Approximately 3 miles turn right on Dutch Road. Another 3 miles turn right on Finzel School is on your right. Allow 30 minutes ARCHBOLD HIGH SCHOOL ~ 445-5579~ Left on 795 East, left on 280 South, right to Ohio Turnpike West. Take exit 34 for Route 108. Turn left on 108, turn right on Route 20 and left on Route 66. Turn right on W. Holland Street. Allow 60 minutes ARCHBOLD SOCCER FIELDS~ Follow directions to the High School, but don’t turn onto Route 108. Pass 108 and turn right on the next street. Cross Route 64, Cross RR Tracks. At the woods, turn left on Tontogony Road and follow to the edge of town. Before you cross the RR Tracks, turn right on North Street. North ends at school. Exit to left at Willys Parkway. Turn right on Willys Parkway, left on Sylvania Road and take slight right turn onto Tremainsville. School is on your right. This comprehensive Community Health Assessment is the result of a strong commitment by dedicated community partners working together to improve the health and quality of life of Wood County residents. The health assessment gives us a snapshot of our community as well as a comparison to the state of Ohio and our nation. It is our wish that the data presented will provide valuable information to many organizations to develop strategies and implementation of services that will focus on wellness, access to care, and unmet community needs. It will provide insight into our community’s structure in the areas of health and well-being. This data will allow community stakeholders to write better grants, formulate strong strategic plans and answer a call to action.     2 0 1 8 WOOD COUNTY C O M M U N I T Y H E A L T H A S S E S S M E N T C o m m i s s i o n e d b y W o o d C o u n t y H e a l t h P a r t n e r s Foreword Members of the Wood County Health Partners are proud to present the 2018 Wood County Community Health Assessment. This report is the result of dedication to a partnership that is focused on improving the health of our community. This Community Health Assessment is a data-driven look at the health of adults, youth and children who live in Wood County. It compares new data to information from previous assessments, and also to data from across the state of Ohio and the United States. Partnership has been the hallmark of the process that led to the creation of this report. Wood County Health Partners sought to include as many stakeholders as possible so that the Community Health Assessment represents their interests and reflects their needs. The goal of the document is to highlight the strengths and needs of Wood County, so that organizations and agencies involved in the assessment process can make sound decisions using the best available data. It should help identify unmet needs, increase access to services, and provide insight into the best ways to favorably move the needle of the health of people in Wood County. While addressing every challenge or opportunity for improvement would surely be overwhelming, the 2018 Community Health Assessment should help guide health priorities in Wood County for the next several years or longer.
