UNIVERSITY AND PUBLIC SERVICE

SERVICE ACTIVITIES SUMMARY I have a long-standing and strong commitment to service focused on improving the US health care system in regard to advancing oral health. Nationally, I was a member of two prestigious National Academies efforts; first the Institute of Medicine Committee on Oral Health (2010- 2011) and second the National Research Council Committee on the Skilled Workforce (2015- 2017). My service on these committees was invited due to my unique sociological, policy and health services research perspectives on the health care delivery system and health professions, especially in dentistry. I have dedicated time to professional organizations both in dentistry and health services research as a board member, committee chair and member, and journal reviewer, and have served on advisory boards for numerous local and state organizations seeking expertise in objective evaluation and research. I am part of an international collaborative group founded in 2015 focused on oral health disparities worldwide. I served as an invited guest editor for two special issues of national journals. In 2010, stemming from interest in the IOM Oral Health Workforce workshop, I served as the sole guest editor of a special issue of The Journal of Public Health Dentistry (JPHD) that examined workforce innovations in the context of dental care delivery improvements. In this role, I secured funding from the California HealthCare Foundation to support publication of the special issue, and I solicited and edited over a dozen manuscripts in addition to preparing one of my own. The articles in this special issue remain among the top downloads of JPHD. In 2012 I served as a co-editor and co-author on a special issue of Nursing Research and Practice that examined nursing’s role in oral health care delivery, contributing to a growing

11 of 45 Prepared: October 27, 2019 body of research about the importance of interprofessional practice to improving the oral health of the nation. In 2018 I led a two-decade update and significant rewrite of the workforce chapter of the premier dental public health textbook (Burt & Eklund) and 2019 I was invited to contribute to the 2020 Surgeon Generals' report on oral health. I have served as a reviewer on multiple NIH review panels; I was selected for my cross-cutting expertise in health services, policy, workforce and disparities research in oral health. I also have served as an advisor and reviewer for numerous foundations and associations on proposals and projects related to oral health care systems and workforce.

At UCSF, I have played an active role on committees and workgroups to advance the overall mission and function of the Healthforce Center (formerly the Center for the Health Professions), where I have worked since 1997. I led our OE pre-award transition and IT system transition, and participated as core faculty in the succession planning for a new director, all significant undertakings. As well, I have developed and deployed leadership impact evaluation efforts for the Center's suite of programs. I have been engaged in numerous faculty activities in the School of Dentistry, providing service in strategic planning and candidate reviews, assisting the department chair with data collection and analysis in his initiative to improve the organizational culture, participating on a select committee focused on bringing quality improvement activities to the curriculum, and chairing a student status committee (4 year commitment). I have served two terms on Faculty Council (2015-2021) and will have chaired it twice in this tenure which comes with responsibility of organizing our annual retreat and being a liaison to the Academic Senate. As well, I have served a three year term on both academic planning and budget (APB) and admissions committee (2016-19). In 2016 I joined the SOD research advisory committee. In 2018 was elected my department representative for the Health Sciences Compensation Plan Committee (HSCP) for a four year term, and appointed to my department research finance planning committee for 2019. In 2017-18 I served on the climate survey work group in SOD, and have now transitioned to be a member of the newly formed Diversity committee. Finally, in 2018-19 I led one of seven workgroups preparing for our 7-year accreditation visit and assisted with numerous meetings during the site visit.

In service to the wider UCSF community, I reviewed abstracts for the UCSF Health Disparities Symposium, and have been a reviewer on the social and behavioral sciences committee of the Resource Allocation Program (RAP) since 2016. I was invited to serve on the Chancellor's Committee for Family Life due to my perspective of being a SOD faculty member with small children and also my previous engagement with the Mission Bay UCSF childcare center. This committee was particularly rewarding for me personally having navigated the challenges of maternity leave, breastfeeding, and childcare while balancing academic productivity since the birth of my two children in 2011 and 2014. As faculty council chair, I serve as ex-officio on UCSF Academic Senate Executive Council and APB. In 2019 I was appointed to chair a Stewardship review committee for the Dr. Lietman, director of the Proctor Foundation.

