Improving the quality of life and social-emotional well-being of children birth to 21 and families in the community.

Dakota County Connections believes that strengthening our community requires all of us to work together. This includes: • Identifying community strengths and needs • Actively coordinating and sharing lessons learned • Defining common outcomes • Aligning individual community needs and resources to meet those outcomes • Seeking additional resources to help reach our goals • Using evaluation and community data to strengthen our programs • Maintaining open communication with our partners and the community

Dakota County Connections does not provide programs, it serves to provide a common table for community partners to gather around and collectively address community needs. We support organizations and individuals to provide programs that are directed toward our common outcomes.

Boys Town Nebraska/Iowa 600 4th St, Suite 901, Sioux City, IA 51101 (712) 258-6523 boystown.org Boys Town is a national organization dedicated to helping children and their families. Boys Town developed its Integrated Continuum of Care as part of an ambitious, on-going effort to expand the life- changing care we provide to children and families across the . The Continuum is unique to Boys Town and enables us to deliver the right care at the right time to troubled children and families who are edging toward crisis. Across the Continuum’s service levels and in all programs, treatment is consistently effective because it’s based on the research-proven Boys Town ModelⓇ. As children and families make progress (or fail to make progress) at one level, they can move to other levels that better fit their needs, without starting over in their care and all within the same care model. Services toward the top of the Continuum provide more intensive care and interventions to children with serious emotional or behavioral problems. Children receive these services outside of their home, so reunification with their families, whenever possible, is always the ultimate goal. Services toward the base of the Continuum focus on helping families stay together, preventing children from being removed from their homes, and assisting parents by providing training, counseling, and other resources. That’s why Boys Town is developing its Integrated Continuum of Care at its sites across the country to help reform the nation’s fragmented and overburdened child and family care system. Existing services and programs at each Boys Town location provide the foundation for building and spreading the Continuum.

We take a systemic approach to complex issues – such as child welfare, immigration policy, affordable healthcare and poverty – and we take our work wherever we believe we can do the most good, whether that’s at the courthouse, in the statehouse or in the community. At Appleseed, we believe that every Nebraskan deserves a shot at the American Dream. Our job is to help clear the way; To work with the community to tear down those obstacles – discrimination, oppression and reckless indifference – that keep the poorest of us poor and further weaken those among us who are the most vulnerable. At Appleseed, we fight for justice and opportunity for all. And we take that fight wherever we think we can do the most good – whether that’s at the courthouse, in the statehouse or on the streets of our communities. We also recognize that fighting the good fight often means not fighting at all – but finding consensus and allies who support our cause. The best way to accomplish real change in Nebraska is by working together. The best way to build a stronger community is as a community. These goals are daunting, we know. The challenges are many. And there’s nothing easy or simple about our mission. But we don’t give up. We never give up. Because when you believe in justice – when you believe in your heart in the American Dream – you can’t walk away from the fight.
