INTRODUCTION This report provides a snapshot of the health, wellbeing, and major issues facing the population in the Manchester region at various levels of geography depending on the data source – Manchester neighborhoods, City of Manchester, and Greater Manchester. Greater Manchester includes both the Greater Manchester Public Health Region (Auburn, Bedford, Candia, Deerfield, Goffstown, Manchester, and New Boston), as well as the Hospital Service Area (Public Health Region Towns plus Londonderry). Sources are noted accordingly throughout the report; including when the data is for the Hospital Service Area (HSA) specifically. The development of this report was a joint community effort spearheaded and guided by the City of Manchester Health Department in partnership with Catholic Medical Center and Elliot Health System. Among other things, this report is intended to satisfy the requirements for all Manchester area health care charitable trusts in connection with the periodic development of a community health needs assessment as required by the Affordable Care Act, as well as State law. Funding for this project was provided by all three partner organizations, including grant funding from The Kresge Foundation. Technical assistance and support to this effort, including the development and summary of all qualitative data and report design, were provided through a contract with the Community Health Institute of Bow, New Hampshire.     JOURNEY TO HEALTH An Analysis of Hospital-Discharged Medical Visits for the Greater Nashua and Milford, NH Region Data Brief BACKGROUND Figure 1: In-State Hospital Choice Among NRPC Residents All licensed hospitals in New Hampshire report patient-level discharge information to the New Hampshire Department of Health and Human Services (NH DHHS), who also collects data from select rehabilitation hospitals, rehabilitation and psychiatric units within acute care hospitals, and from free-standing ambulatory surgical treat- ment centers that are part of a hospital. These discharge data, known as the New Hampshire Uniform Healthcare Facility Discharge Data Set (UHFDDS), can be released to entities upon request for independent study and analysis. The following analyses are based on a custom report of aggregated UHFDD data for the NRPC region for the years 2016 and 2018. 2018 repre- sents the most current data available from NH DHHS, and 2016 is the latest year for which vis- its to out-of-state facilities is included. Trip types include Inpatient, Outpatient, Emergency Department, and Specialty, but do not distin- guish trips by more specific diagnostic code. For these category breakdowns that resulted in greater than 100 trips, the data could be further grouped into the following age cohorts: 0-19, 20 -40, 45-64, 65-74, and 75+.     New Hampshire Doctor Named 2018 Family Physician of the Year Suncook family physician recognized for his compassion and expertise caring for people who battle pain and addiction SAN ANTONIO — The American Academy of Family Physicians today awarded its highest honor to Gerard J. Hevern, MD, FAAFP, of Suncook, New Hampshire. Hevern was named the AAFP’s national 2018 Family Physician of the Year, which honors one outstanding American family physician who provides patients with compassionate, comprehensive care, and serves as a role model in his or her community and to other health professionals. As the United States finds itself amid an opioid crisis, Hevern’s unique combination of excellence and experience as a family physician, an addiction specialist, and a pain management specialist make him an ideal and timely recipient for this year’s award, which he accepted during the AAFP’s annual meeting, the Family Medicine Experience. For nearly four decades, Hevern has practiced the full scope of family medicine in a medically underserved community of 5,000 people in southern New Hampshire. It is here that he continues to provide the full spectrum of care ranging from obstetrics to pediatrics to hospital care and nursing home care of his patients as part of the Elliot Physician Network.
