We continue to strive to create a concise, cumulative and informative listing of the diverse prevention programs offered throughout Wood County. While we have worked diligently to have a complete listing of prevention programs, we recognize that this guide is a work in progress. Behavior Support is the implementation of growth-enhancing strategies that shall actively promote a person’s ability to choose, to safely express individuality, and to engage in personalized interactions through self-determination and self-management. Block Watch Type: Law enforcement Who is Served: Bowling Green residents Where: Bowling Green, Wood County When: Year round Desired Outcome: Block Watches are neighborhood-run. The Bowling Green Police Department only offers assistance/information for those neighborhoods that wish to start a block watch. These neighborhood groups manage themselves; they hold meetings and set up patrolling times. The BGPD assigns Block Watch member numbers to the group members. This speeds up the process of reporting information to the police when they call. To be a recognized Block Watch these groups must have at least one yearly meeting. Bobcat P.L.U.S. (Positive Living Utilizing Skills) Type: Educational Who is Served: Bowling Green high school students Where: Bowling Green High School When: School year Desired Outcome: This program is curriculum-based and taught at Bowling Green High School. It is a follow-up to the E.Z. P.L.U.S. program and demonstrates the differences between the juvenile court system and the adult court system. Drinking and driving is also addressed since many of the students are now driving or will be soon. This program is taught based on scheduling needs of the school. Breast and Cervical Cancer Screening Program Description of program: Assistance with eligibility determination allowing uninsured women over age forty to obtain breast and cervical cancer screening. Wood County Health and Wellness Center Adult Clinic is one site where services may be obtained. Medicaid eligibility conferred on women diagnosed through this program. Who is Served: Uninsured women over age 40 Where: The application is taken at Wood County Health District. Several providers cooperate with the program. The program is part of a 10 county consortium managed by the Fulton County Health Department. When: Ongoing Desired Outcome: Improved mortality rates due to early detection and treatment of breast and cervical cancer. Cost: Income eligibility requirements. Agencies: The Wood County Community Health and Wellness Center

Bureau for Children with Medical Handicaps Description of program: A diagnostic component assists with referrals to specialists and the expenses associated with the initial diagnosis. Ongoing treatment includes service coordination to families with special health care needs. The program includes a payer of last resort component whereby families that exceed their insurance limits and meet financial eligibility may have assistance with medical bills, when the provider is enrolled in the Ohio system, and special supplies that are required for the child’s medical condition. Contact BCMH Nurse. Who is Served: Children birth to age 21 with special health care needs. Where: Home visits. May include visits to school, medical appointments, special clinics, or Health District. When: Health District hours 8:30 A.M. to 4:30 P.M. Monday through Friday. Visits at other hours may be possible to meet the needs of the family. Desired Outcome: All children will have optimal health and maximal achievements. Cost: No cost to family. Diagnostic assistance is provided as a payer of last resort. Eligibility for assistance with treatment is based on family income and may involve a spend–down component. Eligibility is also based on medical diagnosis. Agencies: Wood County Health District

Cash Assistance, Food Assistance, and Medicaid

Ohio Works First (OWF) (TANF) • Emphasizes employment first. It includes job search and preparation plus services to prevent job loss. Temporary cash assistance is provided to families with minor children up to age 18 (19 if still in school) if employment is not available or is inadequate. Recipients are limited to 36 months of cash assistance effective October 1, 1997. After the 36 months expires, there may be up to an additional 24 months of cash if a hardship or good cause (as determined by the CDJFS) exists. Food Assistance • Known nationally as the Supplemental Nutrition Assistance Program (SNAP), this program helps eligible needy Ohioans pay f or food. Ohioans may qualify for benefits if their gross monthly household income is at or below 130% of federal poverty guidelines. If someone is elderly or disabled they may have income above 130% and still qualify. Recipients use an electronic benefits transfer card-Ohio Direction Card- to buy food or food products with the exception of alcohol, tobacco and hot foods. Medicaid • Coverages o Pays doctor, hospital, prescription expenses, dental and optical coverage. o Provides nursing home care for those determined to be eligible. o Waiver services-funds in-home care to prevent institutionalization.

