Myopathy Associated with Pigmentary Degene- Ration
MYOPATHY ASSOCIATED WITH PIGMENTARY DEGENE RATION OF THE RETINA AND HIGH PROTEIN CONTENT OF CEREBROSPINAL FLUID JOSÉ ANTONIO LEVY*; MlLBERTO SCAFF*; ANA MARIA C. TSANACLIS**; VANDERLEI GARCIA RODRIGUES**; EDGARD SILVA LUSVARGHI** Harenko and Lapallainen4 (1962) reported a case of chronic progressive ophtalmoplegia with pigmentary degeneration of the retina, also referring 5 similar reports. Assis1 (1967) published the case of a 16-year-old female patient with progressive ophthalmoplegia, which began with palpebral ptosis and pigmentary degeneration of the retina, spreading to the macular regions; biopsy of the left superior rectus muscle showed a dystrophic process, i.e., a myopathy. Olson6 reported 7 cases of progressive ophthalmoplegia (patients' ages varied between 11 and 47 years, the period of time from the onset of the disease varying between 3 and 12 years) in which the biopsy of clinically normal limb muscles showed alterations confirming the existence of a myo pathy. A biopsy revealing myopathy and the external ophtalmoplegia were common in all these patients. Three of them displayed pigmentary retinosis; three had a slight motor deficit in the limb girdle muscles; four had electro- encephalographic abnormalities; in 5 of the cases which underwent a cerebro spinal fluid examination, a high protein content was encountered; in four cases the muscle biopsy showed alterations which suggested a lesion of the peripheral motor nerve; none of the cases suggested progressive muscular dystrophy with serious motor deficit in the limb girdle muscles. Kearn, quoted by Engel2, reported the case of a myopathic patient with external ophtalmo plegia associated with cardiomyopathy and pigmentary degeneration of the retina. -

Methods: 46 patients with a focal cerebral hemisphere lesion without obvious focal signs and 19 controls with normal imaging were examined using a battery of clinical tests. Abstract: Detrusor sphincter dyssynergia (DSD) is the urodynamic description of bladder outlet obstruction from detrusor muscle contraction with concomitant involuntary urethral sphincter activation. DSD is associated with neurologic conditions such as spinal cord injury, multiple sclerosis, and spina bifida and some of these neurogenic bladder patients with DSD may be at risk for autonomic dysreflexia, recurrent urinary tract infections, or upper tract compromise if the condition is not followed and treated appropriately. It is diagnosed most commonly during the voiding phase of urodynamic studies using EMG recordings and voiding cystourethrograms, although urethral pressure monitoring could also potentially be used. DSD can be sub-classified as either continuous or intermittent, although adoption of this terminology is not widespread. There are few validated oral pharmacologic treatment options for this condition but transurethral botulinum toxin injection have shown temporary efficacy in reducing bladder outlet obstruction. Urinary sphincterotomy has also demonstrated reproducible long term benefits in several studies, but the morbidity associated with this procedure can be high. Simple Summary: The small size of the feline mandible makes its manipulation difficult when fixing dislocations of the temporomandibular joint or mandibular fractures. In both cases, non-invasive techniques should be considered first. When not possible, fracture repair with internal fixation using bone plates would be the best option. Simple jaw fractures should be repaired first, and caudal to rostral. In addition, a ventral approach makes the bone fragments exposure and its manipulation easier. However, the cat mandible has little space to safely place the bone plate screws without damaging the tooth roots and/or the mandibular blood and nervous supply.
