Genetic testing after egg retrieval during an IVF cycle can provide a wealth of information about each embryo before its transferred back into the uterus during Embryo Transfer. The main genetic tests are:
• Pre implantation genetic diagnosis (PGD) • Pre implantation genetic screening (PGS) • Family Balancing through Gender Selection • HLA Matching
It is easy to confuse pre implantation genetic diagnosis (PGD) and pre implantation genetic screening (PGS). However, despite the similarity of name and their common aim to identify a genetic disorder in an embryo, the two techniques are quite different and are used in quite different circumstances.
PGS vs. PGD
PGS is used to screen the embryos of couples undergoing IVF for genetic abnormalities so that only normal embryos are transferred. The aim is to improve pregnancy rates and make embryo transfer more viable. PGS can also be used to check whether the embryos are male and female, and select the embryo gender of choice for ‘family balancing’
PGD, on the other hand, is used when a couple is deemed to be at risk of passing on a genetic disease to their baby. PGD can identify those embryos (if any) unaffected by the altered gene and so ensure that only unaffected embryos are transferred. In this way, PGD removes the risk of inherited transmission of genetic disease. Pre Implantation Genetic Screening (PGS)
The PGS technique is useful in women where chromosomal problems are likely to be higher – in women above the age of 35 and in those with a history of recurrent treatment failure and recurrent early miscarriages. In theory, the transfer of a healthy embryo is more likely to lead to a healthy pregnancy than the transfer of a chromosomally abnormal embryo. Family Balancing
Family Balancing allows for parents to determine the sex of their baby. The gender of an embryo can be determined using Pre implantation Genetic Screening (PGS) although this is usually done to check for genetic abnormalities.
Choosing to do family balancing, along with your IVF-ICSI doesn’t affect the duration of your treatment. After your egg collection, if the eggs are successfully fertilized, a biopsy is taken from each embryo and tested through PGS. The healthy embryos of the desired gender are then transferred back to the uterus.
Pre Implantation Genetic Diagnosis (PGD)
PGD assesses embryos for a specific genetic disease, e.g. Thalassemia, allowing parents with a known hereditary disease to ensure their offspring are free of that genetic condition. During PGD, f each embryo is screened for the genetic variation(s) that the parents carry.
Who is suitable for PGD?
PGD is only appropriate if you and/or your partner are affected with, or carry, an identifiable single gene disorder or a chromosomal rearrangement, known as a translocation.
PGD may be recommended if:
• You have had one or more terminations because your baby had a genetic condition • You already have a child with a serious genetic condition • You have a family history of a serious genetic condition like cystic fibrosis. Men’s Health

Preserving men’s fertility: Sperm Freezing Sperm freezing, or sperm banking, is a method of preserving sperm cells. It is ideal for men who want to delay parenthood until a later stage in life or for men who plan to undergo certain types of medical treatments that could impact their sperm quality. Sperm may also be frozen as a backup during their fertility treatment.
Sperm is collected through ejaculation or surgically assisted retrieval procedures such as TESA, PESA or MicroTESE. It is then analysed and the sperm is frozen. The frozen sperm sample is stored in the laboratory for later use in an Intrauterine Insemination (IUI) or In-Vitro Fertilisation (IVF) cycle.
A back-up plan for future pregnancy
Preserving women’s fertility: Egg Freezing Egg freezing, or oocyte cryopreservation, is one way of “stopping” the biological clock, expanding reproductive options and preserving the younger, possibly healthier eggs for use at a later stage. A woman's eggs are harvested from her ovaries, frozen unfertilised and stored. A frozen egg can be thawed, fertilised with sperm in a lab and implanted in the uterus (in vitro fertilisation). Egg freezing was originally used for women who undergo treatment, such as chemotherapy or radiation, and has today become an increasingly attractive option for women who want to preserve their eggs for use when they are better situated to have a family. A woman's age is the most important factor affecting her fertility. A woman is born with a fixed supply of eggs and as she gets older, this supply gradually reduces, as does the quality of the eggs. The likelihood of genetic abnormalities increases, and with it, the risk of miscarriage, birth defects or disorders that makes conceiving more difficult.
A woman is most fertile from about age 18 to 29. Over 30, her fertility progressively declines, and this decline accelerates in her late 30s and early 40s.

Note: As women get older, they have fewer eggs and therefore freezing becomes less beneficial as only a small number of eggs may be obtained in the cycle. For women aged over 40 or those with reduced egg supply, it is worthwhile considering 2-3 cycles of Egg Freezing in order to maximize the chance of a successful pregnancy later on.

