Terms and Conditions 1. Software can only be sold to WVU Faculty and Staff, and can be paid with a Cost Transfer, IU, Expenditure Reduction form, or University credit card. Complete this Software/Order Distribution form; attach this form to the appropriate financial form. Mail all to: SLIC Program, WVU Technology Support Center, PO Box 6501, Morgantown, WV 26506 2. For most software, annual licenses are distributed and these licenses must be renewed each year. In most cases the renewal license fee is less than the initial license fee. 3. For network installations, a license must be purchased for the number of concurrent users and be metered to prevent usage of more licenses than legally purchased. Departments must verify how the software is metered. 4. Licensee agrees not to create, or attempt to create, or permit or help others to create the source code from the software furnished pursuant to this agreement. Licensee agrees that it will not reverse engineer or decompile the software. 5. Questions regarding this procedure can be directed to the Help Desk at 293-4444. 6. I agree to comply with the terms and conditions of this license agreement. 7. After installation of the above software, you must send SLIC a list of PC’s where the SW was loaded, see attached form.
