    ADA Statement on Dental Patient Rights and Responsibilities Background: The ADA Council on Ethics, Bylaws and Judicial Affairs (CEBJA) has developed the following template Dental Patient Rights and Responsibilities Statement (DPRR Statement) as a guide and as an aid to be used by constituent and component societies and practitioners in creating their own dental patients rights and responsibilities statements. In the DPRR Statement that follows, the term “rights” is used not in a legal sense, but merely to convey an indication that a patient should have an expectation of experiencing treatment in accordance with the enumerated “rights.” Several other dental and medical related organizations publish patient rights statements; indeed, CEBJA reviewed those statements during the development of the DPRR Statement, as well as Standard 5-3 of the ADA Commission on Dental Accreditation (CODA) Standards for Predoctoral Dental Education Programs, which also refers to a statement of patients’ rights. The DPRR Statement grew out of a collaborative ethics summit conducted in March 2006 by the American College of Dentists (ACD) and the American Dental Association (ADA) on the topic of commercialism in dentistry. Members of CEBJA were invited to attend along with representatives from ADA and ACD leadership, the ADA Council on Dental Education and Licensure, the recognized specialty groups, the National Dental Association, the U.S. Department of Veterans Affairs, the American Dental Education Association, dental school deans and faculty, ethicists, dental editors and leading representatives from the insurance, practice management and dental product manufacturers industry. The summit attendees noted that patients have become more assertive in seeking elective procedures and that the dental profession seeks to be mindful of protecting patient autonomy while balancing the importance of overall dental health and lifelong consequences.

    Dental Implants Placement of dental implants is a procedure, not an American Dental Association (ADA) recognized Dental Specialty. Dental implants like all dental procedures require dental education and training. Implant therapy is a prosthodontic procedure with radiographic and surgical components. Using a dental implant to replace missing teeth is dictated by individual patient needs as determined by their dentist. An implant is a device approved and regulated by the FDA, which can provide support for a single missing tooth, multiple missing teeth, or all teeth in the mouth. The prosthodontic and the surgical part of implant care can each range from straightforward to complex. A General Dentist who is trained to place and restore implants may be the appropriate practitioner to provide care for dental implant procedures. This will vary depending on an individual clinician’s amount of training and experience. However, the General Dentist should know when care should be referred to a specialist (a Prosthodontist, a Periodontist or an Oral and Maxillofacial Surgeon). Practitioners should not try to provide care beyond their level of competence. Orthodontists may place and use implants to enable enhanced tooth movement. Some Endodontists may place an implant when a tooth can’t be successfully treated using endodontic therapy. Maxillofacial Prosthodontists may place special implants or refer for placement when facial tissues are missing and implants are needed to retain a prosthesis. General Dentists are experienced in restorative procedures, and many have been trained and know requirements for the dental implant restorations they provide. However, if a patient’s implant surgical procedure is beyond the usual practice of a dentist, this part of the care should be referred to another dentist that is competent in placement of implants.

