Quality Standard Certification: Please indicate if your organisation has acquired a certified quality standard. In detail the following should be briefly described: relevant steps taken to the final certification to an approved quality Management system/standard such as, but not limited to, ISO 9001, ISO 14001, ISO 18001, ISO 22001, ISO 27001, CoBiT, ISM code, TMSA, IIP, or other standard relevant to your organisation’s specialisation. This certified management standard, should furthermore, incorporate at least one documented procedure which will demonstrate sufficient aspects of environmental and safety management

PART 3 Confirmation and Agreement
On behalf of my organisation, I confirm to observe the rules and conditions of the Lebanese Excellence Award and accept as final, the decisions of the Award process. I accept that the LEA Office/QUALEB is entitled to reject applications that do not comply with the requirements of the Award process. PART 4 Fees and Invoicing
6/8 LEA Application Form 1. The process begins when the Application Form is electronically sent to the LEA Office to [email protected] 2. The organisations are then welcome to retrieve a package containing the following: The Lebanese Quality Management Model The Scoring Matrices The Submission Document (Blank) The Self-Assessment Proforma Document. [For the time being, applying to the LEA is free of charge. The price of the package and a modest Application Fee may be determined and communicated at a later stage] 3. The organisations have then to send the Submission Document electronically to the LEA Office at [email protected] 4. A selection of applicant organisations is then executed by the LEA Office. This task consists of going through the Submission Document and making sure that all relevant contents and information are clearly stated.
A fee may be also disbursed only by the organisations that will be granted the ‘Lebanese Excellence Award Winner’ status, right before the ceremony.
Application fees are not refundable once the submission document has been received or an induction workshop for applicants has been held.
