JOB DESCRIPTION

JOB TITLE: PRESCRIPTIONS CLERK

REPORTS TO: DEPUTY PRACTICE MANAGER

HOURS: 37.5 hours

Job Summary:

To process requests for repeat prescriptions and provide an efficient and safe system of reauthorisation of repeat medication, liaising with all relevant parties, including direct contact with patients and other health care professionals regarding medication queries.


Job Responsibilities:

·  Process repeat prescriptions on a daily basis ensuring compliance with the practice protocol.

·  Monitor the effective operation of the Electronic Prescribing Service.

·  Ensure all current medication have relevant, current authorisations.

·  Re-authorise repeat prescriptions where necessary in line with the practice protocol.

·  Promote cost-effective prescribing at the practice through switching drugs to the generic preparation when appropriate and encouraging the use of local primary care trust formulary medication choices.

·  Be the main point of contact for all prescription queries from a range of healthcare professionals including; GP’s, nurses, community pharmacists, nursing home staff as well as reception/administration staff at the practice.

·  Deal with medication changes from all hospital letters, removing old medication, adding new or making dose changes. Letter to patient, including new RHS and confirmation of changes. Add any relevant monitoring diaries.

·  Collate and submit claims to the Prescription Pricing Authority for personally administered items

·  Ensure that expiry dates on repeat prescriptions are harmonised for each patient

·  Working closely with the practice support pharmacist, read and act upon any relevant information as published in the prescribing support newsletter Tablet Press.

·  In liaison with the Office Manager, perform searches and act upon drug alerts.

·  To monitor specific drugs for overuse and report findings to the relevant GP

·  Monitor the issuing of weekly prescriptions to ensure compliance with CCG guidance

·  Add new patient medication. Pass to registered GP for approval highlighting any concerns found, add relevant diaries. Liaise with patient via phone call or letter.

·  Communicate with new doctors in medication review protocols and practice formulary choices.

·  Update existing doctors in new protocol developments

·  Liaise closely with local pharmacies.
