 US 82, Artesia to Lovington, New Mexico. Mr. Borsack was the SUE Task Lead for SUE Level A-D services for the 2 phase 30 mile US 82 project and Utility Coordinating Lead for 15 miles in Phase 1. Over 900,000 LF of utilities were mapped for both phases. Mr. Borsack assisted Parsons Brinckerhoff in the request of utility certification documents, including Phase 1 30% and 60% UC presentation to the utility owners, dissemination of all design submittals and collection of utility documents provided by the utility owners for the NMDOT.  AT&T Texas El Paso. Mr. Borsack was the telecommunication design coordinating manager in 2016 for several AT&T Texas projects in the El Paso area. The projects included the downtown Streetcar project along Street, Stanton Street, Kansas Street, Franklin Avenue, and Father Rahm Avenue. Other civil project design coordination included Darrington Road, Rojas Drive, Eastlake Boulevard, Mission Ridge Road, Alameda Road (SH 20) and Vista del Sol. In addition to conflict analysis following SUE Level A and B, Mr. Borsack coordinated the civil and OPTI cable design packages for AT&T Texas, which included permitting for TxDOT and the Cities of El Paso and Horizon for SUE and manhole mapping, storm-water pollution prevention and construction administration services. A sum of over 10 miles of new conduit facilities were designed.  El Paso Streetcar Project, AT&T Texas/SWBT Proposed Conduit Relocation, ElPaso, Texas. Utility Coordinator/Project Manager. Mr. Borsack’s responsibilities included conflict identification using SUE and manhole mapping analysis, conduit/manhole system quality design review for replacement for all cables to eliminate conflicts with the original streetcar alignment and profile section. He also provided agreement cost estimates following his review of labor and material quantities and overhead. He also assisted AT&T in evaluation of alternative concepts by the CRRMA and Streetcar contractor, following his letters of conflict avoidance recommendations. This project consisted of AT&T utility relocation/coordination for a proposed urban streetcar system. Researching and identifying horizontal and vertical location of approximately 63,000 LF of existing AT&T civil structure and cable that would be impacted by the streetcar and related utility construction. It also included determining the existing condition of each manhole structure that provided splicing locations, cable rack and access position and evaluate limitations to operations and maintenance of cable and conduit if existing access locations were to remain in place. $97 million (est. project construction cost).  I-635 LBJ Managed Lanes Project, Trinity Infrastructure. Mr. Borsack was the Utility Engineering Design Coordination and SUE Manager for the LBJ Managed Lanes project which was implemented to relieve severe congestion on 13.3 miles of IH 635 (LBJ) from just west of IH 35E (near Luna Road) to just east of US 75 (near Greenville Ave.). Not only mapping over 300,000 LF of Quality Levels B and 550 Quality Level A test holes between September 2010 and April 2013 and coordinating the test hole profile data as part of PS&E preparation for developer and owner managed Utilities, Mr. Borsack quality reviewed each PS&E package prior to TxDOT and utility owner review and approval. Value added joint trenches, joint bores and reroutes were implemented by meeting utility owners for approval and considering specific owner master utility development plans. In so doing, some alignments were rerouted and thus eliminated. The PS&E packages for 58 MUAA and UAAA utility assemblies for longitudinal and crossing relocations were sequenced in Sections to accommodate the LBJ managed lane accelerated master construction schedule. He ensured proper approval documentation to ensure horizontal and vertical separations between water and wastewater. He calculated and included betterment ratios in the assembly estimate where applicable. Mr. Borsack coordinated and documented exception requests in rare instances where utility crossings were confined to specific alignment locations with design option cost and constructability comparisons and clarifications of how existing and future traffic movements would not be impeded by utility operations and maintenance. Mr. Borsack formulated an abandonment request of a 36” transmission water line with DWU approval.