• Programs o Adult Extension: Medicaid coverage for individuals ages 19-64. o Parents and Caretaker Relatives: Medicaid coverage for parents and caretaker relatives with children under 18 (19 if still in school) o Pregnant Women: Medicaid coverage for women throughout the pregnancy and 60 days postpartum, o Children: Medicaid coverage for children up to age 19. Coverage for Children in families with incomes above 156% of the federal poverty level is available only if the children have no other creditable health insurance. o Presumptive Eligibility for Children: Immediate, time-limited Medicaid coverage or children up to age 19. o Presumptive Eligibility for Pregnant Women: Immediate, time-limited Medicaid coverage for pregnant women. o Refugee Medical Assistance (RMA): Time-limited Medicaid coverage for refugees. The program provides a medical screening and other medical services to qualified aliens. o Alien Emergency Medical Assistance (AEMA}: Medicaid coverage for the treatment of emergency medical conditions for certain individuals who meet all Medicaid requirements other than the citizenship requirements. o Transitional Medical Assistance (TMA): Twelve months of continuous Medicaid coverage for families who would otherwise lose coverage because a family member got a new job or is earning more money. o Children in Care/Former Foster Children in Care: Medicaid coverage for children in the custody of a public children services agency, in receipt of foster care or adoption assistance under Title IV-E, or in receipt of state or federal adoption assistance. o Continuous Eligibility for Children: Twelve months of continuous eligibility is available to every child up to age 19 who gets Medicaid, o Adults Age 19 and 20: Medicaid coverage for individuals ages 19 and 20, Family income may be used in the eligibility determination. o Aged, blind or disabled (ABO}: Medicaid coverage for people who are at least 65 years old and individuals of any age who are blind or disabled. o Medicare Premium Assistance Program (MPAP): Medicaid assistance programs that help pay Medicare costs, o Qualified Medicare Beneficiary (QMB): Pays Part A and B premiums, deductibles, co-pays and co-insurance. o Specified Low-Income Medicare Beneficiary (SLMB): Pays Part B premiums only, o Qualifying Individual IOI): Pays Part B premiums only, o Qualified Disabled and Working Individuals IOWDI): Pays Part B premiums only. o Medicaid Buy-In for Workers with Disabilities (MBIWD}: Medicaid coverage for working disabled individuals ages 16 to 64. If your income is above a certain amount, you may need to pay a premium to get MBIWD. o Residential State Supplement (RSS): A supplemental cash payment program for aged, blind or disabled people who need a protected level of health care as determined by a health care provider, RSS helps to pay the costs of living in certain residential care facilities. o Long-Term Care or Waiver Services: Long-term care or waiver services are available for individuals who have special care needs, as determined by a health care provider. o Breast and Cervical Cancer Project (BCCP): Medicaid coverage for certain individuals who need treatment for breast or cervical cancer, breast or cervical pre-cancerous conditions. Child Fatality Review Description of program: Annual review of all deaths to Wood County residents, age birth through 17 years. State law mandates the review and specifies committee makeup. Committee work is closed to the public, but is submitted to the Ohio Department of Health for inclusion in a statewide document compiled annually. Child Health Clinic Description of program: Well child checkups, sick care, prescription and laboratory services, and assistance with referrals to other services. Child checkups include anticipatory guidance, early detection of problems, referrals for additional services. Children’s Advocacy Center Type: Provides services for child victims of sexual abuse/severe physical abuse & non- offending caregivers. Provides support & advocacy for families of sexually abused children. Conducts forensic interviews for child victims conducted by trained law enforcement or Children’s Services Worker. Crisis counseling for child victims. Offers safety education for children ages 3-17 & non-offending caregiver sessions to educate & support caregivers through investigative & court process, & the Children Who Witness Violence program for children who have witnessed domestic violence. Children’s Services