أﻣﺎ اﻟﺘﺸﺨﻴﺺ اﻟﺠﻴﻨﻲ ﳌﺮﺣﻠﺔ ﻣﺎ ﻗﺒﻞ زراﻋﺔ اﻟﺒﻮﻳﻀﺔ اﳌﻠﻘﺤﺔ(PGD)، ﻓﻴُﺴﺘﺨﺪم ﻋﲆ اﻟﺠﺎﻧﺐ اﻵﺧﺮ ﻋﻨﺪ وﺟﻮد اﺣﺘ®ﻟﻴﺔ اﻧﺘﻘﺎل أﻣﺮاض وراﺛﻴﺔ ﻣﻦ اﻟﺰوﺟﺎن إﱃ ﻃﻔﻠﻬﻢ، ﻛ® Îﻜﻦ ﻣﻦ ﺧﻼل اﻟﺘﺸﺨﻴﺺ اﻟﺠﻴﻨﻲ ﳌﺮﺣﻠﺔ ﻣﺎ ﻗﺒﻞ ﻧﻘﻞ اﻷﺟﻨﺔ (PGD) ﺗﺤﺪﻳﺪ اﻷﺟﻨﺔ اﻟﺘﻲ Å ﺗﺘﺄﺛﺮ ﺑﺎﻟﺠ اﳌﺘﺤﻮل (إذا وﺟﺪت)، وﻣﻦ ﻫﻨﺎ Îﻜﻦ ﺿ®ن ﻋﺪم ﻧﻘﻞ ﺳﻮى اﻷﺟﻨﺔ اﻟﺴﻠﻴﻤﺔ ﻓﻘﻂ. وﺑﻬﺬه اﻟﻄﺮﻳﻘﺔ، ﻳﻌﻤﻞ اﻟﺘﺸﺨﻴﺺ اﻟﺠﻴﻨﻲ ﳌﺮﺣﻠﺔ ﻣﺎ ﻗﺒﻞ ﻧﻘﻞ اﻷﺟﻨﺔ (PGD) ﻋﲆ اﻟﻮﻗﺎﻳﺔ ﻣﻦ اﺣﺘ®ﻟﻴﺔ اﻧﺘﻘﺎل اﻷﻣﺮاض اﻟﻮراﺛﻴﺔ. اﻟﻔﺤﺺ اﻟﺠﻴﻨﻲ ﳌﺮﺣﻠﺔ ﻣﺎ ﻗﺒﻞ ﻣﺎ ﻗﺒﻞ ﻧﻘﻞ اﻷﺟﻨﺔ (PGS)
ﻳُﺠﺪي اﻟﻔﺤﺺ اﻟﺠﻴﻨﻲ ﳌﺮﺣﻠﺔ ﻣﺎ ﻗﺒﻞ ﻣﺎ ﻗﺒﻞ ﻧﻘﻞ اﻷﺟﻨﺔ (PGS) ﰲ اﻟﺤﺎﻻت اﻟﺘﻲ ﺗﺮﺗﻔﻊ ﻓﻴﻬﺎ اﺣﺘ®ﻟﻴﺔ ﺗﻌﺮض اﻟﺴﻴﺪات ﳌﺸﻜﻼت ﻛﺮوﻣﻮﺳﻮﻣﻴﺔ، وﺗﺼﺎب ﺑﻬﺬه اﳌﺸﻜﻼت اﻟﺴﻴﺪات اﻟﻼô ﺗﺠﺎوزت أﻋ®رﻫﻦ ٣٥ ﻋﺎﻣﺎً واﻟﺴﻴﺪات اﻟﻼÅ ô ﺗﻨﺠﺢ ﻣﻌﻬﻦ ﺳﺒﻞ اﻟﻌﻼج ﻟﻌﺪة ﻣﺮات ﰲ اﳌﺎﴈ واﻟﺴﻴﺪات اﻟﻼô ﺗﻌﺮﺿﻦ ﻟﻌﺪة ﺣﺎﻻت إﺟﻬﺎض ﻣﺒﻜﺮ. ﻓﻤﻦ اﻟﻨﺎﺣﻴﺔ اﻟﻨﻈﺮﻳﺔ، ﻳﺆدي ﻧﻘﻞ اﻟﺠﻨ اﻟﺴﻠﻴﻢ ﻋﲆ اﻷرﺟﺢ إﱃ اﻟﺘﻤﺘﻊ ﺑﺤﻤﻞ ﺻﺤﻲ ﻣﻘﺎرﻧﺔ ﺑﻨﻘﻞ اﻷﺟﻨﺔ اﻟﺘﻲ ﺗﻌﺎ ﻣﻦ ﻣﺸﻜﻼت ﻛﺮوﻣﻮﺳﻮﻣﻴﺔ. ﻳﺘﻴﺢ إﺟﺮاء اﻟﺘﻮازن اﻷﴎي ﻟﻠﻮاﻟﺪﻳﻦ إﻣﻜﺎﻧﻴﺔ ﺗﺤﺪﻳﺪ ﺟﻨﺲ ﻃﻔﻠﻬ®. وÎﻜﻦ ﺗﺤﺪﻳﺪ ﻧﻮع اﻟﺠﻨ ﺑﺎﺳﺘﺨﺪام اﻟﻔﺤﺺ اﻟﺠﻴﻨﻲ ﳌﺮﺣﻠﺔ ﻣﺎ ﻗﺒﻞ ﻣﺎ ﻗﺒﻞ ﻧﻘﻞ اﻷﺟﻨﺔ (PGS) ﻣﻊ أﻧﻪ ﻳﺴﺘﺨﺪم ﻋﺎدة ﻟﻠﻜﺸﻒ ﻋﻦ اﻟﺘﺸﻮﻫﺎت اﻟﻮراﺛﻴﺔ. إن اﺧﺘﻴﺎر إﺟﺮاء اﻟﺘﻮازن اﻷﴎي، إﱃ ﺟﺎﻧﺐ ﻋﻤﻠﻴﺔ اﻟﺤﻘﻦ اﳌﺠﻬﺮي/ أﻃﻔﺎل اﻷﻧﺎﺑﻴﺐ، ﻻ ﺗﺆﺛﺮ ﻣﻄﻠﻘﺎً ﻋﲆ ﻣﺪة اﻟﻌﻼج. ﻓﺒﻌﺪ ﺟﻤﻊ اﻟﺒﻮﻳﻀﺎت، وﰲ ﺣﺎل ﺗﺨﺼﻴﺐ ﺗﻠﻚ اﻟﺒﻮﻳﻀﺎت ﺑﻨﺠﺎح، ﺗﺆﺧﺬ ﻋﻴﻨﺔ ﻣﻦ ﻛﻞ ﺟﻨ ﻻﺧﺘﺒﺎرﻫﺎ ﻣﻦ ﺧﻼل اﻟﻔﺤﺺ اﻟﺠﻴﻨﻲ ﳌﺮﺣﻠﺔ ﻣﺎ ﻗﺒﻞ ﻣﺎ ﻗﺒﻞ ﻧﻘﻞ اﻷﺟﻨﺔ (PGS)، ﺛﻢ ﻳﺠﺮي إﻋﺎدة اﻷﺟﻨﺔ اﻟﺴﻠﻴﻤﺔ ﻟﻠﺠﻨﺲ اﳌﺨﺘﺎر إﱃ اﻟﺮﺣﻢ.
