From the beginning of her career she recognized this con- nection and understood the complexity of health problems and peoples’ needs when seeking care. Her commitment took the form of both scientific output and also her contributions to the various fields in which she worked. Methods: A parallel, cluster-randomized trial in which 20 public primary Health Care Units (PHCUs) were randomly allocated to the intervention (10 PHCUs) or control (10 PHCUs) group. The study population included pregnant women aged 18 or above with a gestational age of 20 weeks or less. Pregnant women receiving ANC in intervention PHCUs were invited through leaflets and posters to register in PRENACEL. Women who registered in PRENACEL received a weekly set of short text messages with health education and health promotion content related to pregnancy and childbirth and were also able to clarify ANC queries through SMS. All women received routine ANC. The primary outcome was the proportion of women with high ANC Score, a composite measure of coverage of recommended ANC practices. Chi-square or Fisher’s exact tests and multivariate log-binomial regression were used to analyze the outcomes. Results: A total of 1210 eligible women received ANC in the participating PHCUs and took part of this study (770 in the intervention group and 440 in the control group).

Methods: A sample of 1,848 dentists were interviewed (1,819 valid responses) working in the PHC of 26 capitals and the Federal District. The Effectiveness of the Assessment Tool for the Promotion of Oral Health Strategieswas used. It is composed of 23 indicators grouped into three dimensions: oral health, health public policies, and human and social development. The answers were arranged in a Likert scale (1–5), and the final score obtained for each sample unit can range from 23 to 115. Higher score values indicate greater potential for the strategy to promote oral health. Results: Statistically significant differences were identified among the analyzed geopopulation units considering the study object. The Southeast and South regions had better performance for the OHP strategies in comparison to the other regions of Brazil (p ≤ 0.01). Conclusions: The OHP strategies identified in the study were heterogeneous, with better results favoring the Southeast regions, with disadvantages for people living in capitals from the Central‑North‑Northeast of Brazil. Efforts should be undertaken aiming to qualify the PHC teams, especially for those in disadvantageous regions.

Brazil is one of at least 115 countries that recognise a constitutional right to health.1 An important part of this right is access to pharmaceutical drugs.2 Although Brazil has the developing worldʼs most advanced HIV/AIDS programme, many of its citizens still go to local pharmacies only to find that essential medicines are out of stock. Brazil is also one of the fastest growing pharmaceutical markets in the world. Doctors increasingly prescribe and patients demand new drugs, some of questionable benefit. Faced with high cost or no availability, many individuals are suing the government to obtain drugs.3 Although lawsuits secure access for thousands of people, this judicialisation of the right to health generates enormous administrative and fiscal burdens and has the potential to widen inequalities in healthcare delivery. Even though such lawsuits draw public attention,4,5 countrywide statistics remain unavailable. However, 6800 medical–judicial claims reached the Attorney Generalʼs Office of the State of Rio Grande do Sul in 2006, an increase from 1126 in 2002.6 By 2008, an average of 1200 new cases were reaching the Office per month.7 In 2008, US$30·2 million was spent by this state of 11 million people on court- attained drugs for about 19 000 patients.

INTRODUCTION The Brazilian version of race is quite different from the American version. In Brazil, blacks1 account for almost half of the population, while they are only a little more than one tenth of the United States population. Racial discrimination in Brazil is based on phenotype and not on ancestry. De jure discrimination (although not de facto discrimination) has been banned in Brazil practically since the end of slavery in 1888. However, even among these differences, we do find some similarities between the two social constructions of racism. The notion of healthcare networks proposes that the health system must be organized through coordinated points to provide continuous and integrated care, based on cooperation between managers, providers, and users. In Brazil in 2010, the Ministry of Health enacted an ordinance defining the healthcare network as a management model to be pursued by the national health policy in order to address most of the difficulties of fragmentation. This paper proposes to discuss the cases of Healthcare Networks in Brazilian metropolitan regions, characterizing its implementation process as well as identifying management problems in various cases. This study is grounded on data provided by agencies responsible for health management in the federal and state levels, as well as the current literature on healthcare networks. Results include the analysis of metropolitan networks implemented within their different stages, challenges and advances. Keywords: Brazil, Fragmentation, Health Care Networks, Health Management Introduction Health care systems are social responses deliberately organized to meet the demands and preferences of societies. In this sense, they must be linked to the health needs of these societies, which are expressed in unique demographic and epidemiological situations.