Children’s Services provides a number of services to Wood County residents regardless of financial status. Investigation/Assessment • Receive & investigate reports of child abuse & neglect • A comprehensive investigation & risk assessment is completed to determine what services or referrals the children & family need. Protective Services • Home based contact with the family in an effort to prevent abuse & neglect & strengthen the family • Families are provided with community resources, education, guidance, & support. Placement Services • If children are unable to remain in their own homes or with relatives, the agency will request temporary custody from the Juvenile Court. Children are then placed in foster homes, group homes, or residential treatment settings. The agency always seeks the least restrictive placement for children & works with the family toward reunification. Foster Home Certification • Recruits, assesses, trains, & supports foster families • Those meeting the state requirements will be certified. • Ongoing training & support is provided to all foster families. Adoption Services • Finds appropriate permanent families for children who are available for adoption • Home studies & certification requirements are completed on families who wish to adopt children. • Post adoption services are also available. Children’s Services Unit • The Wood County Department of Job and Family Services, Children’s Services Unit works with children from birth to age 18 and their families. The agency can work with a child who has been diagnosed with a Developmental Disability until the age of 21. Children’s Services investigates reports of child abuse and neglect, completes a comprehensive family assessment, and determines what services or referrals are needed for the children and family. If it is determined that protective or placement services are necessary, the agency files a complaint through the Juvenile Court. The goal of protective services is to preserve the family unit by strengthening the parent’s ability to provide a safe environment for their children. This program seeks to prevent future abuse and neglect before the situation advances. Placement services involve children who are in the agency’s temporary custody. We do this through a network of 11 Learning Centers located at Glenwood Elementary in Rossford; Northwood Elementary; Powell Elementary in North Baltimore; Conneaut, Crim and Kenwood elementary schools in Bowling Green; Luckey and Pemberville elementary schools in Eastwood; Genoa Elementary in Genoa; Woodmore Elementary; and Elmwood Community Center. Crisis Intervention Program Description: 24-hour services to provide stabilization and assistance to eligible individuals. Eligibility may be determined simultaneously to crisis intervention services. Agency: Wood Lane School and the Wood County Board of Developmental Disabilities

Customer Services and Information/Referral Program Description: This provides access to a wide spectrum of information on disability-related issues, ranging from where to get a disability placard for a car to future planning for yourself or a relative. It also provides links to community resources that assist individuals to live independently. In addition, staff oversee a disability- related equipment loan and gift closet that permit individuals with disabilities to access durable medical equipment such as wheelchairs, walkers and bath benches. Diversions Type: At times, a Wood County family or school requests assistance from the Court when a child/children are exhibiting unruly or delinquent behavior. As an alternative to filing formal charges against a youth, a diversion can be held to dissuade him or her from future inappropriate behavior. The diversion itself is an informal conversation with a youth and parent. The focus is to explain what can legally happen should a youth continue to make poor choices. Families are frequently referred to outside agencies, such as the Children’s Resource Center or the Parent Project for assistance. Tours of the Wood County Juvenile Court Detention Center are typically not a part of the diversion process. Desired Outcome: Offer prevention, intervention, and cessation services in the schools and community in an effort to reduce the youth smoking rate. Agencies: Promedica St. Luke’s Hospital

Drug Abuse Resistance Education (D.A.R.E.) Type: Educational Who is Served: First, Fifth, and Sixth grade students Where: Wood County When: School year Desired Outcome: The Drug Abuse Resistance Education program is a ten-week course. This program focuses on improving the decision-making skills of first, fifth, and sixth grade students. Educating the students on the harmful effects of drugs, alcohol, and tobacco is how this is accomplished. The times that this program is offered is based on the scheduling needs of the individual schools. Agencies: Bowling Green Police Department, Perrysburg Police Department, Rossford Police Department (5th grade), Wood County Sheriff’s Office

