Following this the nurse will have a discussion with a doctor; whilst this is happening you will receive some basic physical health checks such as your blood pressure, pulse and temperature. We can also check your weight and BMI and take blood samples if they are indicated.

Both the nurse and the doctor will meet with you to discuss and agree options available to meet your current needs, either with our team or with another service.

Anything you say will be confidential within the team unless there is a legal duty to share information with other professionals, your family or carer. This will be explained before the assessment. If you have any concerns about confidentiality please talk to a member of staff.

After your assessment, a letter will be sent to your family doctor and the referrer if these are different. You will be asked if you would like to receive a copy of the letter.
