Medical Centre Patient Participation Group Minutes Wednesday 11th September 2019

3. Primary Care Network (PCN)

WHMC now signed up to the Erewash PCN. This means the practice will work closer with practices to deliver local services. How the PCN evolves is down to the PCN and its leadership team. Evolution ranging from the appointment of allied health professionals, that include clinical pharmacists and a social prescriber, to redesigning health pathways and working more at scale. WHMC committed to local service delivery. This meaning delivering as many primary health care services from West Hallam with locally based clinicians.

The PCN is thus very new and only starting to agree its plans and objectives. A key part of this process to develop trust across all practices that form the PCN.

The PCN is expected to hold a patient awareness day in the near future to allow patients to find out more.

4. Surgery Facilities

Building works now completed. Over budget but more has been done than planned. The works now ensure the Practice is fit for purpose for many years to come and has provided more clinical space, totally overhauled the Dispensary to make a better and safer working environment and made the waiting room and reception desk DDA complaint.

RP suggested a practice Open Day. The PPG agreeing with SW suggesting this could help promote the PPG and raise awareness of the PCN. Based on a few recent comments in the community and on facebook it was agreed it would be beneficial to

PPG-Sep19 use the Open Day to outline how the practice works and in particular the appointment system.

RP/SW to progress this idea.

5. Social Prescribing

JH presented information regarding social prescribing: - There are 2 social prescribers now based in Erewash - Part of their role is ‘social connecting’. This means signposted residents living within Erewash to a range of local services. - They also run coffee mornings in Ilkeston. Most recent coffee morning at Littlewick Medical Centre. Any local resident can drop in for advice or to find out more.

To support this initiative JH has taken much time to produce a local directory of groups and activities for West Hallam, Stanley and .

RP to share this with practice staff and send copy to Izzy one of the Social Prescribers based in Ilkeston. RP to ask Izzy if she can keep the directory updated to ensure it remains a live document. RP thanked JH for producing such an excellent document. RP to produce a copy for SW.

6. Flu Season

With autumn fast approaching (if not already arrived) this means flu season. RP asked all of the PPG to raise awareness as we need to vaccinate as many patients aged 65 and over and those in ‘at-risk groups’ as possible.

The Practice having published the clinic dates and asking patients to contact the practice if they have any doubts concerning their eligibility. The Practice website also a good information resource.

7. Care Quality Commission (CQC)

The Practice inspection is due in the next year. This could be anytime with just 2 weeks notice before inspection. RP advised that this inspection will likely take the form of a targeted inspection upon which the CQC will focus in more depth on one particular area of service delivery. RP not sure how the CQC will deal with PCNs. Future inspections possibly at PCN level?

The PPG upon being asked confirming their help and support once the inspection date is known.

8. Any Other Business

Facebook and Community Comments

LS reported comments concerning appointment availability. RP stating the practice has good access and asked whether these concerns were regard to being offered an on the day ACP appointment whereas in the past on the day matters (infections and other acute matters) had been dealt with by a GP? The Practice having adopted the NHS Delivery model and thus ensuring GPs see and deal with the most complex and poorly patients and less urgent/serious/complex matters are now dealt with ACPs.

It was felt the Open Day be used to explain the model operated by the Practice.

RP to speak to LS outside of the meeting to better understand the concerns raised and allow him to investigate and respond.

RP making the PPG aware that health outcome data, referral data, emergency hospital admissions data and finally prescribing data is within the top performance quarter of all practices across . RP suggesting this is the most important data compared to recognise.

Care in the Community Meeting

PW/BW attended the above meeting. They were advised that moving forwards certain patients discharged to Ilkeston Hospital from Royal Hospital would be ‘stepped down’ to a care home provider. This due to the closure of the Hopewell Ward. In total this leaving 18 beds in the Ward and 8 beds within a local authority care home. Those patients in the care home can still access physio but as they are not in a hospital environment much concern was raised at this meeting and by the PPG at this evening’s meeting.

Mock Patients

JH made the PPG aware that the Royal Derby Hospital is seeking ‘mock patients’ to help with the training of future clinicians. They seek patients with specific conditions. Contact RDH to find out more. Tea and coffee provided.

Farewell and thanks

DC announced he would be leaving the area to live closer to his family. The PPG thanking him for his continued efforts over many years and wishing him well with this next chapter in his life.
