We are seeking to appoint a clinical nurse specialist, specialising in mitochondrial diseases, at the John Radcliffe Hospital, University NHS Foundation Trust. The post holder will contribute to the nationally commissioned Highly Specialised Service for Mitochondrial Diseases.

The appointment is on a part-time basis (0.5 whole-time equivalent).

Applications are invited for a clinical nurse specialist, with a special interest in mitochondrial diseases, to join the NHS Highly Specialised Service for Rare Mitochondrial Diseases at the Oxford University Hospitals NHS Foundation Trust (OUHFT). The vacancy has arisen due to the expanding local clinical service and the need for further nursing support for this national service.

Oxford is a large tertiary neurosciences centre, with a busy Clinical Genetics department, and offers a wide range of clinical and academic interests. Oxford is renowned for its expertise in neuromuscular diseases, and is a member of the European Reference Network for such conditions. We are looking for an enthusiastic nursing colleague to develop the role of nurse specialist within the mitochondrial service, together with a well-established group of colleagues from paediatrics, clinical genetics, adult neurology and other disciplines. The post will include providing liaison for the nationally commissioned Highly Specialised Service in Mitochondrial Diseases, and it will deliver a weekly mitochondrial clinic and MDT as well as ward liaison service. While this is a clinical post, there are ample opportunities to participate and develop interests in teaching and academic work in this large University Hospitals Trust.

2. Job Description

1. JOB TITLE: 0.5 whole-time equivalent Clinical Nurse Specialist in Mitochondrial Diseases

2. GRADE: Clinical Nurse Specialist (band 6)

4. ACCOUNTABLE TO: Matron for Clinical Genetics

5. PRINCIPAL ACTIVITIES:

The post-holder’s duties will be primarily at the John Radcliffe Hospital but it is a condition of the appointment that the postholder will be willing to work in any of the Trust’s locations.

Clinical It is preferable that the post-holder will have sub-specialist expertise in neurology and/or clinical genetics, as shown by previous experience in doing specialist clinics and working within a subspecialty multidisciplinary team. The post holder will also have knowledge of the management of mitochondrial disorders.

The Clinical nurse specialist will provide holistic support to patients with mitochondrial disease and work within a multi-disciplinary team. The post-holder will be expected to support clinical activities, including, but not limited to, in-patient reviews, out-patient and telephone clinics under the supervision of the medical consultants for the team.

Teaching/Research OUHFT is a teaching hospital trust, and the post-holder will be required to participate in programmes for teaching clinical students, training junior nurses and in clinical examinations. It is expected that this will be an integral part of everyday clinical activity. Research experience in muscular diseases and/or genetic disorders will be welcome and be supporting evidence of a sub- specialist interest in this field. While this is a clinical post, contribution to academic work will be encouraged.

Clinical Governance The post-holder will participate in all clinical governance activities, including clinical audit, clinical effectiveness, risk management, quality improvement activities as required by the Trust, and external accrediting bodies.

Personal and Professional Development The post-holder will be required to keep him-/herself fully up-to-date with their relevant area of practice, and will have to be able to demonstrate this to the satisfaction of the Trust. Professional or study leave will be granted at the discretion of the Trust, in line with the prevailing Terms and Conditions of Service, to support appropriate study, postgraduate training activities, relevant CME courses and other appropriate personal development needs. The Trust is committed to supporting the appointee for the continued process of revalidation.

Management The post-holder will be required to work within the Trust's management policies and procedures, both statutory and internal, accepting that the resources available to the Trust are finite and that all changes in clinical practice or workload, or developments requiring additional resources must have 3

prior agreement with the Trust. He/she will undertake the administrative duties associated with the care of their patients, and the running of their clinical service under the direction of the lead clinician and matron. Administrative support and facilities for the post holder is provided in the form of office space, and the band 4 and band 3 secretarial support the department receives. .

Risk Management The management of risk is the responsibility of everyone and will be achieved within a progressive, honest and open environment.

Staff will be provided with the necessary education, training and support to enable them to meet this responsibility.

Responsibilities for Health & Safety The post holder is responsible for ensuring that all duties and responsibilities of this post are carried out in compliance with the Health & Safety at Work Act 1974, Statutory Regulations and Trust Policies and Procedures. This will be supported by the provision of training and specialist advice where required.