o Utility Coordinator/Project Manager. Mr. Borsack was responsible for Utility Design coordination and assembly management for reimbursable and non-reimbursable utility relocations. He completed PS&E packages and reviewed for approval agreement documents for 8 fiber companies, 1 major telecommunication company, water and sewer owned by 2 municipalities, 1 major electric distribution and transmission company and 1 major gas company. He also coordinated easement documents, betterment calculation ratios and cost estimates for each utility design, and coordinated for approval utility installation permits during the project.  IH 15, The Point, SR 92 to 12300 South, Utah Department Of Transportation Region 2. Mr. Borsack served as Project Utility Design Coordinator for the coordination and design for the relocation of 28 utility companies (both Peter L. Borsack, P.E. - Page 2

public and private) along 8.5 miles of IH 15. The $215 million project expanded IH 15 to six lanes in each direction from S.R. 92 to 12300 South, replaced the aged concrete pavement with new, durable pavement, replacde the bridges, and realigned the East Frontage Road intersection at the 14600 South interchange in Bluffdale to accommodate future interchange modifications and enhanced the existing traffic management system by installing additional ramp meters, fiber optics, and other infrastructure to improve mobility. CobbFendley was responsible for the relocation or protection coordination and/or developing utility relocation design packages for the all utilities affected by the improvements. Our design package included the relocation of all utilities at Minuteman and the extension along 14600 South (Highland Drive), as well as conflict resolutions from the City of Lehi to Riverton City along IH 15, as well as utilizing joint trench alternatives. Mr. Borsack coordinated and managed telecommunication design relocation for project retaining walls near the Salt Lake County Line at Pony Express. Fiber optic cable, copper cable and conduit were relocated along a new Minuteman Drive intersection with 14600 South and cable design was directed through OSP (outside plant) design system. A major joint conduit and fiber alignment that included AT&T Teleport and Business relocation of fiber was avoided by the direction of conduit adjustment by CobbFendley in order to avoid costly fiber relocation for various end to end splices that were approximately 10,000 lf in length, thus allowing the retaining wall to be constructed without waiting on fiber transfer customer notification and fiber procurement and splicing. Mr. Borsack coordinated SUE techniques in Level B toning and Level A test holes performed by Cobb Fendley. Available fiber optic cable slack was obtained to confirm the constructability of horizontally shifting the conduit without compromising the cable design specifications. CobbFendley performed a joint trench design relocation package for three major water lines, two of which were joint trenched with a large irrigation line at Minuteman Drive and 14600 South. The design was completed to meet water connection schedules in the Spring of 2015 in conjunction with a major pump station improvement project for Draper City. The joint trench allowed the contractor to expedite construction of three separate entities in the same mobilization by saving up to $100,000. CobbFendley coordinated design for owner managed overhead and underground utility construction at Minuteman and I 15 by utilizing project survey control, and adding survey staking information for overhead and underground profiles as necessary to clear crossing utilities. CobbFendley found buried facilities that were not detectable by standard utility managed methods.  Squaw Creek Road in the Phoenix Mountain Preserve, Phoenix, AZ. CobbFendley provided utility mapping for 12,500 Linear Feet (LF) of SUE Level B and 4 Level A test holes incorporated into the PEC topographic survey along a mile of Squaw Creek Road.