In summary, I am invited and volunteer at many levels to assist organizations and individuals to understand, research, and address the challenges facing our oral health care system as it undergoes increasing pressure to transform delivery of care and reform professional practice to meet the oral health needs of a changing society. Personally, I volunteer in my community to teach basketball skills to girls, and I co-founded the organization Women with Altitude to raise global awareness about domestic violence. This organization produced a full length movie about overcoming obstacles which is now distributed worldwide.

CONTRIBUTIONS TO DIVERSITY My research on the health professions workforce exhibits my deep commitment to examining issues of diversity and the impact of workforce diversity on health equity. My work has been used in program development at UCSF and informs our SOD holistic admissions review efforts. My research contributions continue to support efforts to understand disparities within the Center to Address Disparities in Children's Oral Health (CANDO), work that has culminated in health disparities focused grants including a UH2 from NIDCR and R01 from NIMHD. In my teaching, I mentor a diverse set of students and junior faculty and am committed to supporting the success of these individuals. I serve on both the the SOD climate and diversity committee which are focused directly on improving the institutional culture for our diverse set of staff, learners and faculty.

TEACHING AND MENTORING

TEACHING SUMMARY My overall teaching goal is to bring a broad health policy, health services research and sociological understanding of issues confronting the dental delivery system (i.e. quality improvement) to our students, faculty and graduates so they can be better informed consumers of policy information and more strategic in their efforts to improve health and health care. My teaching approach is to create real-world, hands-on projects that require students to apply critical thinking skills to analysis of current events in the context of appropriate theoretical models.

In 2014 I became the course director for the MHA 203 health policy course in the interdisciplinary Masters in Health Administration and Interprofessional Leadership (MS-HAIL) program in the graduate division and school of nursing. In Winter 2017 I updated this online course to reflect the rapidly changing health policy environment, including updating texts, articles, content and links. Students continue to comment to me that this course opened their eyes and empowered them to become more policy engaged.

I bridge this general content in health policy to the School of Dentistry through a focus on teaching dental care policy and health services research methodology, leadership development, and topical lectures in dental care services research. These topics and approaches are an important addition to the dental curriculum as critical elements in preparing the next generation of leaders in dental care -- leaders that will enter the workforce in a time of rapid health system change and be essential to shaping and improving the dental delivery system of the future.

My lecture on the state of quality improvement in dental care is highly regarded and has been requested repeatedly by students and faculty alike, and integrated as a core annual talk in three courses. I continue to be approached by students looking for mentorship who have a strong interest in policy issues but lack other avenues to gain exposure and experience with these topics. I guest lecture every year in existing courses to provide research methodological expertise, updates on current policy and practice issues, and other hot topics such as workforce changes in dentistry. Dental students and residents, dental hygiene master’s students, and even dental faculty have little formal exposure to health policy and health services research outside the lectures and courses I conduct, and yet dental policy’s evolution will have a large impact on the future practice of dentistry.

I have developed curricula for and continue to teach as needed in leadership development programs at the Healthforce Center (formerly Center for the Health Professions) since 2003, including courses utilizing Myers-Briggs Type Indicator (MBTI) and the System for the Multiple Level Observation of Groups (SYMLOG) assessments in which I am certified.

MENTORING SUMMARY I mentor graduate students and serve on thesis and doctoral dissertation committees, oversee research fellowships and advise dental and dental hygiene students and residents. Students interested in dental policy and social science research regularly request my mentorship because they have interest in the real-world applicable research in which I am engaged within dentistry. All of the dental students and residents I have formally mentored have presented their research work at the annual SOD Research and Clinical Excellence Day. Nine of these students have won fellowship awards (i.e. California Pediatric Dental Society Fellowship, Global Oral Health Fellowship and Delta Dental Summer Research Fellowship) to support their mentored research with me, attesting to the significance and quality of their work. I also mentor three junior faculty who I meet with regularly to support their career development.

