Using the Telephone & Television In an effort to make you more comfortable, Saint Agnes provides cable TV and telephone service in your room ..There is a combined charge for these services of $8 per day and a one-time activation fee of $4 .50 ..These charges are billed to your home telephone ..The maximum charge for one admission is $60 .50 (seven days plus the activation fee .) 5 Patient Identification Your safety is important to us! Saint Agnes Hospital staff will frequently ask you to repeat your name and birth date ..They will compare what you tell them to your wristband or to hospital documents to make sure the information matches .. Make sure your nurse or other care provider confirms your identity by checking your wristband and asking your name and birth date before he or she gives you any medicine, draws blood, or gives you a treatment .. Personal Items & Valuables Dentures, hearing aids, and eyeglasses should be placed in a protective case from home when not in use ..Never wrap them in tissue or place them on your meal trays or in your pillow case as they may be inadvertently discarded or damaged .. Valuables such as jewelry, radios, CD players, cell phones, or other personal electronics should not be stored in your room ..Please send all valuables home with a family member ..If this is not possible please ask your nurse to contact our Protective Services Department to have them secure your valuables in the Hospital safe ..NO electronic devices are permitted without safety clearance from our bio-med department .. Saint Agnes Hospital assumes no responsibility for the loss and/or damage to personal belongings including dentures, glasses, hearing aids, or other prosthetic devices . Fall Prevention At Saint Agnes Hospital, our patients’ safety is our main concern ..Hospital falls can cause delays in your recovery ..Falls can be caused by: 1 ..Medications we give you for sleep, pain, your blood pressure, fluid, etc . 2 ..Equipment attached to your such as an IV pole . 3 ..Muscle weakness, due to your illness, surgery, or bedrest . Here are some reminders to help you stay safe from falls: • Always use your call bell to ask for help ..Please be patient ..Remain lying or sitting while waiting; staff will be there as soon as possible ..Do not walk or stand without help . • Do not use objects such as IV poles or bedside tables for support while walking or standing ... 6 • Please do not try to get on or off the toilet by yourself ..Wait for staff or a capable family member to help you . • Family is encouraged to stay with you ..Family members, please alert the nurse when leaving the patient alone . • Remember to keep your call bell and other personal items (phone, tissues, glasses, bedside table, etc ). within reach while lying or sitting in bed or chair . • Help us keep your bedside area free of clutter ..Keep items off the floor so there is a clear path to the bathroom and to the door of the room .. • Keep the bedrails at the head of the bed up at all times . • Please review the Fall Prevention Program brochure for more information about how to prevent falls in the hospital . It is important that everyone, including patients, staff, and family and friends, help prevent hospital falls .. YOUR CARE Advance Directives Advance directives are written instructions on your health record, which are used if you become unable to communicate your wishes ..These instructions can include the name of someone who can make health care decisions for you if you are unable to do so ..If you have an advance directive, please provide it to your doctor ..If you do not have an advance directive prepared, you can prepare one here and have it added to your medical record . Medications It is extremely important that you provide your doctor with a complete list of the medications you are taking, including over-the-counter medications, herbal supplements, and vitamins ..Ask your doctor before taking any of these medications during your stay .. * Reminder: Do not keep any medications at your bedside; please send them home. Your Physician & Your Treatment Each patient is in the care of a specific physician ..If you did not have a physician when you were admitted to the hospital, you were assigned one here ... 7 Your physician is responsible for your care, including ordering tests and providing test results, ordering or changing your medications or treatments, and discharging you from the hospital . Infection Prevention & Control The most important step in preventing the spread of infection is proper hand cleaning by all staff, visitors, and patients ..Please feel free to ask our staff if they have washed their hands . Wash Your Hands: The Right Way When washing hands with soap and water: • Wet your hands with clean running water and apply soap ..Use warm water if it is available .. • Rub hands together to make a lather and scrub all surfaces .. • Continue rubbing hands for 15-20 seconds ..Need a timer? Imagine singing “Happy Birthday” to a friend .. • Rinse hands well under running water .. • Dry your hands using a paper towel or air dryer ..If possible, use your paper towel to turn off the faucet .. • Always use soap and water if your hands are visibly dirty .. Alcohol-based hand rubs significantly reduce the number of germs on skin and are fast-acting . When using an alcohol-based hand sanitizer: • Apply product to the palm of one hand .. • Rub hands together .. • Rub the product over all surfaces of hands and fingers until hands are dry .. To do this, we sometimes need to use special work practices called “isolation precautions .” Some patients may have a higher risk of getting infections while they are in the hospital, especially if they are very sick . Isolation precautions include: • Health care workers must wear protective clothing such as gloves, gowns, and sometimes masks when performing your care .