Robert Gromatzky, P.S. Survey Project Manager

Project Experience Santolina Level A, Western Albuquerque Land Holdings, Bernalillo County, New Mexico. Mr. Gromatzky was the survey project manager and professional surveyor in responsible charge for the Santolina Level A project. The project established a master planned development on the west side of Albuquerque and consisted of 14,000 acres of land. The overall development is expected to be completed over a 70 year period and will provide for mixed use, industrial, commercial, and residential uses. The Level A agreement with Bernalillo required that the entire 14,000 acre development be platted. The property was originally part of the Town of Atrisco Land Grant and had been in an unplatted state until Mr. Gromatzky surveyed and platted the parcel of land. Brazito Sewer Line, Lower Rio Grande Public Water Authority, Mesquite, New Mexico. Mr. Gromatzky was the survey project manager and professional surveyor in responsible charge for the Brazito Sewer Line Project. When constructed, the project will provide sewer service to a rural community and will replace existing septic systems in the area. The project includes over 100,000 LF of sewer pipe alignment and 5 sewer lift stations. The survey included setting photo control and processing the control though OPUS projects, available by the NOAA/NGS, aerial imagery acquisition and mapping at a 1”=50 scale with 1 foot contours. Additional ground surveys to collect utility and subsurface utility locations were employed. Right of Way surveys were completed to determine existing right of way corridors that the utility could be located within to reduce the financial impact on the client for acquisitions and disturbance of the adjoining land owners. Boundary surveys of the parcels for the 5 lift stations were required as well. NM 136, New Mexico Department of Transportation, Santa Teresa, New Mexico. The NM 136 project is a roadway rehabilitation project in southern New Mexico that serves as a port connection to Mexico. Years of heavy truck traffic has caused serious damage to the roadway. The project consists of rehabilitating approximately 12 miles of state highway and includes a partial mill of the asphalt surface and an overlay of concrete paving. Mr. Gromatzky served as the professional surveyor in responsible charge and survey project manager for this endeavor. The project included a retracement of the existing Right-of-Way for the entire 12-mile corridor. A portion of the project was mapped using mobile LiDAR scanning to obtain precise milling volume calculations in the overlay area. The second half of the project was mapped using aerial mapping. Supplemental field surveys were performed to obtain profile data to verify the scanning data and obtain the roadway prism figure in the aerial mapping area. Utility surveys were conducted to provide locations of buried utilities as well. The data from the various and different technologies was verified and was merged into one cohesive data set. Kirtland Air Force Base Landfill Cap, Foster Wheeler, Albuquerque, New Mexico. The Kirtland Air Force Base is a military and science research facility located in Albuquerque, New Mexico. The facility has several disposal sites that include military components and common waste. Several sites are contaminated with jet fuel or nuclear material. The project required that a landfill cap be built over an existing disposal site that was approximately 100 acres in size. Foster Wheeler contacted Mr. Gromatzky to provide construction staking services. The site plans provided for several drainage channels and structures to move water over the disposal site into an adjoining

The project was staked using digital alignment information and a digital surface file that was computed in the office and directly uploaded into the field controllers. Mr. Gromatzky served as the project manager and professional surveyor with oversight. Additionally, periodic topographic surveys were performed to monitor the fill depth, and compute quantities for the client. A final as- built survey was provided at the completion of the project.w Chinle Department of Justice Facility, Dyron Murphy Architects, Chinle, AZ w The intent of the project was to build a new justice facility on an 80-acre site in Chinle, AZ. Aerial mapping was collected to support the engineering design effort. Surveys were conducted to collect additional topographic information, utility information, and boundary location for the facility. The project required the coordination of several agencies that include Navajo Tribal Utility Authority, Navajo Department of Transportation, Bureau of Indian Affairs, Bureau of Land Management, Bureau Indian Land Surveys, and the Chinle Chapter Government.

Andrew Mecham, P.E. Subsurface Utility Engineering Task Lead

Project Experience

 SR-89, Willard Bay Shoulder Widening, Davis County, Utah, UDOT Region 1. Mr. Mecham provided SUE Services for this project that will add an 8-foot safety shoulder along SR-89 from SR-126 and I-15 to 3.5 miles north. CobbFendley’s responsibilities include coordinating with UDOT to locate more than 300,000 LF of utilities to Level D/C standards, designate approximately 50,000 LF of utilities to Level B standards and provide Level A locating services for approximately 25 test holes for the proposed shoulder widening. Provide 30 UT sheets and coordinate relocations with the utility companies.

 Center Street, Legacy Parkway to Redwood Road Intersection Improvements, Salt Lake City, Utah, UDOT Region 1. Mr. Mecham provided SUE services for this roadway reconstruction project on Center Street from Redwood Road to where the Legacy Parkway Project stopped. It also continued Legacy Parkway trail on the south side of Center Street to the Redwood Road project. Mr. Mecham helped perform SUE, coordinate utility conflicts with North Salt Lake City, Questar Gas, Rocky Mountain Power and Century Link.

Sean M. Wolfe, P.G. Principal-in-Charge Project Experience

 US 82, Artesia to Lovington, NMDOT. Mr. Wolfe served as Project Manager responsible for over 60 miles of utility identification and mapping and utilized a process to simultaneously mark and map the existing underground utilities along a 60 mile stretch of US 82. Over 1,000,000 linear feet of existing utility infrastructure was mapped to project tolerances and a Quality Level B file in AutoCAD produced in just 8 weeks. This project was not only completed on time, but the original cost of such a project was well over three times what CobbFendley’s final cost amounted to. This project is currently in final design stages where CobbFendley is completing a Utility Certification for the NMDOT.