Infection Control Infection Control is everyone’s responsibility. All staff, both clinical and non clinical, are required to adhere to the Trusts’ Infection Prevention and Control Policies and make every effort to maintain high standards of infection control at all times thereby reducing the burden of Healthcare Associated Infections.

NHS HIGHLY SPECIALISED SERVICES FOR RARE MITOCHONDRIAL DISEASES

The NHS Highly Specialised Services (HSS) for Rare Mitochondrial Disorders were established in April 2007 to provide a comprehensive high quality NHS service for this rare group of disorders, with centres in Oxford, London and Newcastle. Highly specialised services are provided by the NHS for services with small numbers of patients, such as rare mitochondrial disorders, which are best delivered nationally through a very small number of centres of excellence. Mitochondrial Disorders are an extremely complex and diverse group of disorders. They can cause virtually any symptom in any organ at any age. Patients may present to a variety of different clinical specialties, and diagnosis is often difficult. Also, many aspects of mitochondrial disease can be helped or prevented by early diagnosis and subsequent care. Therefore, the need for a specialised national service has been recognised by the NHS, with the main objective of improving the diagnosis and clinical care for patients with rare mitochondrial disorders. The three centres have an international reputation for clinical care and research in mitochondrial disorders, and provide comprehensive specialised services including:

 multi-disciplinary clinical service consisting of genetic counselling and clinical management for adults and children  diagnostic service for patients with suspected mitochondrial disorders, including analysis of samples referred from local clinicians  clinical management advice for clinicians  specialist nursing input for symptom management, advice and support for health and social care issues – signposting, liaising with local health and social care, and advocating on behalf of individuals as required  specialist physiotherapy assessment and advice – signposting and referring onto local community therapy and social care services as required  comprehensive information for patients and carers  national patient forum for education and information including national patient days  patient access for participation in research to develop new diagnostic techniques and clinical trials to improve the treatment of Mitochondrial Disease

The West Wing, John Radcliffe Hospital, Oxford

The West Wing houses a large neurosciences ward with dedicated neurology and neurosurgical beds, a large neuro-intensive care unit, the neurosurgical operating theatres, a neuro day case unit, the departments of neuroradiology, neuropsychology, neurophysiology and neuropathology, and the Nuffield Department of Clinical Neurosciences, as well as the departments of ophthalmology, ENT, maxillofacial and plastic surgery.

______Currently the Mitochondrial Service hold two clinics per month in the Neurology Out- Patients Department.

The Horton General Hospital in Banbury

The Horton General Hospital in Banbury serves the growing population in the north of Oxfordshire and surrounding areas. Services include an emergency department, acute general medicine and elective day case surgery, trauma, maternity and gynaecology, paediatrics, critical care and the Brodey Centre offering treatment for cancer.

The Mitochondrial Service currently offer an out-patient clinic in the Horton Hospital once every 2-3 months, and the post holder will be expected to attend this clinic with the clinical lead.

6. Main Conditions of Service

1. The post is covered by the national Terms and Conditions of Service of Hospital Medical and Dental Staff ( and Wales) and the Conditions of Service determined by the General Whitley Council for the Health Services (Great Britain), as amended from time to time. As the Trust develops it will review these terms and may vary the national agreements to respond to local conditions and reflect Trust requirements following negotiation with the NMC Local Negotiating Committee.

2. The new clinical nurse specialist will be required to maintain his/her private residence in contact with the public telephone service, and to reside within ten miles by road from the John Radcliffe Hospital unless specific approval for a greater distance is given by the Trust.

3. The successful candidate must hold full registration and licence to practice with the NMC.

4. The successful candidate must have diploma or degree level nursing (or equivalent for non-UK applicants of equivalent status), and be on the NMC Register in the specialty appropriate for the post at the time of taking up the appointment.

5. The successful candidate is not required to subscribe to a recognised professional defence organisation to fulfil his/her contractual obligations to this Trust, but should ensure he/she has adequate defence cover as appropriate, for example, for private and Category 2 work, and for NMC disciplinary proceedings.
