Community Dietetic Service
Community Dietetic Services provide group education plus, if indicated, one-to-one consultation to patients referred by GPs, PNs, PHNs or Area Medical Officers. Patients can self refer for structured education sessions.

Diabetic Retinascreen
The National Diabetic Retinal Screening Programme
This programme is a government funded programme that offers free, regular diabetic retinopathy screening to people with diabetes aged 12 years and older / Referral process
People with diabetes can be referred to the programme via Freephone 1800454555 or online at www.diabeticretinascreen.ie by a Health Care Professional or by patient self referral by downloading a referral form accessed via www.diabeticretinascreen.ie and signed by the patient general practitioner
Once the person with diabetes is registered with the retinascreen programme. An invitation letter will be sent to consent to the retinascreen programme.
When this letter is received the person with diabetes must call the freephone number 1800454555 to consent to the programme. An appointment will then be sent for diabetic retinopathy screening.
Results of the screening will be forward to the GP
Public Health Nursing
The Community Nursing Service offers a range of advice and support to all people with diabetes holding a Medical Card. This support and advice and education is in conjunction with acute diabetic services to:
·  Newly and established people with diabetes of all ages who hold a medical card in the home or primary school setting.
·  Women in the ante natal and post natal period who suffer from chronic diabetes or gestational diabetes.
·  Management of diabetic leg ulcers.
·  Onward referral to Podiatry or G.P.
Every effort is made by hospital and PHN staff to train newly diagnosed people with diabetes to administer their own Insulin in order to make them independent. Where the person with diabetes cannot administer her/his insulin, the Public Health Nursing Service will provide this service.

Counselling in Primary Care service (CIPC) available to people aged 18years and over
Discuss with the patient & agreement confirmed
Give the patient a information leaflet on CIPC counselling in primary care and instruct the patient on importance of contacting the service to “opt in” this is essential for counselling to begin details available on the information leaflet

SUPPORT SERVICES
Long Term Illness Scheme
All people diagnosed with diabetes are eligible for for a Long Term Illness Card (LTI) regardless if teh person has a GMS card, this will entitle the person with diabetes to free diabetes-related medication, devices and blood glucose monitoring strips. LTI application are available from the GP practice or Pharmacy

The application from is used for both the medical card and GP visit card
Your eligibility will be checked for both.
Medical card patients can also apply to the Long Term Illness scheme to avail of free prescription charge

Parents of diabetic children (or any other condition) are entitled to apply for Domiciliary Care Allowance.
In order to be considered medically eligible the following must be satisfied:

Diabetes Ireland
Diabetes Ireland has a network of support branches and groups throughout the country. The activities of the organisation include among others, dissemination of non-judgmental advice and information through meetings, workshops, website, media and a quarterly magazine. Support is available through literature lo-call helpline, website and public meetings. Awareness of diabetes is raised through PR campaigns and lobbying on behalf of people with diabetes in areas where they are being discriminated. / Western Regional Office