 NM 188 & NM 28 (Valley Drive), Las Cruces, New Mexico, NMDOT. Mr. Wolfe served as Project Manager for this project. SUE Quality Levels D, C and B services were completed. CobbFendley identified all utility owners affected by the project and performed record research (SUE Level D) with each to obtain the most up to date as built information available. During the record research phase of work, CobbFendley’s SUE technicians also performed field investigations to identify/verify all above ground appurtenances related to utilities; including all manholes, handholes, pull boxes, etc. (if accessible). Upon completion of the record research and utility contact, existing utilities were be designated within the project limits. Designate means to record and mark the horizontal location of the existing toneable utility facilities and their service laterals to existing buildings using non-destructive surface geophysical techniques. All deliverables for this project were delivered in AutoCAD Civil 3D format. The project was delivered on- time and on budget with no variances.

 NMDOT I-25 / Paseo del Norte, Albuquerque, New Mexico, NMDOT. CobbFendley, as part of the Wilson & Company team, provided the latest update to the existing SUE Level D-A information. The SUE data prepared by CobbFendley was used for the procurement of the Design/Build contractor for the active I-25/PDN project. In addition to traditional SUE services, CobbFendley provided a storm drain assessment using stationary and robotic CCTV equipment.

 On-Call Utility Locating, University of New Mexico. CobbFendley has responded to over 200 UNM On-Call utility designating requests during the past year and has toned, marked and updated UNM facility maps as a response to each regular, emergency or weekend/holiday request including locations throughout the Central Campus, Downtown, and Valencia Campuses. Tickets included emergency and standard on call requests from Sunday through Saturday. Our field teams have been called to respond quickly as well as meet with field representatives on-site to clarify the results of the task. Project field photos, sketches and notes have been performed in the field for purposes of field task tracking for future coordination. In addition to standard ticket requests, our field crews located utilities in a large residential housing area. After each field ticket was completed, our field crews updated the utility maps in the central planning office. CobbFendley has complete projects on schedule to the satisfaction of UNM.  Drainage Master Plan, City of Farmington, New Mexico. Mr. Wolfe served as Project Manager responsible for providing SUE Levels C & D services, video inspections, and GIS mapping for the City’s existing storm drainage system. After compiling the record data, CobbFendley correlated the above ground surface features with the collected utility information and prepared a color coded utility composite drawing, which was used for utility coordination and to identify conflicts. The storm drainage inspection services associated with this project were completed in two phases: Phase I: Manhole Inspection, which included location, condition and photographs of each manhole, and Pole Camera Line Inspections, and Phase II: CCTV Line Inspection.  Subsurface Utility Locating Services, City of Albuquerque, New Mexico. Mr. Wolfe is serving as Project Manager under this contract with the City of Albuquerque to provide On-Call Utility Locating services. Under this contract, CobbFendley is to accurately mark the horizontal location of existing subsurface utilities and, if needed, coordinate with contractors/consultants to have the lines either protected during construction or accurately identified during design. This on-call contract was recently re-written from a strictly utility potholing contract to a full service utility locating contract, which includes utility research, utility locating, surveying, CAD drafting, vacuum excavation, ground penetrating radar and utility coordination services. Each task under this three year contract is scoped specifically to meet the project needs and coordination is streamlined to ensure the facilities are marked as soon as possible and delays are avoided.  Laguna Pueblo Water Facility Upgrades, Laguna, New Mexico. Mr. Wolfe served as Project Manager for the SUE Level A (pothole) project related to the Pueblo’s water facility upgrade program. Working closely with the design consultant, CobbFendley assisted in conflict analysis and completed over 50 test holes at specific locations critical to Sean M. Wolfe, P.G. - Page 2

