Background • Given the lack of protective immunity, on-going risk behaviours can lead to hepatitis C virus (HCV) reinfection after successful treatment • Incidence of reinfection following treatment in a meta-analysis of 5 studies among people who inject drugs (PWID)1 • 2.4/100 PY among patients with a history of injecting drug use (IDU) • 6.4/100 PY among patients with on-going IDU after treatment • Risk of reinfection 5-years after SVR was 8% a in meta-analysis of 16 studies among PWID or prisoners2 • Tolerable DAAs will likely increase HCV treatment uptake among PWID and reinfection will probably emerge as an increasingly important topic 1. AASLD 2014 Aims of the study In a population of PWID who previously had achieved SVR following at least six months of abstinence from drug use prior to HCV treatment, we aimed to assess 1. Predictors of reinfection and relapse to IDU • All cases of reinfection occurred among those who had relapsed to IDU after treatment • Reinfection was not associated with any baseline variables and was not associated with post treatment injecting risk behaviours • Relapse to IDU was associated with • Low age at treatment: aOR 0.89 per year (95% CI 0.83-0.95) • Low education level: aOR 4.10 (95% CI 1.56-10.8) Conclusions and implications • The incidence of HCV reinfection after SVR among PWID was moderate, but lower than reported rates of primary infection • At the individual level, reinfection might compromise long-term benefits of treatment for patients with on-going risk behaviours • At the population level, treating patients at high risk of reinfection may have great prevention potential as these patients are being “kept out of the pool” for a period and prevented from transmitting the virus • Strategies to prevent reinfection should be addressed and evaluated in Foreword The publication of this updated day surgery atlas is an important event for several reasons. The term day surgery covers health services characterised by different issues and drivers. While ‘necessary care’ is characterised by consensus about indications and treatment and makes up about 15% of the health services, ‘preference-driven care’ is based more on the preferences of the treatment providers and/or patients. The preference-driven services account for about 25% of health services. The final and biggest group, which includes about 60% of all health services, is often called supply-driven and can be described as ‘supply creating its own demand’. Day surgery is a small part of the public health service in terms of resource use. However, it is a service that can be used to treat more and more conditions, and it is therefore becoming increasingly important for both clinical and resource reasons. How day surgery is prioritised and delivered is very important to patient treatment and to the legitimacy of the public health service. Information about how this health service is distributed in the population therefore serves as an important indicator of whether we are doing our job and whether the regional health authorities are fulfilling their responsibility to provide healthcare to their region’s population.

A bottom-up approach was used to collect complementary information on the routine use of telemedicine through a survey conducted at the five largest publicly funded hospitals. Results: Results show that routine telemedicine has been adopted in all health regions in Norway and in 68% of hospitals. Despite being widely adopted, the current level of use of telemedicine is low compared to the number of face-to-face visits. Examples of routine telemedicine can be found in several clinical specialties. Most services connect different hospitals in secondary care, and they are mostly delivered as teleconsultations via videoconference. Conclusions: Routine telemedicine in Norway has been widely adopted, probably for geographical reasons, as in other settings. However, the level of use of telemedicine in Norway is rather low, and it has significant potential for further development as an alternative to face-to-face outpatient visits.

Considerable efforts have gone into developing a comprehensive maternity care since the submission of Report No 12 to the Storting (2008–2009) - En gledelig begivenhet (‘A happy event’ - in Norwegian only) and the publication of the Norwegian Directorate of Health’s guide Et trygt fødetilbud (‘Safe maternity services’ - in Norwegian only). The guide and the subsequent regional discipline plans have provided a basis for developing even better described and more predictable maternity care. The quality requirements have been clarified and the requirements that apply to maternity units have been specified. Cooperation between specialist communities across institutional boundaries have helped to develop more uniform and medically documented practices. It is therefore pleasing, and perhaps also expected, that SKDE’s eighth healthcare atlas shows that overall, mothers and babies receive good and equitable health services throughout pregnancy and childbirth. This is primarily important to the women and the babies born, as it allows them to feel safe before the most important event in life.