RESEARCH AND CREATIVE ACTIVITIES

RESEARCH AND CREATIVE ACTIVITIES SUMMARY Health services and policy research is a rich field with a long and distinguished history at UCSF. Dental health services research is nascent in comparison, and yet increasingly becoming critical as the dental care field enters a time of rapid transformation. The Affordable Care Act (ACA) has reshaped the dental marketplace, the dental workforce is evolving with the advance of technology and introduction of new types of providers, and new models of medical- dental integration are being explored. New science on the oral-systemic connection is pushing translational researchers to develop cross-cutting approaches. Faculty at the School of Dentistry collectively are rethinking how to best prepare students to be dentists of the future. Designing inter-professional practice, implementing quality metrics and improvement, developing value-based purchasing, and finding effective means of translating science to evidence based practice are all critical issues that dentistry now faces. Health services and policy research is needed more than ever to inform, support and guide these practice and policy changes.

My research program centers on advancing this dental health services and policy body of work. My public policy and sociology training provide me with a strong theoretical base along with quantitative and qualitative methodological expertise, while 22 years of research at UCSF has provided deep content knowledge of both dental and broader health care issues. These skills and knowledge, combined with my UCSF affiliations and connections to dental health services researchers around the globe, position me to successfully continue my already well- established portfolio of research. The United States is not alone in facing challenges and rapid change in ensuring access to high-quality dental care, therefore I am actively working with an international network of dental health services researchers who collaboratively can advance the understanding of dental care delivery systems and professions and supply this evidence to policy makers, educators, and practitioners working to address the oral health care needs of the public.

My creative work has evolved during my career at UCSF, which spans 12 years as a staff researcher (five of which I was also a doctoral student) and ten years as SOD faculty. Supply, demand, distribution and diversity of the workforce has been the staple of my scholarship: from medical education, to nursing practice, to dental workforce, to pharmacy leadership roles, this research has been based on the examination of how the health professions shape the changing health care environment, how they are shaped by it, and what the implications are of this dynamic for access to high quality, affordable health care. Using both primary data collection and analyses of secondary data I have pioneered studying the dental and dental hygiene workforce, producing a number of articles and reports examining labor market trends, shortages, diversity (or lack thereof) and supply projections. This work is currently funded through a HRSA cooperative agreement that funds the Oral Health Workforce Research

Center, of which I am PI at UCSF. These workforce analyses have had direct policy impact by contributing to the national policy discussion about mechanisms to address the distribution and composition of health providers, an ongoing problem that contributes to poor access to health care, and a topic upon which I am a known national expert.

Proposed dental care delivery system reforms provide the basis for a forward-looking agenda that will examine the capacity, diversity and differentiation of the dental workforce, as well as focusing on modalities for delivering preventive dental care. Utilization of primary care providers and the creation of new dental providers are underway, even though quality issues, the scope of practice of new providers, and appropriate supervision levels are unresolved. I am well-prepared to lead health services and policy research that will inform dental leaders and policy makers about the impacts and value of such reforms, as shown by my award to lead a three year evaluation of a national Dental Therapy Initiative evaluation and a new award to examine the impact of policy change in California on adult dental access. My work leading a national sample survey of Underrepresented Minority Dentists is the first comprehensive documentation of the contributions of minority providers to minority oral health care. A key publication from this work was included in the first special issue of Health Affairs on Oral Health, and I was asked to present at policy briefings in both Washington DC and Sacramento on the topic.

I am currently PI on an R01 funded by NIMHD. This disparities study examines outcomes between Medicaid and commercially insured children receiving a standardized preventive care protocol, building from a UH2 feasibility study funded by NIDCR. This work moves dental health services and policy research from its traditional focus on provider supply to a more nuanced examination of evidence-based practice and progress toward the Triple Aim in dental care. This study built on our work examining the implementation of clinical decision support tools in the electronic dental record of a large group dental practice. That study was the first to examine the impact on providers, workflow and patient satisfaction of clinical decision support in dental health. As an acknowledged expert in this arena, and a pioneer in scholarship on these topics, I expect to lead the way in developing an expanded knowledge base on all aspects of the dental workforce with a broad goal of improving access, efficiency, and the transparency and measurement of quality care.

Research on the dental care delivery system and the dental workforce is rarely framed within sociological theory or practice. Understanding challenges and changes in the dental care field will require the use of a wide sociological lens that can fully account for the power of the professions to enact or prohibit change, alongside empirical health services and policy research that documents trends in the organizational, environmental and social context and demonstrates where and how improvements can be made.