8 • Special equipment that stays in the patient’s room . • Limited movement in and out of the room . For Visitors • If you are sick or not feeling well, please do not visit the patient until you are well . • Before entering an isolation room, please check in at the nursing station . • Before entering an isolation room, please read the sign posted outside the isolation room ..The sign shows what protective clothing is needed to enter the room . • Clean your hands with soap and water OR use waterless hand sanitizer before entering and when leaving a patient’s room . • When leaving the patient’s room, remove protective clothing – gowns, gloves, and masks ..Please dispose of these in the trash just inside the patient room door . • Visitors may be limited to two at a time . • Take as few things as possible into the isolation room . Infections can be spread by contact with clothing, hands, personal items, surfaces (such as furniture and countertops), or health care equipment .. If you have questions please ask a Saint Agnes Hospital staff person or physician .. Residents Saint Agnes is a community-teaching hospital ..We have residency programs in medicine and surgery, and we participate with Johns Hopkins University in a pediatrics residency program ..Residents are physicians who have earned their undergraduate and medical degrees, and are continuing their training in a chosen specialty ..A resident will, in most cases, assist your attending physician in your care .. SPEAK UP At Saint Agnes Hospital we want you and your family to be an active partner in your health care ..One way that you can be involved is to speak up .. Speak up if you have questions or concerns, and if you don’t understand, ask again ..It’s your body and you have a right to know .. Pay attention to the care you are receiving ..Make sure you’re getting the right treatments and medications by the right health care professionals ..Don’t assume anything ..

Educate yourself about your diagnosis, the medical tests you are undergoing, and your treatment plan .. Ask a trusted family member or friend to be your advocate .. Know what medications you take and why you take them ..Medication errors are the most common health care mistakes .. Use a hospital, clinic, surgery center, or other type of health care organization that has undergone a rigorous on-site evaluation against established, state-of-the-art quality and safety standards, such as that provided by The Joint Commission .. Participate in all decisions about your treatment ..You are the center of the . health care team .. As a patient at Saint Agnes Hospital, you have rights and responsibilities that protect you and allow us to deliver the best care during your stay .. Spiritual Care The Spiritual Care Department at Saint Agnes Hospital provides for the spiritual well being of patients, families, and staff members ..We help all people experience the presence and companionship of God, whomever they perceive their God to be, in times of stress and crisis ..Priest, Chaplains and Spiritual Care volunteers make regular rounds throughout the hospital ..Specific referrals can be made to Spiritual Care though your health care team or by calling x3197 .. The hospital chapel is located on the first floor, near the North Tower visitor elevators ..The Mass schedule is posted outside the door to the chapel . Masses in the chapel are broadcast on Channel 12 ..Additional Masses and liturgical services can be viewed on EWTN channel 31 .