اﻟﺘﺸﺨﻴﺺ اﻟﺠﻴﻨﻲ ﳌﺮﺣﻠﺔ ﻣﺎ ﻗﺒﻞ ﻧﻘﻞ اﻷﺟﻨﺔ (PGD) ﻳﻌﻤﻞ اﻟﺘﺸﺨﻴﺺ اﻟﺠﻴﻨﻲ ﳌﺮﺣﻠﺔ ﻣﺎ ﻗﺒﻞ ﻧﻘﻞ اﻷﺟﻨﺔ (PGD) ﻋﲆ ﺗﻘﻴﻴﻢ اﻷﺟﻨﺔ ﻟﻠﻜﺸﻒ ﻋ® إذا ﻛﺎﻧﺖ ﺗﻌﺎ ﻣﻦ أﻣﺮاض وراﺛﻴﺔ، ﻣﺜﻞ اﻟﺜﻼﺳﻴﻤﻴﺎ، ﻣ® ﻳﺘﻴﺢ ﻟﻠﻮاﻟﺪﻳﻦ اﻟﻠﺬﻳﻦ ﻳﻌﺎﻧﻴﺎن ﻣﻦ ﻣﺮض وراø ﻣﺤﺪد ﺿ®ن ﻋﺪم اﻧﺘﻘﺎل ﻫﺬا اﳌﺮض اﻟﻮراø إﱃ ذرﻳﺘﻬ®. أﺛﻨﺎء اﻟﺘﺸﺨﻴﺺ اﻟﺠﻴﻨﻲ ﳌﺮﺣﻠﺔ ﻣﺎ ﻗﺒﻞ ﻧﻘﻞ اﻷﺟﻨﺔ (PGD)، ﻳﺠﺮي ﻓﺤﺺ ﻛﻞ ﺟﻨ ﻟﻠﻜﺸﻒ ﻋﻦ اﻟﺘﻐ ات اﻟﻮراﺛﻴﺔ اﻟﺘﻲ ﻳﺤﻤﻠﻬﺎ ﻛﻞ ﻣﻦ اﻟﻮاﻟﺪﻳﻦ. ﻣﻦ اﳌﺮﺷﺢ اﳌﻨﺎﺳﺐ ﻟﻠﺨﻀﻮع إﱃ اﻟﺘﺸﺨﻴﺺ اﻟﺠﻴﻨﻲ ﳌﺮﺣﻠﺔ ﻣﺎ ﻗﺒﻞ ﻧﻘﻞ اﻷﺟﻨﺔ (PGD)؟ اﻟﺘﺸﺨﻴﺺ اﻟﺠﻴﻨﻲ ﳌﺮﺣﻠﺔ ﻣﺎ ﻗﺒﻞ ﻧﻘﻞ اﻷﺟﻨﺔ (PGD) ﻣﻨﺎﺳﺐ ِﻟﻚ ِوﻟﺰوﺟﻚ إذا ﻛﻨﺘ® ﻣﺼﺎﺑ ﺑﺎﺿﻄﺮاﺑﺎت وراﺛﻴﺔ أﺣﺎدﻳﺔ اﻟﺠ أو ﺑﺈﻋﺎدة ﺗﺮﺗﻴﺐ اﻟﻜﺮوﻣﻮﺳﻮﻣﺎت، وﻫﻮ ﻣﺎ ﻳﻌﺮف ﺑﺎﻻزﻓﺎء.
ﻳﻮﴅ ﺑﺈﺟﺮاء اﻟﺘﺸﺨﻴﺺ اﻟﺠﻴﻨﻲ ﳌﺮﺣﻠﺔ ﻣﺎ ﻗﺒﻞ ﻧﻘﻞ اﻷﺟﻨﺔ (PGD) ﰲ اﻟﺤﺎﻻت اﻟﺘﺎﻟﻴﺔ: • اﻟﺴﻴﺪة اﻟﺘﻲ ﺗﻌﺮﺿﺖ إﱃ اﻹﺟﻬﺎض ﳌﺮة أو أﻛð ﺑﺴﺒﺐ ﻣﻌﺎﻧﺎة ﻃﻔﻠﻬﺎ ﻣﻦ ﻣﺮض وراø. • اﻟﺴﻴﺪة اﻟﺘﻲ ﻟﺪﻳﻬﺎ ﺑﺎﻟﻔﻌﻞ ﻃﻔﻞ ﻳﻌﺎ ﻣﻦ ﻣﺮض وراø ﺧﻄ • اﻟﺴﻴﺪة اﻟﺘﻲ ﺗﻌﺎ ﻋﺎﺋﻠﺘﻬﺎ ﻣﻦ ﺗﺎرﻳﺦ ﻣﺮﴈ ﳌﺮض وراø ﺧﻄ ﻛﺎﻟﺘﻠﻴﻒ اﻟﻜﻴﴘ.. اﻟﻌﻘﻢ ﻋﻨﺪ ﺻﺤﺔ اﻟﺮﺟﺎل إن اﻟﺼﺤﺔ اﻹﻧﺠﺎﺑﻴﺔ ﻋﻨﴫاً ﻣﻬ®ً ﰲ ﺻﺤﺔ اﻟﺮﺟﺎل ﻋﻤﻮﻣﺎً. وﺗُﻌﺪ اﻟﺼﺤﺔ اﻹﻧﺠﺎﺑﻴﺔ ﻟﺪى اﻟﺮﺟﺎل ﺷﺎﺋﻌﺔ ﻟﻠﻐﺎﻳﺔ وÎﻜﻦ أن ﺗﺆﺛﺮ ﰲ ﺟﻮدة ﺣﻴﺎة اﻟﺮﺟﻞ وﻋﻼﻗﺎﺗﻪ.