    GRADUATE PROGRAM IN DENTAL PUBLIC HEALTH Advanced Training Program The Department of Preventive and Community Dentistry offers an advanced training program in dental public health. The specialty of dental public health is one of the nine dental specialties formally recognized by the American Dental Association. The dental public health training program at the College of Dentistry fulfills all of the educational requirements for dentists wishing to complete the board-certifying examination given by the American Board of Dental Public Health. The graduate program in dental public health offers advanced training for both dentists and dental hygienists who are interested in careers in dental academic settings or dental public health administration. The program requires two years of full-time study and culminates in the awarding of a Master of Science (M.S.) degree in dental public health. The training program places an emphasis on research and includes the preparation and defense of a master's thesis. A minimum of 40 semester hours of coursework is required, including several electives. Most required courses are taken within the College of Dentistry, but some required courses and elective courses are taken through the Colleges of Public Health, Medicine, Education, Business Administration or Liberal Arts.     Group Dental Frequently Asked Questions About Periodontal Disease What is periodontal disease? Periodontal (gum) disease is an infection of the soft and hard tissue that support your teeth. This condition is caused when plaque—a sticky film of bacteria—builds up on the teeth and hardens into tartar. In early stages, the gums become inflamed and there may be some bleeding evident when brushing and/or flossing. This stage is called “gingivitis.” Gingivitis can be reversed with treatment from your dentist and good oral hygiene. If gingivitis is not treated, the disease may progress into “periodontitis,” where plaque and tartar build up below the gumline. Irritation and inflammation of the gums occur, creating pockets (increased space between your teeth and gums) that are infected. As periodontitis worsens, the pockets get deeper and the bone that supports the teeth is lost. If left untreated, this will eventually lead to tooth loss.1,2 How is periodontal disease diagnosed? If you’ve ever heard your dentist or dental hygienist calling out numbers and noting them in your dental chart at your regular checkup, then you’ve experienced “pocket probing” or “charting.” As part of a routine checkup, the dentist completes a periodontal examination to measure the depth of the spaces (“pockets”) between your teeth and gums, and checks for bleeding. Dental professionals need to understand the advantages and limitations of these therapies and use this knowledge in a positive approach that might help in decreasing den- tine hypersensitivity among patients.     Direct Access States The American Dental Hygienists’ Association (ADHA) defines direct access as the ability of a dental hygienist to initiate treatment based on their assessment of a patient’s needs without the specific authorization of a dentist, treat the patient without the presence of a dentist, and maintain a provider-patient relationship (ADHA Policy Manual, 13-15). Alaska 2008 Sec. 08.32.115 Collaborative Agreement: Dental hygienist may provide services according to the terms of a collaborative agreement. The dentist’s presence, diagnosis or treatment plan are not required unless specified by agreement. Care under the agreement can be provided in settings outside of the “usual place of practice” (i.e. private dental office). Requirements: Dental hygienist must have minimum of 4,000 hours of clinical experience within preceding 5 years. Agreement must be approved by state board of dental examiners. Dentists are limited to 5 or fewer collaborative agreements. Provider Services: Agreement can authorize nearly the entire dental hygiene scope of practice (patient education, prophylaxis, sealants, radiographs, etc). Arizona 2004/2015/ 2019 Sec. 32-1281, 32-1289 Affiliated Practice Agreement: Dental hygienist with a written affiliated practice agreement may perform dental hygiene services in specified settings outside the private dental office. The written agreement must be submitted to state board of dental examiners. The affiliated practice dental hygienist shall consult with the affiliated practice dentist before initiating further treatment on patients who have not been seen by a dentist within 12 months of the initial treatment by the dental hygienist. Requirements: Dental hygienist must have held an active license for at least 5 years and be actively engaged in dental hygiene practice for at least five hundred hours in each of the 2 years immediately preceding the affiliated practice relationship.

SPECIALIST ANNOUNCEMENT OF CREDENTIALS IN NON-SPECIALTY INTEREST AREAS Ethical Advertising under ADA Code: The American Dental Association recognizes the right of its members to promote their practices within the ethical standards set forth in the ADA Principles of Ethics and Code of Professional Conduct (“Code”). Section 5.F. ADVERTISING of the Code states: ALTHOUGH ANY DENTIST MAY ADVERTISE, NO DENTIST SHALL ADVERTISE OR SOLICIT PATIENTS IN ANY FORM OF COMMUNICATION IN A MANNER THAT IS FALSE OR MISLEADING IN ANY MATERIAL RESPECT. The standard of “false or misleading in any material respect” is accepted in ethics and the law to distinguish between permissible and non-permissible advertising. The rationale for this standard is the protection of the public. Truthful, nondeceptive advertising of a dentist’s qualifications, services or facilities can help patients make informed choices about practitioners and services. On the other hand, advertising which is false or misleading can harm patients by making it more difficult and costly for them to make informed choices. Ethical Concerns Raised by Advertising of Credentials by Specialists: Section 5.H. ANNOUNCEMENT OF SPECIALIZATION AND LIMITATION OF PRACTICE, of the Code sets forth the General Standards for determining the education, experience and other appropriate requirements for announcing specialization and limitation of practice.