10 FOR PATIENTS & VISITORS General Visiting Hours 8:30 a.m. to 8:30 p.m. All visitors must check in at the front desk and get a visitor pass ..Visitors to the third and fifth floors must provide a photo ID in order to obtain a pass ..In areas where children under the age of 12 are permitted, adult supervision is required . Visitors should be limited to two per patient ..In order to protect patients, visitors and staff, all visitors of patients in isolation will be required to wear the appropriate protective equipment while visiting . Maternity patients’ visitors can access the hospital’s front entrance 24/7 . Medical & Surgical ICU/Coronary Care Unit No Visitation between 7:00 a.m. to 9:00 a.m. and 7:00 p.m. to 9:00 p.m. • Only two visitors per patient • Children under 12 years of age are discouraged • No food or drink permitted • Cell phones should be turned off • No photography permitted Pediatrics, Pediatric & Neonatal ICU • 24/7 as condition allows . • Parents & grandparents only . MEDICAL EMERGENCY TEAMS A MET Team (Medical Emergency Team) much like 911 is used here in the hospital ...The team enables us to respond urgently to changes in a patient’s condition, often before a severe medical crisis occurs ...The team is made up of highly trained medical professionals – an ICE nurse, physician, and respiratory therapist ...The team works with providers already at the bedside to evaluate the patient quickly, address concerns and intervene immediately when needed .

Can Patients/families call the METS team directly?

Yes, you can call the METS team by dialing extension 5555 from your room phone ...Say “METS Team needed,” and give the operator your name or the patient’s name and room number ...This provides a safety net in addition to the primary staff caring for you and can be used for you or your loved ones if there is a sudden severe change in condition ..

When should I call the METS Team?. If the patient is having: . • Difficulty breathing . • Seizures . • Stroke symptoms- facial droop, arm or leg weakness, speech.. . . difficulty, or change in alertness . • Chest Pain 12 Meet Your Medical Team Attending Physician - The doctor who is directly responsible for your care . Consulting Physicians - Physicians who have been asked to see you by your attending physician . Resident - A resident is a physician who has received their medical degree and who are continuing their specialty training ..A resident may assist your attending physician . Nurses - Nurses will be your primary contact and will provide care for you 24 hours a day ..Your care is coordinated by a registered nurse ..Your registered nurse will oversee a team including certified nursing assistants, technicians, clerks and other patient care associates ..Any questions or concerns regarding your care should be directed to a nurse or nurse manager . Spititual Care - The spiritual care associates at Saint Agnes Hospital understand the need for patients and families to not only be cared for physically, but sometimes spiritually as well ..Their services are available 24 hours a day .. Case Managers - Case Managers are Registered Nurses who specialize in working with some patients to insure that their care is coordinated before, during and after their hospital stay . Social Workers - A wide variety of services can be provided by a Social Worker including discharge planning, referrals to outside agencies and more . Dietitians - Dietitians can help to insure that your nutritional needs are met . Patient Escort Services - These associates will transport you to different areas of the hospital . Environmental Services - The associates in environmental services clean your room daily, remove trash, and replenish any products necessary . Patient Advocate - A patient advocate will help resolve any issues that you may have as well as answer any questions regarding hospital policies and procedures .

13 PAIN MANAGEMENT The severity of pain and the amount of medication required to treat it vary from patient to patient ..The physicians and nurses caring for you must rely on you to help determine the best way to treat your pain ..You will be asked to asses your pain and rate the level on a scale of 0 to 10, 0 being no pain and 10 being the worst possible pain ..With the use of this scale your physicians and nurses can gauge the effectiveness of their pain control methods and adjust accordingly .. Side Effects of Pain Medications All medications that are used to treat pain have side effects but, not everyone experiences them ..Common pain medication side effects include constipation, nausea, sleepiness, slowed breathing, and itching ..These side effects are treatable ..If you have any questions or concerns about side effects please ask any member of your health care team . YOUR DISCHARGE Instructions Your doctor will determine whether you are ready to leave the hospital, and will write a discharge order for you at that time ..You should be given printed discharge instructions, including which medications you are to take at home .. For your health and safety, be sure to discuss these directions with your doctor and nurse ..Checkout time is two hours after your doctor writes your discharge order ..Your discharge may be coordinated by a nurse, social worker, or an administrator with the hospital ..You or your care giver should meet this person towards the beginning of your stay ..Make sure that you are given the following information before you leave the hospital: • Discharge summary . • A medications list . • A prescription for any medications that you will need . • Any documentation detailing further instructions that you may need to follow once you have left the hospital such as dietary restrictions, activity restrictions, wound or incision care instructions, or care giver instructions .