This pilot study will lay the groundwork for an examination of dentistry’s path to its current policy state, aiming 1) to map the evolution of contemporary U.S. dental care policy and practice since 1950, (2) to elucidate the policy circumstances leading to the dental system’s separation from the rest of health care, and (3) to explore the implications of this ongoing separation for the future of oral health policy, and especially for system reform. SIGNIFICANT PUBLICATIONS 1. This 2012 article stemmed from my dissertation work, Professional Projects and Institutional Change in Healthcare: The Case of American Dentistry published in Social Science and Medicine, examines sociologically the development of alternative practice archetypes in American Dentistry through the professionalization projects of dental hygienists. This paper is one the very few theoretically rich sociological examinations of dental care ever to be published in this highly regarded journal. Author: conceived of the research project and hypotheses, obtained the data, performed qualitative and quantitative analyses, drafted the initial manuscript, addressed reviewers' concerns and approved final manuscript.

2. “Shaking up the Dental Safety-Net: Elimination of Optional Adult Dental Medicaid Benefits in California” was chosen as part of a special journal issue focused on the roll out of the Affordable Care Act and the impact on vulnerable populations. This paper informs policy makers in States as they weight the decision to expand Medicaid under the ACA and the implications of including optional benefits such as adult dental care. Senior Author: designed the research project, hypotheses, and the data analyses, collected primary data, edited the initial manuscript and approved final manuscript. 3. “Underrepresented minority dentist: Quantifying their numbers and characterizing the communities they serve” (2016) was a follow up to “Community Characteristics that Predict the Low Supply of Dentists in California,” (2001), both of which I led. This paper was featured in the first ever special issue on oral health of Health Affairs, and was among the first contemporary research to establish the link between the race/ethnicity of dental practitioners and their propensity to serve underserved communities, and the first to pursue this line of inquiry over time. This work informs the ongoing examination of the dental workforce issues of supply, demand, distribution, and diversity -- all topics that are still of great national concern -- and has been followed by large philanthropic and federal investments in dental education to address workforce diversity. Lead Author: obtained data, performed analyses, drafted initial manuscript, addressed reviewer concerns and approved final manuscript 4. "The impact of federal and state policy on oral health care delivery in long-term care settings"' was published in the Journal of Health Care for the Poor and Underserved, following multiple years of data collection from stakeholders, and policy analysis related to key findings. This paper is the first to comprehensively examine policy barriers on a state by state basis to the already difficult process of engaging dental care providers in this setting. The findings provide a clear set of recommendations for policy change to improve the dire state of dental care for seniors living in long-term care. Senior Author: Designed the research project, hypotheses, and the data analyses, oversaw collection of primary data, edited the initial manuscript and approved final manuscript. 5. Most recently, the publication, “Developing and testing electronic health record-derived caries indices" in Caries Research, 2019, is the first ever development of an EHR-based caries (dental decay) outcome measure replicating the long standard of Decayed, Missing, Filled, Teeth (DMFT) used in national epidemiological studies. This work provides a clinical outcome measure for use in health services research in dental care, formerly restricted only to treatment and utilization measures. Senior Author: PI on UH2 grant that produced this work, contributed at all stages, data extraction, design, writing, editing, addressed reviewer concerns and approved final manuscript.

ACADEMIC LEADERSHIP In 2017-18 and again in 2019-20 I was elected chair of the school of dentistry faculty council. As chair in both cases I was tasked with participating in significant issues of concern including recruitment for a new Dean, planning and execution of our SOD accreditation, and revisions to many of the committees of the council (Admissions, EPC, ABP, Scholarship, Student Status) bylaws and functions. I planned two all-school retreats and liaison with multiple other senate committees in this role. As well, I have been elected to be my department representative on our Health Sciences Compensation Plan committee. Finally, I was asked to Chair a stewardship review for the Proctor Foundation.

4.Centers for Medicare and Medicaid Innovations Challenge. Evaluation of the Virtual Dental Home. $11 million. Received favorable reviews and were encouraged to resubmit in the second round. Facilitated a RWJF planning process that ended in awarding UCSF a $900,000 grant to evaluation this program and 11 others across the country.
