ventricular sampling, for an accurate diagnosis that may aid treatment and prognostication. Summary Although overall myocarditis accounts for a minority of cardiomyopathy and heart failure presentations, the clinical presentation is variable and the pathophysiology of myocardial damage is unique. Cardiac MRI has significantly improved diagnostic abilities, but endomyocardial biopsy remains the gold standard. However, current treatment strategies are still focused on routine heart failure pharmacotherapies and supportive care or cardiac transplantation/mechanical support for those with end-stage heart failure. However, there was no literature description of supraventricular tachycardia (SVT) as the initial presentation of IE. Herein we report a case of bacterial IE with SVT as the initial presentation of disease, which finally leaded to catastrophic outcome. -

It provides talking points clinicians can use when discussing the benefits and risks of these vaccines with their patients and offers guidance on what to do if they have a patient who presents with myocarditis following vaccination. Myocarditis information What are myocarditis and pericarditis? • Myocarditis is an inflammation of the heart muscle. • Pericarditis is an inflammation of the heart muscle covering. • The body’s immune system can cause inflammation often in response to an infection. The body’s immune system can cause inflammation after other things as well. What is the connection to COVID-19 vaccination? • A CDC safety panel has determined there is a “probable association” between myocarditis and pericarditis and the mRNA COVID-19 vaccines, made by Moderna and Pfizer-BioNTech, in some vaccine recipients. • Reports of myocarditis and pericarditis after vaccination are rare. • Cases have mostly occurred in adolescents and young adults under the age of 30 years and mostly in males. • Most patients who developed myocarditis after vaccination responded well to rest and minimal treatment. Unstable angina with tachycardia: Clinical and therapeutic implications We prospectively evaluated 19 patients with prolonged chest pain not evolving to myocardiai infarction and accompanied with reversible ST-T changes and tachycardia (heart rate >lOO beats/min) in order to correlate heart rate reduction with ischemic electrocardiographic (ECG) changes. Fourteen patients (74%) received previous long-term combined treatment with nifedipine and nitrates. Continuous ECG monitoring was carried out until heart rate reduction and at least one of the following occurred: (1) relief of pain or (2) resolution of ischemic ECG changes. The study protocol consisted of carotid massage in three patients (IS%), intravenous propranolol in seven patients (37%), slow intravenous amiodarone infusion in two patients (lo%), and intravenous verapamil in four patients (21%) with atrial fibrillation. In three patients (16%) we observed a spontaneous heart rate reduction on admission. Patients responded with heart rate reduction from a mean of 126 + 10.4 beats/min to 64 k 7.5 beats/min (p < 0.005) and an ST segment shift of 4.3 k 2.13 mm to 0.89 k 0.74 mm (p < 0.005) within a mean interval of 13.2 + 12.7 minutes. Fifteen (79%) had complete response and the other four (21%) had partial relief of pain. A significant direct correlation was observed for heart rate reduction and ST segment deviation (depression or elevation) (f = 0.7527 and 0.8739, respectively). These patients represent a unique subgroup of unstable angina, in which the mechanism responsible for ischemia is excessive increase in heart rate. Pérez Riera The J wave is a positive deflection in the ECG normal (present in 2%–14% of healthy individuals and is more prevalent in young males, particularly if athletic and African descent. Additionally, ERP is a common finding in young teen athletes (the prevalence in the athletic population rises to 20-90%.).1 In this population ERP in both inferior and lateral leads is more common (18.2%) than isolated inferior (9.1%) or lateral (8.2%) ERP. Hypertrophic cardiomyopathy (HCM) is a complex type of heart disease that affects the heart muscle. It causes thickening of the heart muscle (especially the ventricles, or lower heart chambers), left ventricular stiffness, mitral valve changes and cellular changes. Thickening of the heart muscle (myocardium) occurs most commonly at the septum. The septum is the muscular wall that separates the left and right side of aortic valve narrowed the heart. Stiffness in the left ventricle occurs as a result of cellular changes that occur in the heart muscle when it thickens. The left ventricle is unable to relax normally and fill with blood. Since there is less blood at the end of filling, there is less oxygen-rich blood pumped to the organs and muscles. The stiffness in the left ventricle causes pressure to increase inside the heart and may lead to the symptoms described below. -

BADEMOSI From the Departments of Medicine and Paediatrics, University College Hospital, Ibadan, Nigeria SYNOPSIS Thirty-eight percent of patients with infective endocarditis (36 of 95) had neuropsychi- atric manifestations. In 750 (27 of 36), these features were the major presenting picture. Fifteen patients (42%) presented with cerebrovascular lesions and seven (19%) with meningitis. Toxic encephalopathy (12.5%) was not uncommon. Other neurological syndromes seen included psychosis and spinal cord lesions. The mortality was high especially when the infective endocarditis was acute in onset. It is essential to search diligently for an underlying cardiac cause in patients who present with neuropsychiatric symptoms because treatment of the underlying pathology improves prognosis.