the proposed design. As a result of this effort, unknown utilities were identified and confirmed locations of known lines were found to be, in some cases, far different than expected.  Utility Locating, Various Public Schools, New Mexico. As subconsultant, CobbFendley currently provides Utility Locating services at multiple public school locations throughout the State. For each, CobbFendley utilized what available record information was provided and accurately marked the location of existing utilities. Coordinating with survey crews, CobbFendley ensured all marked lines were surveyed and the CAD file was reviewed by CobbFendley prior to submittal. Some of these school locations include: Alameda MS, Atalaya ES, Belen HS, Colinas del Norte ES, Santa Fe HS, SER Academy, Martin Luther King ES and Santa Fe HS Stadium.  Carnuel Waterline Extension, Albuquerque, New Mexico, Albuquerque Bernalillo County Water Utility Authority (ABCWUA). Mr. Wolfe served as Project Manager for the SUE Level D, B and A services on this water line project between Albuquerque and Carnuel, New Mexico. Coordination with utility owners to obtain current record information, review and recommendation of conflicts, Level A test holes at critical locations and assisting the ABCWUA in accurately locating their line. CobbFendley found multiple facilities owned by the ABCWUA to be grossly miss-marked by their locators. To minimize conflicts with their utilities in the future, CobbFendley exposed the lines and placed Electronic Marker Balls, supplied by the owner, at specific locations so the line could be accurately marked. These services were completed at no additional cost to the project or owner.  On-Call SUE Services, Bernalillo County, New Mexico. In September 2012, Bernalillo County requested the use of CobbFendley’s City of Albuquerque on-call Subsurface Utility Locating contract. This extension of the City’s Contract has allowed Bernalillo County to utilize CobbFendley’s Subsurface Utility Locating services on multiple Task assignments. The on-call Contract is currently active with both City and County.  IH 15, The Point, SR 92 to 12300 South, UDOT Region 2. Mr. Wolfe served as Project Manager for the coordination and design for the relocation of 28 utility companies (both public and private) along 8.5 miles of IH 15. The $215 million dollar project will expand IH 15 to six lanes in each direction from S.R. 92 to 12300 South, replace the aged concrete pavement with new, durable pavement, replace the bridges, and realign the East Frontage Road intersection at the 14600 South interchange in Bluffdale to accommodate future interchange modifications and enhance the existing traffic management system by installing additional ramp meters, fiber optics, and other infrastructure to improve mobility. CobbFendley is responsible for the relocation or protection coordination and/or developing utility relocation design packages for the following: AT&T METRO, American Fork, Central Utah Water Conservancy District, Metropolitan Water District, UTOPIA, City of Bluffdale, Veracity Networks, Geneva Rock Products, XO Communications, , CenturyLink, , Department of Natural Resources, City of Draper, Waterpro, Electric Lightwave, Jordan Valley Water Conservancy District, City of Lehi, City of Lehi Power, Provo Water Users Association, Rocky Mountain Power, Questar Gas, South Valley Sewer District, Syringa Networks, Verizon Business, Department of Corrections, Department of Technological Services, Bureau of Reclamation, and Zayo Group. Our design package includes the relocation of all utilities at Minuteman, as well as conflict resolutions from the City of Lehi to Riverton City along IH 15, as well as utilizing joint trench alternatives. Survey Experience  On-Call Surveying Services, Sandoval County, New Mexico, SSCAFCA. Mr. Wolfe is serving as Principal for topographic survey and SUE Quality Level A and B on parallel gas pipeline corridors. Two high-pressure gas pipelines are being mapped to Quality Level B under two task orders covering three separate pipeline crossings. Fourteen test holes were requested at critical locations and completed by CobbFendley.  On-Call Surveying Services, Bernalillo County, New Mexico. Mr. Wolfe is serving as Principal to provide professional land surveying services throughout the County limits. The scope of this contract provides for full land surveying services on both locally and federally funded projects.  NM 197 Erosion Control Mitigation Project, Sandoval County, New Mexico, NMDOT. Mr. Wolfe served as Principal for surveying services necessary for both the design and construction phases requested by NMDOT as an emergency task order. Initially, CobbFendley conducted topographic/SUE surveys of the NM 197 bridge crossing over the Rio Puerco River extending upstream approximately 2,500-feet and east approximately 0.7 miles to the intersection with US 550. Upon submittal of the completed topographic survey, construction slope-staking was conducted by CobbFendley for the new river meander channel per the provided NMDOT design. Finally, as-built surveys were conducted on the newly constructed meander channel and earthwork spoils piles. CobbFendley completed this project to the clients’ satisfaction on a tight schedule.