Drug Testing Type: The Wood County Juvenile Court offers drug testing to the community’s juvenile population for a small fee. At present, the department can test for alcohol, amphetamines, barbiturates, benzodiazepines, cannabinoids, cocaine, ecstasy, methamphetamines, opiates, and oxycodone. Who is Served: Juveniles under the age of 18 Where: The Wood County Juvenile Court When: A parent can call the Court directly and ask to schedule an appointment for a urine screen. Desired Outcome: A parent can determine whether intervention is needed. Agencies: Wood County Juvenile Court Probation Department

Early Childhood Mental Health Project Type: Consultation, outreach education, public awareness. Who is Served: Child care providers, preschool teachers, parents. Where: On-site consultation; workshops at community locations. Educational Advocacy and Consulting Services Description of Program: The Ability Center’s educational advocates are a cross- departmental team that works with students, their families, other service providers and school districts to ensure that students with disabilities get the education and services they need and require. El Camino Hacia el Futuro (The Road Towards the Future) Mentors Program Type: Educational Who is Served: At risk Latino youth primarily, but no youth will be turned away. Where: Wood County When: Year round Desired Outcome: Eliminate high-risk use of alcohol, tobacco, and other illegal drugs among migrant/seasonal farm workers and rural Latino youths; reduce youth’s favorable attitudes toward alcohol, tobacco, and drug abuse after completion of a year long mentoring program; enhance their self-esteem and sense of personal control and power; enhance problem-solving and decision-making abilities, and increase level of knowledge about future opportunities by providing career exploration, information about education and skill requirements, and university tours to encourage youth to plan for college. Emergency Preparedness Description of Program: Planning with other agencies/organizations and local emergency responders for numerous potential public health threats. Threats may be natural disasters or acts of terrorism. They may be biological, nuclear, or chemical. Planning occurs on a local, regional, state, and national level. Environmental Health Description of program: The Environmental Division carries out restaurant inspections, monitoring of public swimming pools, nuisance complaints, private water quality testing, investigates complaints about sewage discharge, landfill inspections, etc. Who is Served: Wood County Where: At the site. E.Z. P.L.U.S. (Eric Zimmerman Positive Living Utilizing Skills) Type: Educational Who is Served: Seventh and eighth grade students living in Bowling Green Where: Bowling Green When: School year Desired Outcome: Eric was a Bowling Green Police Department officer who died in an off-duty car crash. This program is offered to Bowling Green Middle School and St. Aloysius seventh and eighth graders. There is a curriculum that builds on the lessons learned in D.A.R.E. and introduces the juvenile court system. The students can then join the E.Z.+ program if they sign a pledge stating they will remain drug, violence, and crime free. If they follow through on their pledge they can attend E.Z.+ sponsored events for free (dances, football games, movies, etc.) Agencies: Bowling Green Police Department

Family Planning Description of program: Family planning, contraception, screening & treatment for sexually transmitted diseases, HIV testing & counseling, pre-pregnancy counseling, education on sexual coercion & violence, pelvic & breast exams & patient education, screening for cervical cancer. Family Support Services Description of Program: Financial assistance to enable eligible families to care for their family at home. The program offers financial assistance for respite care, approved camps or recreation programs, adaptive equipment and incontinence products, home modification, special diets, counseling, training, and education for family members living in the household, and assistive technology, adapted toys, and repairs of adaptive equipment. FASTRAC (Fetal Alcohol Syndrome Education) Type: Education program in collaboration with other Wood County social service agencies. Who is Served: Wood County Junior high, high school, and BGSU students Where: Available in all Wood County school districts and student groups. Fitness is FUN! Type: Educational Who is Served: Children in Preschool – 5th grade. Where: Wood County Hospital or onsite When: Year round Desired Outcome: A fun, hands-on, 20 to 30 minute presentation that teaches children the importance of physical activity and how to stay healthy. Students will learn: benefits of exercise, different types of exercise, daily exercise requirements and fun and easy home exercises. Agencies: Wood County Hospital/Wellness Works

