HEALTHEDUCATOR AWARD
AWARD QUALIFICATIONS & NOMINATION FORM

The TAPHE Regional Health Educator Award hounours an individualfor their work to further the importance of health education in Nova Scotia and/or who had served as a champion in promoting the importance of Health Promoting Schools at the regional, provincial, or federal level.

Number of Awards:

One recipient will be selected from each Nova Scotia School Board (the TAPHE Awards committee reserves the rights to award multiple recipients if the nominations are equal in achievements/quality).

Cape Breton-Victoria Regional School Board
Chignecto-Central Regional School Board
Halifax Regional School Board
South Shore Regional School Board / Strait Regional School Board
Tri-County Regional School Board
Consiel Scolaire Acadien Provincial
MK First Nations School Board

Eligibility Requirements:

Directly involved in school-based health education (teacher, administration, researcher, physician or community health professional)

Have made a positive contribution to improving the field of health education through research, curriculum development and/or delivery, program development and/or delivery within a school setting.

Selection Process:

Completed nomination packages will be reviewed by the TAPHE Awards committee. All components of the award must be completed and received by the deadline. Incomplete or late applications may not be considered (these nominations could be sent back for completion and/or held for the next calendar year).