Goals for Gals Type: Education Who is Served: The workshop was developed with female probationers in mind. It is, however, open to all adolescent girls between 12 and 17 years old. Where: Wood County Juvenile Court When: The program is typically offered every summer Desired Outcome: The Goals for Gals workshop is held in four sessions over a period of two weeks. Topics covered include STDs and HIV, abusive relationships and dating violence. It also incorporates a Baby-Think-It-Over Doll to care for overnight. The aim of the workshop is to provide adolescent females with valuable information regarding decision making, goal setting, and risks in general. Housing Resource Center Description of program: The Ability Center's Housing Resource Center manages two programs. The Home Accessibility Program assists individuals living with disabilities to achieve their independent living goals through modifications to their homes. Immunizations Description of program: Childhood vaccines, adult vaccines, and travel vaccines are available. Desired Outcome: Prevention of vaccine preventable diseases Cost: Cost of vaccine varies. For state supplied vaccines, there is a $10 fee. No one is denied service for inability to pay. Incredible Years Parenting Program Type: Educational Who is Served: Parents of children 3 through 8 years. Where: At community locations. When: The 14 week program is offered several times per year, child care provided. Desired Outcome: Enhance parent-child relationships; increase parents’ awareness of behavior and development; improve parenting skills. Agency: Children’s Resource Center

Inclusive Communities Program Description: Community inclusion is a key goal of the Ability Center of Greater Toledo. Unfortunately, many community inclusion activities focus on the medical model, which is built around an external perception of disability and attempts to “fix” perceived problems. The key to true community success occurs when people initiate, nurture and keep natural relationships. Toward that end, the Ability Center works to ensure that communities include everyone irrespective of ability. Agency: The Ability Center of Greater Toledo.

Individual One-on-One Tobacco Cessation Counseling Program Description: Meet with a Certified Tobacco Treatment Specialist and receive personalized instruction and help on quitting tobacco use. This is a 2-appointment, 3- hour total program that is free to children under 18 years of age, or those who are still enrolled in a local high school. Agencies: Wood County Hospital/Wellness Works

Infectious Disease Control Description of program: Follow-up of reportable communicable diseases to assure that appropriate treatment has taken place and appropriate contact follow-up is achieved. Consultation is also available to schools, businesses, and the general public regarding specific communicable disease concerns including Blood borne Pathogens. Stay Tobacco-Free Athlete Mentoring Program (STAMP) Type: Peer to peer tobacco prevention program. These mentors then teach students in grades 5-7 within their own school system. Other teen organizations can be trained to educate younger teens within that organization or the community. When: Mentor trainings are in the fall with classes in grade 5-7 held throughout the school year. This American Cancer Program consists of four tobacco education sessions for grades 5-7 taught by high school students who are role models in their own schools. Usually classes are taught once per week for four weeks but other schedules are appropriate. Each session is taught using proficiency testing subjects. Desired Outcome: The intent of the program is to provide diversion from further behavior problems at school and ensure supervision in a structured environment for a child while suspended. Susan G. Komen Education Program Wood County Breast Health Description of program: Wood County Health and Wellness Center District Adult Clinic provides breast cancer awareness education/materials & performs clinical breast exams. Wood County Health and Wellness Center will arrange for a mammogram to be provided at Wood County Hospital at no charge to the patient if patient has an abnormal clinical breast exam or has not had a mammogram in the past 1 to 2 years. Who is Served: Uninsured and under-insured women and men between the ages of 40 to 64 (or under the age of 40 who are at a higher risk for breast cancer) in Wood County who have not had a clinical breast exam in the past 12 months or a mammogram in the past 1 to 2 years. Where: Wood County Health and Wellness Center coordinates the education/clinical breast exam/mammogram. Mammogram screening is performed at Wood County Hospital. When: Ongoing Desired Outcome: Improved mortality rates due to early detection and treatment of breast cancer. Tobacco Awareness Program (TAP) Type: This is a comprehensive, voluntary tobacco cessation program for high school students who want to quit using tobacco. This program provides teens with the steps, options, resources, education, motivation and support to stop using tobacco. Who is Served: High school age teens throughout Wood County. Where: Classes are available in most high schools in Wood County. Desired Outcome: Smoking cessation for teens. Agencies: Promedica St. Luke’s Hospital

Tobacco Education Group “Intervening with Teen Tobacco Users” (TEG) Type: This is a practical program for high school students who have violated a school’s tobacco policy or broken the law. It serves as a positive alternative to suspension or a diversion for the court. TEG teaches teens about the negative consequences of tobacco use and encourages them to adopt healthier lifestyles. Who is Served: Wood County teens Where: Promedica St. Luke’s Hospital When: Throughout the year, Saturday sessions Desired Outcome: Tobacco education. Agencies: Promedica St. Luke’s Hospital

Traditional One-on-One Mentoring Type: Children are matched with qualified adult volunteers, based upon similar interests, geographic location, and other factors. These mentors make a one-year commitment and agree to spend several hours a week with their Little Brother or Little Sister, enjoying activities and building strong and lasting relationships. Matches are to complete one community service project a year. Who is Served: Primarily children between the ages of 5-14 and part of a single parent family or demonstrate a need. Our children have to live within the service area and be willing to participate in the program. Where: Matches go on outings (park, movies, out to eat, etc.) or to each other’s homes as long as they are having fun. When: Typically a one-year commitment or longer. Desired Outcome: Children and youth matched to appropriate role models will become more competent, caring, and self-confident. Agencies: Big Brothers Big Sisters of Northwestern Ohio

Transition to Independence Process (TIP) Program Type: Mental Health and Substance Abuse Treatment, both outpatient and residential services; Please note that residential services are available for only a small number of clients Who is Served: Wood County residents ages 16-24 (18-24 for residential) Where: Behavioral Connections of Wood County—various locations When: Year Round Desired Outcome: Develop necessary skills in the areas of education, employment, living situation, personal effectiveness, and community involvement to foster transition to adulthood and independent living. Cost: All services are offered on a sliding fee scale; Fees depend on income; Will assist with Medicaid application if appropriate Agencies: Behavioral Connections of Wood County

Weapon Safety and Awareness Program Type: The Weapon Safety and Awareness Program is an educational option for dealing with offenses involving firearms, BB guns, Pellet guns, soft air guns or other weapons. Who is Served: Juveniles from 11 to 18 years of age. Where: The Wood County Juvenile Court. When: Scheduled on an as-needed basis Desired Outcome: The youth will leave with knowledge of proper handling of firearms, primary safety rules, consequences and dangers of misuse, and laws governing the use and possession of firearms. Women, Infants and Children Supplemental Nutrition Program (WIC) Description of program: Nutrition education, breastfeeding support and coupons redeemable for food at cooperating stores. Foods include fruits, vegetables, whole grains, milk, eggs, cereal, etc. Contact WIC. Who is Served: Pregnant women, nursing mothers and children up to age five; a resident of Wood County, income that falls within the WIC guideline, and at medical or nutritional risk according to WIC guidelines. Where: Wood County Health District and three satellite locations – North Baltimore, Northwood, and Perrysburg. When: Clinic hours 8:30 A.M. to 4:30 P.M. Monday through Friday, by appointment. Some evening hours are available. Satellite clinic services are available on scheduled days at Perrysburg, Northwood, and North Baltimore. Desired Outcome: Healthy pregnant women, newborns, and children Cost: No charge. Eligibility based on gross income and medical/nutritional risk factors. Agencies: Wood County Health District

Need a ride? Consider using the B.G. Transit, Bowling Green’s public transportation system. It operates within the corporation limits of the city of Bowling Green, and all B.G. Transit vehicles are handicap accessible. Hours of operation are Monday through Friday 6:00 a.m. until 8:00 p.m. and Saturdays 10:00 a.m. until 4:00 p.m. The general fare is $3.50, but with a B.G. Transit ID card, reduced fares are available for persons aged 65 years or older, children (4-13 years of age) and those with disabilities. Children under the age of 4 and personal care attendants ride for free.
